1Neoadjuvant radiotherapy for rectal cancer management显示文摘Thirty per cent of all colorectal tumours develop in the rectum.The location of the rectum within the bony pelvis and its proximity to vital structures presents significant therapeutic challenges when considering neoadjuvant options and surgical interventions.Most patients with early rectal cancer can be adequately managed by surgery alone.However,a significant proportion of patients with rectal cancer present with locally advanced disease and will potentially benefit from down staging prior to surgery.Neoadjuvant therapy involves a variety of options including radiotherapy,chemotherapy used alone or in combination.Neoadjuvant radiotherapy in rectal cancer has been shown to be effective in reducing tumour burden in advance of curative surgery.The gold standard surgical rectal cancer management aims to achieve surgical removal of the tumour and all draining lymph nodes,within an intact mesorectal package,in order to minimise local recurrence.It is critically important that all rectal cancer cases are discussed at a multidisciplinary meeting represented by all relevant specialties.Pre-operative staging including CT thorax,abdomen,pelvis to assess for distal disease and magnetic resonance imaging to assess local involvement is essential.Staging radiology and MDT discussion are integral in identifying patients who require neoadjuvant radiotherapy.While Neoadjuvant radiotherapy is potentially beneficial it may also result in morbidity and thus should be reserved for those patients who are at a high risk of local failure,which includes patients with nodal involvement,extramural venous invasion and threatened circumferential margin.The aim of this review is to discuss the role of neoadjuvant radiotherapy in the management of rectal cancer.Gerard Feeney Rishabh Sehgal Margaret Sheehan Aisling Hogan Mark Regan Myles Joyce Michael Kerin 2019World Journal of Gastroenterology2019,25,33:28
2Historical development of mesenteric anatomy provides a universally applicable anatomic paradigm for complete/total mesocolic excision显示文摘Although total mesorectal excision has now become the‘gold standard’for the surgical management of rectal cancer,this is not so for colon cancer.Recent data,provided by Hohenberger and West et al.and others,have demonstrated excellent oncological outcomes when mesenterectomy is extensive(as is implicit in the concept of a‘high tie’)and the mesenteric package not violated.Such studies highlight the importance of understanding the basics of the mesenteric organ(including the small intestinal mesentery,mesocolon,mesosigmoid and mesorectum)and of abiding to principles of planar surgery.In this review,we first offer classic descriptions of the mesocolon and then detail contemporary thinking.In so doing,we provide an anatomical basis for safe and effective complete mesocolic excision(CME)in the management of colon cancer.Finally we list opportunities associated with the new anatomical paradigm,demonstrating benefits across multiple disciplines.Perhaps most importantly,we feel that a crystallized view of mesenteric anatomy will overcome factors that have hindered the general uptake of CME.Rishabh Sehgal J.Calvin Coffey 2014Gastroenterology Report2014,2,4:5
3Optimisation of laser welding parameters for welding of P92 material using Taguchi based grey relational analysis显示文摘Creep strength enhanced ferritic(CSEF) steels are used in advanced power plant systems for high temperature applications. P92(Cr–W–Mo–V)steel, classified under CSEF steels, is a candidate material for piping, tubing, etc., in ultra-super critical and advanced ultra-super critical boiler applications. In the present work, laser welding process has been optimised for P92 material by using Taguchi based grey relational analysis(GRA).Bead on plate(BOP) trials were carried out using a 3.5 k W diffusion cooled slab CO_2 laser by varying laser power, welding speed and focal position. The optimum parameters have been derived by considering the responses such as depth of penetration, weld width and heat affected zone(HAZ) width. Analysis of variance(ANOVA) has been used to analyse the effect of different parameters on the responses. Based on ANOVA, laser power of 3 k W, welding speed of 1 m/min and focal plane at-4 mm have evolved as optimised set of parameters. The responses of the optimised parameters obtained using the GRA have been verified experimentally and found to closely correlate with the predicted value.? 2016 China Ordnance Society. 5Impact of global climate change on stream low flows:A case study of the great Miami river watershed, Ohio, USA显示文摘Climate change will profoundly affect hydrological processes at various temporal and spatial scales.This study is focused on assessing the alteration of water resources availability and low flows frequencies driven by changing climates in different time periods of the 21st century.This study evaluates the adaptability of prevailing Global Circulation Models(GCMs)on a particular watershed through streamflow regimes.This analysis was conducted in the Great Miami River Watershed,Ohio by analyzing historical and future simulated streamflow using 10 climate model outputs and the Soil and Water Assessment Tool(SWAT).The climate change scenarios,consisting of ten downscaled Coupled Model Intercomparision Project Phase 5(CMIP5)climate models in combination with two Representative Concentration Pathways(RCP 4.5 and RCP 8.5)were selected based on the correlation between observed records and model outputs.Streamflow for three future periods,2016-2043,2044-2071 and 2072-2099,were independently analyzed and compared with the baseline period(1988-2015).Results from the average of ten models projected that 7-day low flows in the watershed would increase by 19%in the 21st century under both RCPs.This trend was also consistent for both hydrological(7Q10,1Q10)and biological low flow statistics(4B3,1B3).Similarly,average annual flow and monthly flows would also increase in future periods,especially in the summer.The flows simulated by SWAT in response to the majority of climate model projections showed a consistent increase in low flow patterns.However,the flow estimates using the Max-Planck-Institute Earth System Model(MPI-ESM-LR)climate output resulted in the biological based low flows(4B3,1B3)decreasing by 22.5%and 33.4%under RCP 4.5 and 56.9%and 63.7%under RCP 8.5,respectively,in the future when compared to the baseline period.Regardless,the low flow ensemble from the 10 climate models for the 21st century seemed to be slightly higher than that of historical low flows.Sabin Shrestha Suresh Sharma Rishabh Gupta Rabin Bhattarai 2019International Journal of Agricultural and Biological Engineering2019,12,1:1

9Assessment of Above- and Below-Ground Carbon Pools in a Tropical Dry Deciduous Forest Ecosystem of Bhopal, India显示文摘This study estimated 18.35 Mg C/ha in standing biomass of natural forest and 15 Mg C/ha in Hardwickia binata Roxb. plantation in a tropical dry deciduous forest located in the capital city of Madhya Pradesh. The study area of Indian Institute of Forest Management(IIFM), Bhopal,resembled a degraded dry scrubland in 1988 and for over about three decades, the degraded forest recovered remarkably, and ecological processes evolved favorably with canopy cover reaching over 60% in some patches and about 50% in general at most part of the campus. The study was conducted in 18 randomly laid plots in natural forest and over one-acre(0.405-ha) plantation area of Hardwickia binata for assessing the above-ground biomass, below-ground biomass and subsequent carbon content. The lower-diameter classes accounted for the maximum above-ground biomass, basal area and tree density. The forest is predominantly occupied by Leucaena leucocephala, an exotic tree species which showed higher standing biomass carbon storage of 3.79 Mg C/ha followed by Holoptelea integrifolia(2.11 Mg C/ha), Azadirachta indica(1.29 Mg C/ha),Gardenia latifolia(1.26 Mg C/ha) and Lannea coromandelica(1.24 Mg C/ha) besides Hardwickia binata plantation(15 Mg C/ha). 10Priming the tooth surface with chlorhexidine and antibacterial activity of resin cement显示文摘AIM: To evaluate the effect of priming the tooth surface with 2% chlorhexidine gluconate on antibacteria activity of resin cement. METHODS: Ten patients in whom a single missing tooth was present on both the right and left side in the upper or lower arch were selected. Two fixed partia dentures(FPDs) in each patient on the right and left side were planned. Each FPD was assigned either to the control or test group. In the control group, FPD was luted with resin cement and in the test group, the tooth surface was primed with 2% chlorhexidine gluconate before luting with resin cement. Bacteriological samples were collected at base line level, as the patient came to the outpatient department before the start of any treatment, 5 wk prior to cementation of FPD and at 13 wk(8wk after final cementation). Microbiological processing of all samples was done and the results were statistically analyzed. RESULTS: In the test group, a predominance of aerobic/facultative gram positive cocci rod was seen which indicates a healthy periodontal site, whereas in thecontrol group, a predominance of anaerobic gram negative rods was present which indicates an unhealthy periodontal condition. This is evident by the fact that the anaerobic bacteria percentage in the control sample is 57% and 15% in the test sample after 13 wk, whereas the aerobic/facultative bacteria percentage is 43% in the control sample and 85% in the test sample after 13 wk. The percentage of gram negative bacteria in the control sample is 61% and in the test sample is 20% after 13 wk, whereas the percentage of gram positive bacteria in the control sample is 39% and in the test sample is 80% after 13 wk. The shift from anaerobic gram negative bacteria to aerobic gram positive bacteria is clearly seen from the control to test sample after 13 wk. 13Is Regular Probiotic Practice Safe for Management of Sepsis?