| 1 | Diabetic neuropathic pain:Physiopathology and treatment显示文摘Diabetic neuropathy is a common complication of both type 1 and type 2 diabetes,which affects over 90% of the diabetic patients.Although pain is one of the main symptoms of diabetic neuropathy,its pathophysiological mechanisms are not yet fully known.It is widely accepted that the toxic effects of hyperglycemia play an important role in the development of this complication,but several other hypotheses have been postulated.The management of diabetic neuropathic pain consists basically in excluding other causes of painful peripheral neuropathy,improving glycemic control as a prophylactic therapy and using medications to alleviate pain.First line drugs for pain relief include anticonvulsants,such as pregabalin and gabapentin and antidepressants,especial y those that act to inhibit the reuptake of serotonin and noradrenaline.In addition,there is experimental and clinical evidence that opioids can be helpful in pain control,mainly if associated with first line drugs.Other agents,including for topical application,such as capsaicin cream and lidocaine patches,have also been proposed to be useful as adjuvants in the control of diabetic neuropathic pain,but the clinical evidence is insufficient to support their use.In conclusion,a better understanding of the mechanisms underlying diabetic neuropathic pain will contribute to the search of new therapies,but also to the improvement of the guidelines to optimize pain control with the drugs currently available. | Anne K Schreiber Carina FM Nones Renata C Reis Juliana G Chichorro Joice M Cunha | 2015 | World Journal of Diabetes2015,6,3: | 34 |

| 6 | Heavy rockbursts due to longwall mining near protective pillars:A case study显示文摘Rockburst represents a very dangerous phenomenon in deep underground mining in unfavourable conditions such as great depth, high horizontal stress, proximity of important tectonic structures, and unmined pillars. The case study describes a recorded heavy rockburst in the Czech part of the Upper Silesian Coal Basin, which occurred during longwall mining near the protective pillar. The artificial dividing of geological blocks and creation of mining protective pillars(shaft pillars, crosscut pillars etc.) is a dangerous task in light of rockbursts occurring mainly due to overstressing of remaining pillars. A simple model of this situation is presented. Natural and mining conditions are analysed and presented in detail as well as registered seismicity during longwall mining in the area. Recorded rockbursts in the area of interest are described and their causes discussed. Many rockbursts near protective pillars were recorded in this mining region. Methodical instructions for rockburst prevention in proximity of protective pillars as well as for gates driving were devised based on the evaluation of rockburst causes. | 9 | Endoscopic and non-endoscopic approaches for the management of radiation-induced rectal bleeding显示文摘Pelvic radiation is a commonly utilized treatment for malignancy of the genitourinary and lower gastrointestinal tract. Radiation proctitis and the resultant clinical picture varies from asymptomatic to potentially life threatening. Similarly, treatment options also vary greatly, from medical therapy to surgical intervention. Commonly utilized medical therapy includes sucralfate enemas, antibiotics, 5-aminosalicylic acid derivatives, probiotics, antioxidants, short-chain fatty acids, formalin instillation and fractionated hyperbaric oxygen. More invasive treatments include endoscopic-based, focally ablative interventions such as dilation, heater and bipolar cautery, neodymium/yttrium aluminum garnet argon laser, radiofrequency ablation or argon plasma coagulation. Despite its relatively common frequency, there is a dearth of existing literature reporting headto-head comparisons of the various treatment options via a randomized controlled approach. The purpose of our review was to present the reader a consolidation of the existing evidence-based literature with the goal of highlighting the comparative effectiveness and risks of the various treatment approaches. Finally, we outline a pragmatic approach to the treatment of radiation proctitis. In light of the lack of randomized data, our goal is to pursue as least invasive an approach as possible, with escalation of care tailored to the severity of the patient's symptoms. For those cases that are clinicallyasymptomatic or only mildly symptomatic, observation or medical management can be considered. Once a patient fails such management or symptoms become more severe, invasive procedures such as endoscopically based focal ablation or surgical intervention can be considered. | 11 | Open label trial of granulocyte apheresis suggests therapeutic efficacy in chronically active steroid refractory ulcerative colitis显示文摘AIM:To study the efficacy, safety, and feasibility of a granulocyte adsorptive type apheresis system for the treatment of patients with chronically active ulcerative colitis despite standard therapy.METHODS: An open label multicenter study was carried out in 39 patients with active ulcerative colitis (CAI6-8) despite continuous use of steroids (a minimum total dose of 400 mg prednisone within the last 4 wk).Patients received a total of five aphereses using a granulocyte adsorptive technique (Adacolumn(R), Otsuka Pharmaceutical Europe, UK). Assessments at wk 6 and during follow-up until 4 mo comprised clinical (CAI) and endoscopic (EI) activity