1Quality of life in patients with minimal hepatic encephalopathy显示文摘Minimal hepatic encephalopathy(MHE) represents the mildest type of hepatic encephalopathy(HE). This condition alters the performance of psychometric tests by impairing attention, working memory, psychomotor speed, and visuospatial ability, as well as electrophysiological and other functional brain measures. MHE is a frequent complication of liver disease, affecting up to 80% of tested patients, depending of the diagnostic tools used for the diagnosis. MHE is related to falls, to an impairment in fitness to drive and the development of overt HE, MHE severely affects the lives of patients and caregivers by altering their quality of life(QoL) and their socioeconomic status. MHE is detected in clinically asymptomatic patients through appropriate psychometric tests and neurophysiological methods which highlight neuropsychological alterations such as video-spatial orientation deficits, attention disorders, memory, reaction times, electroencephalogram slowing, prolongation of latency evoked cognitive potentials and reduction in the critical flicker frequency. Several treatments have been proposed for MHE treatment such as non-absorbable disaccharides, poorly absorbable antibiotics such rifaximin, probiotics and branched chain amino acids. However, because of the multiple diagnosis methods, the various endpoints of treatment trials and the variety of agents used in trials, to date the treatment of MHE is not routinely recommended apart from on a case-by-case basis. 2Spontaneous porto-systemic shunts in liver cirrhosis:Clinical and therapeutical aspects显示文摘Spontaneous porto-systemic shunts(SPSS)are frequent in liver cirrhosis and their prevalence increases as liver function deteriorates,probably as a consequence of worsening portal hypertension,but without achieving an effective protection against cirrhosis'complications.Several types of SPSS have been described in the literature,each one associated with different clinical manifestations.In particular,recurrent or persistent hepatic encephalopathy is more frequent in patients with splenorenal shunt,while the presence of gastric varices and consequently the incidence of variceal bleeding is more common in gastrorenal shunt.In the advanced stage,the presence of large SPSS can lead to the so called“portosystemic shunt syndrome”,characterized by a progressive deterioration of hepatic function,hepatic encephalopathy and,sometimes,portal vein thrombosis.The detection of SPSS in patients with liver cirrhosis is recommended in order to prevent or treat recurrent hepatic encephalopathy or variceal bleeding.Silvia Nardelli Oliviero Riggio Stefania Gioia Marta Puzzono Giuseppe Pelle Lorenzo Ridola 2020World Journal of Gastroenterology2020,26,15:11
3An efficient and accurate framework for calculating lattice thermal conductivity of solids:AFLOW-AAPL Automatic Anharmonic Phonon Library显示文摘One of the most accurate approaches for calculating lattice thermal conductivity,κ_(l),is solving the Boltzmann transport equation starting from third-order anharmonic force constants.In addition to the underlying approximations of ab-initio parameterization,two main challenges are associated with this path:high computational costs and lack of automation in the frameworks using this methodology,which affect the discovery rate of novel materials with ad-hoc properties.Here,the Automatic Anharmonic Phonon Library(AAPL)is presented.It efficiently computes interatomic force constants by making effective use of crystal symmetry analysis,it solves the Boltzmann transport equation to obtain κ_(l),and allows a fully integrated operation with minimum user intervention,a rational addition to the current high-throughput accelerated materials development framework AFLOW.An“experiment vs.theory”study of the approach is shown,comparing accuracy and speed with respect to other available packages,and for materials characterized by strong electron localization and correlation.Combining AAPL with the pseudo-hybrid functional ACBN0 is possible to improve accuracy without increasing computational requirements.Jose J.Plata Pinku Nath Demet Usanmaz Jesus Carrete Cormac Toher Maarten de Jong Mark Asta Marco Fornari Marco Buongiorno Nardelli Stefano Curtarolo 2017npj Computational Materials2017,,1:5
