| 1 | Incidental gallbladder cancer during laparoscopic cholecystectomy:Managing an unexpected finding显示文摘AIM:To evaluate the impact of incidental gallbladder cancer on surgical experience.METHODS:Between 1998 and 2008 all cases of cholecystectomy at two divisions of general surgery,one university based and one at a public hospital,were retrospectively reviewed.Gallbladder pathology was diagnosed by history,physical examination,and laboratory and imaging studies [ultrasonography and computed tomography(CT)].Patients with gallbladder cancer(GBC) were further analyzed for demographic data,and type of operation,surgical morbidity and mortality,histopathological classification,and survival.Incidental GBC was compared with suspected or preoperatively diagnosed GBC.The primary endpoint was diseasefree survival(DFS).The secondary endpoint was the difference in DFS between patients previously treated with laparoscopic cholecystectomy and those who had oncological resection as first intervention.RESULTS:Nineteen patients(11 women and eight men) were found to have GBC.The male to female ratio was 1:1.4 and the mean age was 68 years(range:45-82 years).Preoperative diagnosis was made in 10 cases,and eight were diagnosed postoperatively.One was suspected intraoperatively and confirmed by frozen sections.The ratio between incidental and nonincidental cases was 9/19.The tumor node metastasis stage was:pTis(1),pT1a(2),pT1b(4),pT2(6),pT3(4),pT4(2);five cases with stageⅠa(T1 a-b);two with stageⅠb(T2 N0);one with stage Ⅱa(T3 N0);six with stage Ⅱb(T1-T3 N1);two with stage Ⅲ(T4 Nx Nx);and one with stage Ⅳ(Tx Nx Mx).Eighty-eight percent of the incidental cases were discovered at an early stage(≤Ⅱ).Preoperative diagnosis of the 19 patients with GBC was:GBC with liver invasion diagnosed by preoperative CT(nine cases),gallbladder abscess perforated into hepatic parenchyma and involving the transversal mesocolon and hepatic hilum(one case),porcelain gallbladder(one case),gallbladder adenoma(one case),and chronic cholelithiasis(eight cases).Every case,except one,with a T1b or more advanced invasion underwent Ⅳb + Ⅴ wedge liver resection and pericholedochic/hepatoduodenal lymphadenectomy.One patient with stage T1b GBC refused further surgery.Cases with Tis and T1a involvement were treated with cholecystectomy alone.One incidental case was diagnosed by intraoperative frozen section and treated with cholecystectomy alone.Six of the nine patients with incidental diagnosis reached 5-year DFS.One patient reached 38 mo survival despite a port-site recurrence 2 years after original surgery.Cases with non incidental diagnosis were more locally advanced and only two patients experienced 5-year DFS.CONCLUSION:Laparoscopic cholecystectomy does not affect survival if implemented properly.Reoperation should have two objectives:R0 resection and clearance of the lymph nodes. | Andrea Cavallaro Gaetano Piccolo Vincenzo Panebianco Emanuele Lo Menzo Massimiliano Berretta Antonio Zanghì Maria Di Vita Alessandro Cappellani | 2012 | World Journal of Gastroenterology2012，18，30： | 33 |
| 2 | T1-weighted dual-echo MRI for fat quantification in pediatric nonalcoholic fatty liver disease显示文摘AIM:To determine in obese children with nonalcoholic fatty liver disease (NAFLD) the accuracy of magnetic resonance imaging (MRI) in assessing liver fat concentration.METHODS:A case-control study was performed.Cases were 25 obese children with biopsy-proven NAFLD.Controls were 25 obese children matched for age and gender,without NAFLD at ultrasonography and with normal levels of aminotransferases and insulin.Hepatic fat fraction (HFF) by MRI was obtained using a modification of the Dixon method.RESULTS:HFF ranged from 2% to 44% [mean,19.0% (95% CI,15.1-27.4)] in children with NAFLD,while in the controls this value ranged from 0.08% to 4.69% [2.0% (1.3-2.5),P < 0.0001].HFF was highly correlated with histological steatosis (r=0.883,P < 0.0001) in the NAFLD children.According to the histological grade of steatosis,the mean HFF was 8.7% (95% CI,6.0-11.6) for mild,21.6% (15.3-27.0) for moderate,and 39.7% (34.4-45.0) for severe fatty liver infiltration.With a cutoff of 4.85%,HFF had a sensitivity of 95.8% for the diagnosis of histological steatosis ≥ 5%.All control children had HFF lower than 4.85%;thus,the specificity was 100%.After 12 mo,children with weight loss displayed a significant decrease in HFF.CONCLUSION:MRI is an accurate methodology for liver fat quantification in pediatric NAFLD. | Lucia Pacifico Michele Di Martino Carlo Catalano Valeria Panebianco Mario Bezzi Caterina Anania Claudio Chiesa | 2011 | World Journal of Gastroenterology2011，17，25： | 9 |

| 14 | Activity of Tagetes minuta Linnaeus(Asteraceae)essential oil against L3 Anisakis larvae type 1显示文摘Objective:To evaluate in vitro effects of Tagetes minuta L. essential oil(TEO) on L3 Anisakis larvae type 1. Methods:In order to evaluate the potential use of Tagetes minuta essential oil against L3 Anisakis larvae three different media were tested:1) a saline solution(SS); 2) an industrial marinating solution; 3) sunflower seeds oil(SO). For each media and concentrations of TEO(0.1%,0.5%,1.0% and 5.0% v/v),20 parasites were introduced into plastic Petri dishes(diameter 90 mm) and maintained at room temperature. As controls,larvae were maintained without TEO under identical experimental conditions in SS,MS and SO. A total of 900 larvae were tested. The normalized mean viability,LT100,LT50 and the percentage of inactivation at 24 hours were calculated. Results:In vitro tests revealed a complete inactivation of parasites in saline solution after 2 hours with 5% and 1% of TEO. In marinating solution,a complete inactivation of parasites was observed after 4 hours at all concentrations used. A slower activity for all TEO concentration was reported in SO.
