| 2 | Expanding etiology of progressive familial intrahepatic cholestasis显示文摘BACKGROUND Progressive familial intrahepatic cholestasis(PFIC)refers to a disparate group of autosomal recessive disorders that are linked by the inability to appropriately form and excrete bile from hepatocytes,resulting in a hepatocellular form of cholestasis.While the diagnosis of such disorders had historically been based on pattern recognition of unremitting cholestasis without other identified molecular or anatomic cause,recent scientific advancements have uncovered multiple specific responsible proteins.The variety of identified defects has resulted in an ever-broadening phenotypic spectrum,ranging from traditional benign recurrent jaundice to progressive cholestasis and end-stage liver disease.AIM To review current data on defects in bile acid homeostasis,explore the expanding knowledge base of genetic based diseases in this field,and report disease characteristics and management.METHODS We conducted a systemic review according to PRISMA guidelines.We performed a Medline/PubMed search in February-March 2019 for relevant articles relating to the understanding,diagnosis,and management of bile acid homeostasis with a focus on the family of diseases collectively known as PFIC.English only articles were accessed in full.The manual search included references of retrieved articles.We extracted data on disease characteristics,associations with other diseases,and treatment.Data was summarized and presented in text,figure,and table format.RESULTS Genetic-based liver disease resulting in the inability to properly form and secrete bile constitute an important cause of morbidity and mortality in children and increasingly in adults.A growing number of PFIC have been described based on an expanded understanding of biliary transport mechanism defects and the development of a common phenotype.CONCLUSION We present a summary of current advances made in a number of areas relevant to both the classically described FIC1(ATP8B1),BSEP(ABCB11),and MDR3(ABCB4)transporter deficiencies,as well as more recently described gene mutations--TJP2(TJP2),FXR(NR1H4),MYO5B(MYO5B),and others which expand the etiology and understanding of PFIC-related cholestatic diseases and bile transport. | Sarah AF Henkel Judy H Squires Mary Ayers Armando Ganoza Patrick Mckiernan James E Squires | 2019 | World Journal of Hepatology2019，11，5： | 13 |

| 4 | Impact of community-based interventions on HIV knowledge, attitudes, and transmission显示文摘In 2012,an estimated 35.3 million people lived with HIV,while approximately two million new HIV infections were reported.Community-based interventions(CBIs)for the prevention and control of HIV allow increased access and ease availability of medical care to population at risk,or already infected with,HIV.This paper evaluates the impact of CBIs on HIV knowledge,attitudes,and transmission.We included 39 studies on educational activities,counseling sessions,home visits,mentoring,women’s groups,peer leadership,and street outreach activities in community settings that aimed to increase awareness on HIV/AIDS risk factors and ensure treatment adherence.Our review findings suggest that CBIs to increase HIV awareness and risk reduction are effective in improving knowledge,attitudes,and practice outcomes as evidenced by the increased knowledge scores for HIV/AIDS(SMD:0.66,95%CI:0.25,1.07),protected sexual encounters(RR:1.19,95%CI:1.13,1.25),condom use(SMD:0.96,95%CI:0.03,1.58),and decreased frequency of sexual intercourse(RR:0.76,95%CI:0.61,0.96).Analysis shows that CBIs did not have any significant impact on scores for self-efficacy and communication.We found very limited evidence on community-based management for HIV infected population and prevention of mother-to-child transmission(MTCT)for HIV-infected pregnant women.Qualitative synthesis suggests that establishment of community support at the onset of HIV prevention programs leads to community acceptance and engagement.School-based delivery of HIV prevention education and contraceptive distribution have also been advocated as potential strategies to target high-risk youth group.Future studies should focus on evaluating the effectiveness of community delivery platforms for prevention of MTCT,and various emerging models of care to improve morbidity and mortality outcomes. | Rehana A Salam Sarah Haroon Hashim H Ahmed Jai K Das Zulfiqar A Bhutta | 2014 | Infectious Diseases of Poverty2014，3，1： | 3 |

| 10 | Cyanidin 3-glucoside modulated cell cycle progression in liver precancerous lesion,in vivo study显示文摘BACKGROUND Cyanidin-3-O-glucoside(cyan)exhibits antioxidant and anticancer properties.The cell cycle proteins and antimitotic drugs might be promising therapeutic targets in hepatocellular carcinoma.AIM To investigate the effect of cyan administration on cell cycle in hepatic precancerous lesion(PCL)induced by diethylnitrosamine/2-acetylaminofluorene(DEN/2-AAF)in Wistar rats.METHODS In vivo,DEN/2-AAF-induced hepatic PCL,rats were treated with three doses of cyan(10,15,and 20 mg/kg/d,for four consecutive days per week for 16 wk).Blood and liver tissue samples were collected for measurement of the followings;alpha fetoprotein(AFP)liver function and RNA panel differential expression was evaluated via real time polymerase chain reaction.Histopathological examination of liver sections stained with H&E and immunohistochemical study using glutathione S-transferase placental(GSTP)and proliferating cell nuclear antigen(PCNA)antibodies were assessed.RESULTS Cyan administration mitigated the effect of DEN/2-AFF induced PCL,decreased AFP levels,and improved liver function.Remarkably,treatment with cyan dose dependently decreased the long non-coding RNA MALAT1 and tubulin gamma 1 mRNA expressions and increased the levels of miR-125b,all of which are involved in cell cycle and mitotic spindle assembly.Of note,cyan decreased GSTP foci percent area and PCNA positively stained nuclei.CONCLUSION Our results indicated that cyan could be used as a potential therapeutic agent to inhibit liver carcinogenesis in rat model via modulation of cell cycle. | Marwa Matboli Amany H Hasanin Reham Hussein Sarah El-Nakeep Eman K Habib Rawan Ellackany Lobna A Saleh | 2021 | World Journal of Gastroenterology2021，27，14： | 1 |