4Ambient Backscatter Communications over NOMA Downlink Channels显示文摘In this paper,we investigate the performance of commensal ambient backscatter communications(AmBC)that ride on a non-ortho go nal multiple access(NOMA)downlink transmission,in which a backscatter device(BD)splits part of its received signals from the base station(BS)for energy harvesting,and backscatters the remaining received signals to transmit information to a cellular user.Specifically,under the power consumption constraint at BD and the peak transmit power constraint at BS,we derive the optimal reflection coefficient at BD,the optimal total transmit power at BS,and the optimal power allocation at BS for each transmission block to maximize the ergodic capacity of the ambient backscatter transmission on the premise of preserving the outage performance of the NOMA downlink transmission.Furthermore,we consider a scenario where the BS is restricted by a maximum allowed average transmit power and the reflection coefficient at BD is fixed due to BD’s low-complexity nature.An algorithm is developed to determine the optimal total transmit power and power allocation at BS for this scenario.Also,a low-complexity algorithm is proposed for this scenario to reduce the computational complexity and the signaling overheads.Finally,the performance of the derived solutions are studied and compared via numerical simulations.Weiyu Chen Haiyang Ding Shilian Wang Daniel Benevides daCosta Fengkui Gong Pedro Henrique Juliano Nardelli 2020China Communications2020,17,6:4
5Sarcopenia and cognitive impairment in liver cirrhosis: A viewpoint on the clinical impact of minimal hepatic encephalopathy显示文摘Minimal hepatic encephalopathy (MHE) represents the mildest type of hepatic encephalopathy (HE). MHE is considered as a preclinical stage of HE and is part of a wide spectrum of typical neurocognitive alterations characteristic of patients with liver cirrhosis, particularly involving the areas of attention, alertness, response inhibition, and executive functions. MHE can be detected by testing the patients’ psychometric performance, attention, working memory, psychomotor speed, and visuospatial ability, as well as by means of electrophysiological and other functional brain measures. MHE is very frequent, affecting from 20% up to 80% of patients tested, depending of the diagnostic tools used. Although subclinical, MHE is considered to be clinically relevant. In fact, MHE has been related to the patients’ falls, fitness to drive, and working ability. As a consequence, MHE affects the patients and caregivers lives by altering their quality of life and even their socioeconomic status. Recently sarcopenia, a very common condition in patients with advanced liver disease, has been shown to be strictly related to both minimal and overt HE. 7Coordination corrected ab initio formation enthalpies显示文摘The correct calculation of formation enthalpy is one of the enablers of ab-initio computational materials design.For several classes of systems(e.g.oxides)standard density functional theory produces incorrect values.Here we propose the“coordination corrected enthalpies”method(CCE),based on the number of nearest neighbor cation–anion bonds,and also capable of correcting relative stability of polymorphs.CCE uses calculations employing the Perdew,Burke and Ernzerhof(PBE),local density approximation(LDA)and strongly constrained and appropriately normed(SCAN)exchange correlation functionals,in conjunction with a quasiharmonic Debye model to treat zero-point vibrational and thermal effects.The benchmark,performed on binary and ternary oxides(halides),shows very accurate room temperature results for all functionals,with the smallest mean absolute error of 27(24)meV/atom obtained with SCAN.The zero-point vibrational and thermal contributions to the formation enthalpies are small and with different signs—largely canceling each other.Rico Friedrich Demet Usanmaz Corey Oses Andrew Supka Marco Fornari Marco Buongiorno Nardelli Cormac Toher Stefano Curtarolo 2019npj Computational Materials2019,,1:3

10Is porto sinusoidal vascular disease to be actively searched in patients with portal vein thrombosis?显示文摘Porto sinusoidal vascular liver disease (PSVD) and portal vein thrombosis (PVT) are distinct vascular liver diseases characterized, respectively, by an intrahepatic and a prehepatic obstacle to the flow in the liver portal system. PVT may also occur as a complication of the natural history of PSVD, especially if a prothrombotic condition coexists. In other cases, it is associated to local and systemic pro-thrombotic conditions, even if its cause remains unknown in up to 25% despite an active search. In our opinion, the presence of PSVD should be suspected in patients with PVT especially in those with PVT “sine causa” and the active search of this condition should be included in their diagnostic work-out. However, sometimes the diagnosis of pre-existing PSVD is very hard. Biopsy cannot be fully discriminant as similar histological data have been described in both conditions. Liver stiffness may help as it has been shown to be higher in PSVD than in “pure” PVT, due to the presence of sclerosis in the portal venous radicles observable in PSVD patients. Nevertheless, comparing liver stiffness between PVT and PSVD has until now been restricted to very limited series of patients. In conclusion, even if it is still totally hypothetical, our point of view may have clinical consequences, especially when deciding to perform a liver biopsy in patients with a higher liver stiffness and suspending the anticoagulation in patients with PVT and no detectable prothrombotic factors.Stefania Gioia Silvia Nardelli Lorenzo Ridola Giulia d’Amati Oliviero Riggio 2019World Journal of Hepatology2019,11,8:2
