| 5 | Split-dose vs same-day reduced-volume polyethylene glycol electrolyte lavage solution for morning colonoscopy显示文摘AIM:To compare same-day whole-dose vs split-dose of 2-litre polyethylene glycol electrolyte lavage solution(PEG-ELS)plus bisacodyl for colon cleansing for morning colonoscopy.METHODS:Consecutive adult patients undergoing morning colonoscopy were allocated into two groups i.e.,same-day whole-dose or split-dose of 2-litre PEGELS.Investigators and endoscopists were blinded to the allocation.All patients completed a questionnaire that was designed by Aronchick and colleagues to assess the tolerability of the bowel preparation regime used.In addition,patients answered an ordinal fivevalue Likert scale question on comfort level during bowel preparation.Endoscopists graded the quality of bowel preparation using the Boston bowel preparation scale(BBPS).In addition,endoscopists gave an overall grading of the quality of bowel preparation.Cecal intu-bation time,withdrawal time,total colonoscopy time,adenoma detection rate and number of adenomas detected for each patient were recorded.Sample size was calculated using an online calculator for binary outcome non-inferiority trial.Analyses was based upon intent-to-treat.Significance was assumed at P-value<0.05.RESULTS:Data for 295 patients were analysed.Mean age was 62.0±14.4 years old and consisted of 50.2%male.There were 143 and 152 patients in the split-dose and whole-dose group,respectively.Splitdose was as good as whole-dose for quality of bowel preparation.The total BBPS score was as good in the split-dose group compared to the whole-dose group[6(6-8)vs 6(6-7),P=0.038].There was no difference in cecal intubation rate,cecal intubation time,withdrawal time,total colonoscopy time and adenoma detection rate.Median number of adenoma detected was marginally higher in the split-dose group[2(1-3)vs 1(1-2),P=0.010].Patients in the whole-dose group had more nausea(37.5%vs 25.2%,P=0.023)and vomiting(16.4%vs 8.4%,P=0.037),and were less likely to complete the bowel preparation(94.1%vs 99.3%,P=0.020).Patients in the split-dose group were less likely to refuse the same bowel preparation regime(6.3%vs 13.8%,P=0.033)and less likely to want to try another bowel preparation regime(53.8%vs 78.9%,P<0.001).CONCLUSION:Splitting reduced-volume PEG-ELS for morning colonoscopy is as effective as taking the whole dose on the same morning but is better tolerated and preferred by patients. | Wah-Kheong Chan Najib Azmi Sanjiv Mahadeva Khean-Lee Goh | 2014 | World Journal of Gastroenterology2014,20,39: | 7 |

| 7 | Neural Network Based Adaptive Tracking Control for a Class of Pure Feedback Nonlinear Systems With Input Saturation显示文摘In this paper, an adaptive neural networks(NNs)tracking controller is proposed for a class of single-input/singleoutput(SISO) non-affine pure-feedback non-linear systems with input saturation. In the proposed approach, the original input saturated nonlinear system is augmented by a low pass filter.Then, new system states are introduced to implement states transformation of the augmented model. The resulting new model in affine Brunovsky form permits direct and simpler controller design by avoiding back-stepping technique and its complexity growing as done in