index, histology, quality of life(IBDQ), and laboratory tests.RESULTS: Thirty-five out of thirty-nine patients were qualified for intent-to-treat analysis. After the apheresis treatment at wk 6, 13/35 (37.1%) patients achieved clinical remission and 10/35 (28.6%) patients had endoscopic remission (CAI<4, EI<4). Quality of life (IBDQ) increased significantly (24 points, P<0.01)at wk 6. Apheresis could be performed in all but one patient. Aphereses were well tolerated, only one patient experienced anemia.CONCLUSION: In patients with steroid refractory ulcerative colitis, five aphereses with a granulocyte/monocyte depleting filter show potential short-term efficacy. METHODS: Three strains of bifi dobacteria, 4 strains of lactobacilli, and E. coli nissle were sonicated and centri- fuged in order to divide them into cell extract and cell debris. PBMNC were separated by density gradient and incubated for 36 h with either the cell debris or the cell extract of single strains of probiotic bacteria in doses from 102 to 108 CFU/mL. Cell supernatants were taken and interleukin (IL)-10, IL-1β, and tumor necosis factor (TNF)-α were determined by ELISA. RESULTS: Depending on the species super-family, the strains had different stimulation patterns. Except for both L. casei strains, the cell extract of bifidobacteriaand lactobacilli had less stimulating capacity than cell debris, whereas the cell extract of E. coli nissle had simi- lar stimulating properties to that of the cell debris of the strain and significantly more stimulating capacity than that of bifi dobacteria and lactobacilli. The cell debris of bifi dobacteria stimulated more cytokine release than the cell debris of lactobacilli. The cell debris of lactobacilli did not have a stimulating capacity when lower concentra- tions were used. Neither cell extraction nor cell debris had an inhibitory effect on the production of the tested cytokines by stimulated PBMNC. CONCLUSION: The incubation of probiotic strains, which have been used in clinical trials for inflammatory diseases, with immunocompetent cells leads to different species specifi c reactions. High IL-10 response to cell de- bris of bifi dobacteria and E. coli nissle can be found. | 15 | Domestication and crop evolution of wheat andbarley: Genes, genomics, and future directions显示文摘Wheat and barley are two of the founder crops of the agricultural revolution that took place 10,000 years ago in the Fertile Crescent and both crops remain among the world's most important crops. Domestication of these crops from their wild ancestors required the evolution of traits useful to humans, rather than survival in their natural environment. Of these traits, grain retention and threshability, yield improvement, changes to photoperiod sensitivity and nutritional value are most pronounced between wild and domesticated forms. Knowledge about the geographical origins of these crops and the genes responsible for domestication traits largely pre-dates the era of nextgeneration sequencing, although sequencing will lead to new insights. Molecular markers were initially used to calculate distance(relatedness), genetic diversity and to generate genetic maps which were useful in cloning major domestication genes. Both crops are characterized by large,complex genomes which were long thought to be beyond the scope of whole-genome sequencing. However, advances in sequencing technologies have improved the state of genomic resources for both wheat and barley. | 18 | Effects of torpedo blasting on rockburst prevention during deep coal seam mining in the Upper Silesian Coal Basin显示文摘In the Upper Silesian Coal Basin(USCB),coal seams are exploited under progressively more difficult geological and mining conditions(greater depth,higher horizontal stress,more frequent occurrence of competent rock layers,etc.).Mining depth,dislocations and mining remnants in coal seams are the most important factors responsible for the occurrence of rockburst hazards.Longwall mining next to the mining edges of neighbouring coal seams is particularly disadvantageous.The levels of rockburst hazards are minimised via the use of rockburst prevention methods.One active prevention method is torpedo blasting in roof rocks.Torpedo blastings are performed in order to decrease local stress concentrations in rock masses and to fracture the roof rocks to prevent or minimise the impact of high-energy tremors on excavations.The estimation of the effectiveness of torpedo blasting is particularly important when mining is under difficult geological and mining conditions.Torpedo blasting is the main form of active rockburst prevention in the assigned colliery in the Polish part of the USCB.The effectiveness of blasting can be estimated using the seismic effect method,in which the seismic monitoring data and the mass of explosives are taken into consideration.The seismic effect method was developed in the Czech Republic and is always being used in collieries in the Czech part of the coal basin.Now,this method has been widely adopted for our selected colliery in the Polish part of the coal basin.The effectiveness of torpedo blastings in the faces and galleries of the assigned longwall in coal seam 506 has been estimated.The results show that the effectiveness of torpedo blastings for this longwall was significant in light of the seismic effect method,which corresponds to the in situ observations.The seismic effect method is regularly applied to estimating the blasting effectiveness in the selected colliery. | L.
