| 1 | Subclinical abnormal glucose tolerance is a predictor of death in liver cirrhosis显示文摘AIM:To determine if subclinical abnormal glucose tolerance(SAGT)has influence on survival of non-diabetic patients with liver cirrhosis.METHODS:In total,100 patients with compensatedliver cirrhosis and normal fasting plasma glucose were included.Fasting plasma insulin(FPI)levels were measured,and oral glucose tolerance test(OGTT)was performed.According to OGTT results two groups of patients were formed:those with normal glucose tolerance(NGT)and those with SAGT.Patients were followed every three months.The mean follow-up was932 d(range of 180-1925).Survival was analyzed by the Kaplan-Meyer method,and predictive factors of death were analyzed using the Cox proportional hazard regression model.RESULTS:Of the included patients,30 showed NGT and70 SAGT.Groups were significantly different only in age,INR,FPI and HOMA2-IR.Patients with SAGT showed lower 5-year cumulated survival than NGT patients(31.7%vs 71.6%,P=0.02).Differences in survival were significant only after 3 years of follow-up.SAGT,Child-Pugh B,and high Child-Pugh and Model for EndStage Liver Disease(MELD)scores were independent predictors of death.The causes of death in 90.3%of cases were due to complications related to liver disease.CONCLUSION:SAGT was associated with lower survival.SAGT,Child-Pugh B,and high Child-Pugh and MELD scores were independent negative predictors of survival. | Diego García-Compeán Joel Omar Jáquez-Quintana Fernando Javier Lavalle-González José Alberto González-González Linda Elsa Mu?oz-Espinosa Jesús Zacarías Villarreal-PérezEndocrinology Service and Department of Internal Medicine University Hospital 'Dr. José E. | 2 | Computer Aided Simulation on Precision Cold Forging of Universal Joint Cross显示文摘The 3D FEM numerical simulation on multi-action precision cold forging technology of universal joint cross and differential spider is done in this article using DEFORM Software, a commercial computer aided engineering software specializing in forming and heat treatment simulation technology, and suitable for cold, warm and hot forging process. The material flow properties, the dynamic variation of stress and strain in the process of deformation and the load-stroke curve have also been achieved. A good consistency is exhibited between simulation results and practical data. Based on the DEFORM simulation results, the optimized procedure has been found for forging a universal joint cross. What should be emphasized here is that a better understanding of practical forging characters and the environmental factors can greatly improve the simulation accuracy thus make the simulation results more reliable. | H. | 3 | Simulation of moving boundary of the reaction shaft in a flash smelting furnace显示文摘As to solve the online monitoring of the inner temperatur e and freezing profile of the reaction shaft of flash smelting furnace, simulation of the wall in the reaction shaft in a flash smelting furnace was made through numerical computation. The computational results are very near the data got in s ite. The error of the moving boundary is approximately 3%, and that of the tempe rature is less than 5%. It is proved that the simulation software is applicable for practice to monitor the temperature and moving boundary inside the hearth on line. | 4 | Quality control measures for lowering the seroconversion rate of hemodialysis patients with hepatitis B or C virus显示文摘BACKGROUND: Hemodialysis (HD) patients are at high risk of infection by hepatitis B virus (HBV) or hepatitis C virus (HCV). The present study was designed to determine the impact of quality control measures on the prevention of transmission of blood-borne viruses. METHODS: A total of 6182 adult maintenance HD patients from all HD units in Zhejiang Province were recruited on January 1, 2007. The baseline demographic and clinical characteristics were recorded and all patients were followed up until death or survival at 4 years later. The Quality Control Standards of Hemodialysis were gradually implemented in HD units. The HBV or HCV seroconversion rates of the recruited patients were calculated and compared every year during the observation period. RESULTS: The prevalence of HBV was 8.3% at the beginning of the study, and 6.6% for HCV. With the implementation of the HD quality control measures, the HBV seroconversion rate tended to decrease year by year (χ 2 =6.620, P=0.085), and the HCV seroconversion rate decreased significantly (χ 2 =10.41, P=0.015). Compared with the data in 2007, the HBV seroconversion rate (χ 2 =4.204, P=0.040, relative risk ratio 0.393, 95% CI 0.156-0.991) and the HCV seroconversion rate (χ 2 =7.373, P=0.007, relative risk ratio 0.386, 95% CI 0.189-0.787) decreased significantly in 2010.