existing methods in the literature.In controller design of the proposed approach, a state observer,based on the strictly positive real(SPR) theory, is introduced and designed to estimate the new system states, and only two neural networks are used to approximate the uncertain nonlinearities and compensate for the saturation nonlinearity of actuator. The proposed approach can not only provide a simple and effective way for construction of the controller in adaptive neural networks control of non-affine systems with input saturation, but also guarantee the tracking performance and the boundedness of all the signals in the closed-loop system. The stability of the control system is investigated by using the Lyapunov theory. | 8 | 美洲大蠊提取物逆转人肝癌细胞HepG2/ADM多药耐药的机制研究显示文摘目的:研究美洲大蠊提取物脱脂膏及CⅡ-3(分别简称'脱脂膏''CⅡ-3')逆转耐阿霉素(ADM)人肝癌细胞HepG2/ADM多药耐药的作用机制。方法:采用MTT法考察不同质量浓度索拉非尼(阳性对照)、脱脂膏和CⅡ-3对HepG2/ADM细胞的毒性作用,并计算20%抑制浓度(IC20)。试验设置敏感组、耐药组、索拉非尼组、脱脂膏组和CⅡ-3组,敏感组使用HepG2细胞,后4组均使用HepG2/ADM细胞。敏感组和耐药组细胞给予常规培养基,其余3组细胞给予相应药物(浓度均为IC20)。采用激光扫描共聚焦显微技术测定细胞中ADM含量;采用Western blotting法测定细胞中凋亡相关蛋白[B细胞淋巴瘤2(Bcl-2)、剪切型胱天蛋白酶9 p37(Cleaved-Caspase-9 p37)]的表达水平;分别采用实时荧光定量-聚合酶链式反应法和免疫细胞化学染色法检测细胞中多药耐药相关基因mRNA及蛋白[P-糖蛋白(P-gp)(MDR1基因表达产物)、肺耐药蛋白(LRP)、乳腺癌耐药相关蛋白(BCRP)]和酶介导多药耐药途径中相关基因mRNA及蛋白[谷胱甘肽转移酶(GST-π)、DNA拓扑异构酶Ⅱ(TopoⅡ)]的表达水平。结果:索拉非尼、脱脂膏、CⅡ-3对HepG2/ADM细胞的IC20分别为(2.40±0.16)、(200.44±27.52)、(18.00±1.82)μg/mL。与敏感组比较,耐药组细胞中Bcl-2、P-gp、LRP、BCRP、TopoⅡ蛋白表达水平以及MDR1、LRP、BCRP、GST-πm RNA表达水平均显著升高(P<0.05或P<0.01)。与耐药组比较,脱脂膏组和CⅡ-3组细胞中ADM含量显著增加(P<0.05或P<0.01),MDR1 m RNA表达水平和LRP、BCRP、GST-πm RNA及其蛋白表达水平均显著降低(P<0.05或P<0.01);CⅡ-3组细胞中Bcl-2蛋白表达水平、TopoⅡmRNA表达水平均显著降低(P<0.01),Cleaved-Caspase-9 p37蛋白表达水平显著升高(P<0.05)。结论:脱脂膏、CⅡ-3可通过减少药物外排、促进细胞凋亡、减少多药耐药相关基因和酶介导多药耐药途径中相关基因的mRNA及其蛋白的表达等方式,逆转HepG2/ADM细胞的多药耐药性,且CⅡ-3的效果优于脱脂膏。 | 李彩琳 吴定宇 吕鸿 张鸿翰 王彦权 Najib Mohammerd 彭芳 | 2020 | 中国药房2020,31,15: | 4 |

Ultrasonic assisted FSW offers numerous advantages over the conventional FSW process. The superimposing of high-frequency vibrations improves various phenomena of the process and the physical,metallurgical,mechanical and tribological properties of the welded joint. Hepatitis C infection is associated with a wide range of extra-hepatic manifestations in various organs including the kidneys. A strong association between hepatitis C and chronickidney disease has come to light. Hemodialysis in supporting the end stage renal disease patients unfortunately carries a risk for hepatitis C infection. Despite much improvement in the care of this group of patients,the prevalence of hepatitis C infection in hemodialysis patients is still higher than the general population. Hepatitis C infection has a negative effect on the survival of hemodialysis and renal transplant patients. Treatment of hepatitis C in end stage renal disease patients using conventional or pegylated interferon with or without ribavirin remains a clinical challenge with low response rate,high dropout rate due to poor tolerability and many unmet needs. The approval of new direct acting antiviral agents for hepatitis C may dramatically change the treatment approach in hepatitis C infected patients with mild to moderate renal impairment. However it remains to be confirmed if the newer Hepatitis C therapies are safe in individuals with severe renal impairment.
