| 1 | Medical management of gastric cancer:A 2014 update显示文摘Gastric cancer represents a serious health problem on a global scale. It is the second leading cause of cancer-related death worldwide. Novel therapeutic targets are desperately needed because the meager improvement in the cure rate of about 10% realized by adjunctive treatments to surgery is unacceptable as > 50% patients with localized gastric cancer succumb to their disease. Either postoperative chemoradiotherapy(United States),pre-and post-operative chemotherapy(Europe),and adjuvant chemotherapy after a D2 resection(Asia) can all be regarded as standards of care in the localized gastric cancer management. In metastatic disease the addition of trastuzumab to chemotherapy is standard of care in Her2 positive disease. | 3 | Withdrawal time in excellent or very poor bowel preparation qualities显示文摘AIM: To evaluate association(s) between withdrawal time and polyp detection in various bowel preparation qualities. METHODS: Retrospective cohort analysis of screening colonoscopies performed between January 2005 and June 2011 for patients with average risk of colorectal cancer. Exclusion criteria included patients with a personal history of adenomatous polyps or colon cancer, prior colonic resection, significant family history of colorectal cancer, screening colonoscopy after other abnormal screening tests such as flexible sigmoidoscopy or barium enema, and screening colonoscopies during in-patient care. All procedures were performed or directly supervised by gastroenterologists. Main measurements were number of colonic segments with polyps and total number of colonic polyps.RESULTS: Multivariate analysis of 8331 colonosco-pies showed longer withdrawal time was associated with more colonic segments with polyps in good(adjusted OR = 1.16; 95%CI: 1.13-1.19), fair(OR = 1.13; 95%CI: 1.10-1.17), and poor(OR = 1.18; 95%CI: 1.11-1.26) bowel preparation qualities. A higher number of total polyps was associated with longer withdrawal time in good(OR = 1.15; 95%CI: 1.13-1.18), fair(OR = 1.13; 95%CI: 1.10-1.16), and poor(OR = 1.20; 95%CI: 1.13-1.29) bowel preparation qualities. | 5 | Long-term study of at-sea distribution of seabirds and marine mammals in the Scotia Sea, Antarctica显示文摘The Scotia Sea is one of the most biologically rich regions of Antarctica, and it hosts a large community of upper trophic-level predators. Long-term at-sea monitoring provides valuable information on the Antarctic marine ecosystem and relationships among top predators. This paper presents the results of at-sea monitoring of seabirds and cetaceans over five consecutive summer seasons(2010—2014) in the Scotia Sea, Antarctica. A total of 11 656 flying birds belonging to 24 species were recorded in 884 ten-minute counts. Six Procellariiformes species were abundant: Black-browed Albatross, Cape Petrel, Southern Fulmar, Antarctic Prion, Wilson's Storm-petrel, and Black-bellied Storm-petrel. Only three of these species accounted for 82% of the total abundance: Antarctic Prion(40%), Southern Fulmar(22%), and Cape Petrel(20%). A total of 678 baleen whales belonging to five species were recorded along a sampling effort of 2 351 nautical miles: Humpback, Sei, Southern Right, