Acute calculous cholecystitis: Review of current best practices显示文摘Acute calculous cholecystitis(ACC) is the most frequent complication of cholelithiasis and represents one-third of all surgical emergency hospital admissions, many aspects of the disease are still a matter of debate. Knowledge of the current evidence may allow the surgical team to develop practical bedside decision-making strategies, aiming at a less demanding procedure and lower frequency of complications. In this regard, recommendations on the diagnosis supported by specific criteria and severity scores are being implemented, to prioritize patients eligible for urgency surgery. Laparoscopic cholecystectomy is the best treatment for ACC and the procedure should ideally be performed within 72h. Early surgery is associated with better results in comparison to delayed surgery. In addition, when to suspect associated common bile duct stones and how to treat them when found are still debated. The antimicrobial agents are indicated for high-risk patients and especially in the presence of gallbladder necrosis. The use of broad-spectrum antibiotics and in some cases with antifungal agents is related to better prognosis. Moreover, an emerging strategy of not converting to open, a difficult laparoscopic cholecystectomy and performing a subtotal cholecystectomy is recommended by adept surgical teams. Some authors support the use of percutaneous cholecystostomy as an alternative emergency treatment for acute Cholecystitis for patients with severe comorbidities. | Carlos Augusto Gomes Cleber Soares Junior Salomone Di Saveiro Massimo Sartelli Michel Denis Kelly Camila Couto Gomes Felipe Couto Gomes Lívia Dornellas Correa Camila Brandao Alves Samuel de Fádel Guimaraes | 2017 | World Journal of Gastrointestinal Surgery2017,9,5: | 18
Multi-task Joint Sparse Representation Classification Based on Fisher Discrimination Dictionary Learning显示文摘Recently, sparse representation classification(SRC)and fisher discrimination dictionary learning(FDDL)methods have emerged as important methods for vehicle classification. In this paper, inspired by recent breakthroughs of discrimination dictionary learning approach and multi-task joint covariate selection,we focus on the problem of vehicle classification in real-world applications by formulating it as a multi-task joint sparse representation model based on fisher discrimination dictionary learning to merge the strength of multiple features among multiple sensors. To improve the classification accuracy in complex scenes,we develop a new method, called multi-task joint sparse representation classification based on fisher discrimination dictionary learning, for vehicle classification. In our proposed method, the acoustic and seismic sensor data sets are captured to measure the same physical event simultaneously by multiple heterogeneous sensors and the multi-dimensional frequency spectrum features of sensors data are extracted using Mel frequency cepstral coefficients(MFCC).Moreover,we extend our model to handle sparse environmental noise. We experimentally demonstrate the benefits of joint information fusion based on fisher discrimination dictionary learning from different sensors in vehicle classification tasks. | Rui Wang Miaomiao Shen Yanping Li Samuel Gomes | 2018 | Computers, Materials & Continua2018,,10: | 6

Coronal sulcus-based ventral mucosal flap to help penile coverage in severe concealed penis显示文摘Introduction There are many techniques to treat congenital concealed penis(CP).Skin resurfacing is the most difficult step in severe cases. We aim to show medium-term results of coronal sulcus-based triangular ventral mucosal flap(CBVMF)as a treatment of prepubertal severe CP,a recently reported technique. We aim to determine whether results are durable and if the technique is associated with persistent mucosal redundancy or with a permanent unequal penile color pattern. Methods CP cases reconstructed with CBVMF were reviewed.Preoperative complaints, degree of motivation of the child/parent to surgery, satisfaction of parent/child with results, and surgical complications were described. Results Seven patients(6 months to 6 years old)were treated with CBVMP.Two patients showed megaprepuce and another was submitted to a limited postectomy 3 years before. One family was not fully satisfied(expected“bigger penis”),but acknowledged that the penis was now well exposed. No child talked about the problem preoperatively, but all of the boys were fully satisfied with the results of the surgery and verbalized this in the interviews.Flap edema resolved after 3 months in all but one patient. The flaps assumed the color of penile skin in the medium term. Conclusions CBMVP results were satisfactory.Serious complications did not occur.Flap edema does not persist in the medium term, and redundancy was not a problem. The color of the flap tended to evolve into a pattern similar to the penile skin. | Lisieux Jesus Flavia Amaro Jamel Fernanda Gomes Talia Dias Ribeiro Samuel Dekermacher | 2021 | World Journal of Pediatric Surgery2021,4,3: | 0
Origami-Based Design for 4D Printing of 3D Support-Free Hollow Structures显示文摘The integration of additive manufacturing(AM)in design and engineering has prompted a wide spectrum of research efforts, involving topologically optimized solid/lattice structures, multimaterial structures, bioinspired organic structures, and multiscale structures,to name a few. However, except for obvious cases, very little attention has been given to the design and printing of more complex three-dimensional(3D)hollow structures or folded/creased structures. One of the main reasons is that such complex open or closed 3D cavities and regular/freeform folds generally lead to printing difficulties from support-structure-related issues. To address this barrier, this paper aims to investigate four-dimensional(4D)printing as well as origami-based design as an original research direction to design and build 3D support-free hollow structures. This work consists of describing the rough 3D hollow structures in terms of two-dimensional(2D)printed origami precursor layouts without any support structure.Such origami-based definitions are then embodied with folding functions that can be actuated and fulfilled by 3D printed smart materials. The desired 3D shape is then built once an external stimulus is applied to the active materials, therefore ensuring the transformation of the 2D origami layout to 3D structures. To demonstrate the relevance of the proposal, some illustrative cases are introduced. | Bingcong Jian Frederic Demoly Yicha Zhang H.Jerry Qi Jean-Claude Andre Samuel Gomes | 2022 | Engineering2022,8,5: | 0
Kaphta:Text mining web tool to extract information on the anticancer activity of polyphenols显示文摘In this paper,we describe the application of Kaphta architecture,a resource for text mining of the anticancer activity of polyphenols. The anticancer activity of these compounds against different types of cancer has been widely reported in the literature and they are one of the most promising molecules for the development of anticancer drugs. The architecture, which comprises four sequential and well-defined steps, uses a hybrid approach composed of a dictionary, rules and machine learning to identify abstracts containing sentences with associations between polyphenol, cancer and gene entities. The application of the architecture on 23826 PubMed abstracts generated a knowledge base of indexed abstracts with 172169 sentences containing,polyphenol-cancer and polyphenol-gene associations.A Web tool was implemented that allowed the user to search for information on 2006 polyphenols,240 cancers and 3121 genes entities, and 11750 polyphenol-cancer and 9160 polyphenol-gene associations indexed in the knowledge base.A ranking algorithm calculates scores for each indexed abstract considering the number and type of sentences with entities and rules recognized.A test with users demonstrated that the visualization resources on the web tool contributes to the understanding of the association between polyphenols, genes and cancers,in comparison with the PubMed Tool. The Kaphta architecture and web tool permits to extract knowledge on the anticancer activity of polyphenols and can thus contribute to the exploration of these molecules in the development of anticancer therapies. | Ramon Gustavo Teodoro Marques da Silva Samuel Lucas Santos Gomes Paulo Muniz deÁvila Gustavo Joséda Silva Ana Lucia Fachin Edilson Carlos Caritá Mozart Marins | 2022 | Journal of Polyphenols2022,4,2: | 0
