| 1 | Basic Soil Productivity of Spring Maize in Black Soil Under Long-Term Fertilization Based on DSSAT Model显示文摘Increasing basic farmland soil productivity has significance in reducing fertilizer application and maintaining high yield of crops. In this study, we defined that the basic soil productivity(BSP) is the production capacity of a farmland soil with its own physical and chemical properties for a specific crop season under local environment and field management. Based on 22-yr(1990-2011) long-term experimental data on black soil(Typic hapludoll) in Gongzhuling, Jilin Province, Northeast China, the decision support system for an agro-technology transfer(DSSAT)-CERES-Maize model was applied to simulate the yield by BSP of spring maize(Zea mays L.) to examine the effects of long-term fertilization on changes of BSP and explore the mechanisms of BSP increasing. Five treatments were examined:(1) no-fertilization control(control);(2) chemical nitrogen, phosphorus, and potassium(NPK);(3) NPK plus farmyard manure(NPKM);(4) 1.5 time of NPKM(1.5NPKM) and(5) NPK plus straw(NPKS). Results showed that after 22-yr fertilization, the yield by BSP of spring maize significantly increased 78.0, 101.2, and 69.4% under the NPKM, 1.5NPKM and NPKS, respectively, compared to the initial value(in 1992), but not significant under NPK(26.9% increase) and the control(8.9% decrease). The contribution percentage of BSP showed a significant rising trend(P<0.05) under 1.5NPKM. The average contribution percentage of BSP among fertilizations ranged from 74.4 to 84.7%, and ranked as 1.5NPKM>NPKM>NPK≈NPKS, indicating that organic manure combined with chemical fertilizers(1.5NPKM and NPKM) could more effectively increase BSP compared with the inorganic fertilizer application alone(NPK) in the black soil. This study showed that soil organic matter(SOM) was the key factor among various fertility factors that could affect BSP in the black soil, and total N, total P and/or available P also played important role in BSP increasing. Compared with the chemical fertilization, a balanced chemical plus manure or straw fertilization(NPKM or NPKS) not only increased the concentrations of soil nutrient, but also improved the soil physical properties, and structure and diversity of soil microbial population, resulting in an iincrease of BSP. | 6 | Prognostic value of pretreatment contrast-enhanced computed tomography in esophageal neuroendocrine carcinoma: A multicenter follow-up study显示文摘BACKGROUND The rare incidence of esophageal neuroendocrine carcinoma(NEC)and limited treatment experience result in insufficient clinical observations and unsuitable guidelines for its management.AIM To investigate the prognostic value of pretreatment contrast-enhanced computed tomography(CT)characteristics in patients with esophageal NEC.METHODS Seventy-seven esophageal NEC patients who received contrast-enhanced CT at two hospitals were enrolled in this study from June 2014 to December 2019.The clinical features and image characteristics were recorded accordingly.Univariate survival analysis was performed using the Kaplan-Meier method and log-rank test,and multivariate analysis was carried out with a Cox proportional hazards model.RESULTS The multivariate analysis performed using the Cox proportional hazards model showed that N stage,adjuvant chemotherapy,and degree of enhancement were independent prognostic factors for overall survival(OS).Meanwhile,adjuvant chemotherapy was an independent prognostic factor for progression-free survival(PFS).The hazard ratios(HRs)of N stage,adjuvant chemotherapy,and degree of enhancement(mild vs moderate/marked)for OS were 0.426(P=0.024),3.862(P=0.006),and 2.169/0.809(P=0.037),respectively.The HR of adjuvant chemotherapy for PFS was 6.432(P<0.001).Adjuvant chemotherapy was significantly associated with degree of enhancement(P=0.018).CONCLUSION Adjuvant chemotherapy is an independent prognostic factor for OS and PFS.Additionally,N stage and degree of enhancement are prognostic factors for OS in patients with esophageal NEC. | Yue Zhou Ping Hou Kai-Ji Zha Feng Wang Kun Zhou Wei He Jian-Bo Gao | 2020 | World Journal of Gastroenterology2020,26,31: | 2 |

| 8 | Low-dose spectral insufflation computed tomography protocol preoperatively optimized for T stage esophageal cancer-preliminary research experience显示文摘AIM To evaluate the T stage of esophageal squamous cell carcinoma(ESCC) using preoperative low-dose esophageal insufflation computed tomography(EICT).METHODS One hundred and twenty ESCC patients confirmed by surgery or esophagoscopy were divided into three groups. Groups B and C were injected with 300 mgI/kg contrast medium for automatic spectral imaging assist(GSI assist), while group A underwent a conventional 120 kVp computed tomography(CT) scan with a 450 mg I/kg contrast medium injection. EICT was performed in group C. Group A was reconstructed with filtered back projection, and groups B and C were reconstructed with 50% adaptive statistical iterative reconstruction. The contrast-to-noise ratio of lesion-to-mediastinal adipose tissue and the radiation dose were measured. Specific imaging features were observed, and T stage ESCCs were evaluated.RESULTS The sensitivity and accuracy of the T1/2 stage were higher in group C than in groups A and B(sensitivity: 43.75% vs 31.82% and 33.33%; accuracy: 54.29% vs 46.67% and 52.50%, respectively). With regard to the T3 stage, the sensitivity and specificity in group C were higher than those in groups A and B(sensitivity: 56.25% vs 41.17% and 44.44%; specificity: 73.68% vs 67.86% and 63.64%, respectively). The diagnostic sensitivity, specificity and accuracy of the T4 stage were similar among all groups. There were no significant differences in volume CT dose index [(5.91 ± 2.57) mGy vs(3.24 ± 1.20) vs(3.65 ± 1.77) mGy], doselength product [(167.10 ± 99.08) mGy·cm vs(113.24 ± 54.46) mGy·cm vs(117.98 ± 32.32) mGy·cm] and effective dose [(2.52 ± 1.39) vs(1.63 ± 0.76) vs(1.73 ± 0.44) mSv] among the groups(P > 0.05). However, groups B and C received similar effective doses but lower iodine loads than group A [(300 vs 450) mgI/kg].CONCLUSION EICT combined with GSI assist allows differential diagnosis between the T1/2 and T3 stages. | 17 | Effect of different ventilation methods combined with pulmonary surfactant on neonatal acute respiratory distress syndrome显示文摘BACKGROUND Acute respiratory distress syndrome precipitates is widespread pulmonary injury in impacted individuals,the neonatal respiratory distress syndrome(NRDS),primarily observed in preterm infants,represents a prevalent critical condition in neonatal clinical settings.AIM To investigate the clinical efficacy of various ventilation strategies combined with pulmonary surfactant(PS)therapy in the treatment of NRDS.METHODS A total of 20 neonates diagnosed with respiratory distress syndrome,admitted between May 2021 and June 2022,were randomly assigned to either a research group or a control group.Neonates in the research group received treatment involving high-frequency oscillatory ventilation(HFOV)in conjunction with PS.In contrast,neonates in the control group were administered either controlled mechanical ventilation or synchronous intermittent mandatory ventilation,combined with PS.Arterial blood samples from the neonates in both groups were collected before treatment,as well as 6 h,12 h,24 h,and 48 h post-treatment.These samples underwent blood gas analysis,with measurements taken for pH value,partial pressures of oxygen(O_(2))and carbon dioxide.Concurrently,data was collected on the duration of ventilator use,length of hospitalization time,O_(2) treatment time,treatment outcomes,and complications of the ventilator.RESULTS From 6-48 h post-treatment,both groups demonstrated significant improvements in arterial blood pH and oxygen partial pressure,along with a significant decrease in carbon dioxide partial pressure compared to pre-treatment values(P<0.05).Although these changes progressed over time,there were no significant differences between the two groups(P>0.05).However,the research group had significantly lower X-ray scores,shorter hospitalization time,and less time on O_(2) therapy compared to the control group(P<0.05).Mortality rates were similar between the two groups(P>0.05),but the research group had a significantly lower incidence of complications(P<0.05).CONCLUSION The integration of HFOV combine with PS has proven to effectively expedite the treatment duration,decrease the occurrence of complications,and secure the therapeutic efficacy in managing NRDS. | Qing Qing Ping Zha Li-Ying Dai Yang Wang | 2023 | World Journal of Clinical Cases2023,11,25: | 1 |