显示文摘For decades,the gut has been thought to play an important role in sepsis pathogenesis.Sepsis is a serious life-threatening,chronic condition of an infection caused by dysregulated host immune response in most of the intensive care unit patients.Probiotics have dual roles in polymicrobial sepsis i.e.probiotics may induce sepsis in many cases and may prevent its prognosis in many cases.Experimental evidence from both pre-clinical and clinical studies have demonstrated that probiotic therapy ameliorates various inflammatory mediators such as tumor necrosis factor,interleukin-10(IL-10),IL-6,etc.,in septicemia.In addition,probiotic use was also found to reduce the severity of pathological conditions associated with irritable bowel disorder and prevent development of endocarditis in septicemia.On contrary,probiotic therapy in neonatal and athymic adult mice fail to provide any beneficial effects on mortality and sepsis-induced inflammation.Importantly,in few clinical trials probiotic use was found to aggravate sepsis by promoting inflammatory cascade rather than suppressing it.This review discusses various studies regarding the beneficial or harmful effects associated with probiotic therapy in sepsis.Rishabh Kumar Alok Shiomurti Tripathi Nidhi Sharma Gaaminepreet Singh Lucy Mohapatra 2022Chinese Journal of Integrative Medicine2022,28,2:0
14Amebic liver abscess: An update显示文摘Amebic liver abscess(ALA)is still a common problem in the tropical world,where it affects over three-quarters of patients with liver abscess.It is caused by an anaerobic protozoan Entamoeba hystolytica,which primarily colonises the cecum.It is a non-suppurative infection of the liver consisting primarily of dead hepatocytes and cellular debris.People of the male gender,during their reproductive years,are most prone to ALA,and this appears to be due to a poorly mounted immune response linked to serum testosterone levels.ALA is more common in the right lobe of the liver,is strongly associated with alcohol consumption,and can heal without the need for drainage.While majority of ALA patients have an uncomplicated course,a number of complications have been described,including rupture into abdomino-thoracic structures,biliary fistula,vascular thrombosis,bilio-vascular compression,and secondary bacterial infection.Based on clinico-radiological findings,a classification system for ALA has emerged recently,which can assist clinicians in making treatment decisions.Recent research has revealed the role of venous thrombosis-related ischemia in the severity of ALA.Recent years have seen the development and refinement of newer molecular diagnostic techniques that can greatly aid in overcoming the diagnostic challenge in endemic area where serology-based tests have limited accuracy.Metronidazole has been the drug of choice for ALA patients for many years.However,concerns over the resistance and adverse effects necessitate the creation of new,safe,and potent antiamebic medications.Although the indication of the drainage of uncomplicated ALA has become more clear,high-quality randomised trials are still necessary for robust conclusions.Percutaneous drainage appears to be a viable option for patients with ruptured ALA and diffuse peritonitis,for whom surgery represents a significant risk of mortality.With regard to all of the aforementioned issues,this article intends to present an updated review of ALA.Ramesh Kumar Rishabh Patel Rajeev Nayan Priyadarshi Ruchika Narayan Tanmoy Maji Utpal Anand Jinit R Soni 2024World Journal of Hepatology2024,16,3:0
15A Comparative Study of Artificial Neural Network and Response Surface Methodology for Optimization of Friction Welding of Incoloy 800 H显示文摘This article deals with the optimization of process parameters for friction welding of Incoloy 800 H rod and compares the results obtained by response surface methodology(RSM) and artificial neural network(ANN).The experiments were carried out on the basis of a five-level,four-variable central composite design.The output parameters were the tensile strength and burn-off length(BOL).They were considered as a function of four independent input variables,namely heating pressure(HP),heating time,upsetting pressure(UP),and upsetting time.The RSM results showed that the quadratic polynomial model depicted the interconnection between individual element and response.For optimizing the process parameters,ANN analysis was used,and the optimal configuration of the ANN model was found to be 4–9–2.For modeling aspect,a requisite trained multilayer perceptron neural network was rooted,and a quick propagation training algorithm was used to train ANN.The purpose of optimization was to decide the maximum tensile strength and minimum burn-off length of the welded joint which was done by varying the friction welding process variables.The order of importance of input parameters for friction welding of Incoloy 800 H was HP [ UP [ N [ BOL.After predicting the model using RSM and ANN,a comparison was made for predicting the effectiveness of two methodologies.By analyzing the results,it was observed that as compared to RSM,ANN model was more specific.K.Anand Rishabh Shrivastava K.Tamilmannan P.Sathiya 2015Acta Metallurgica Sinica(English Letters)2015,28,7:0
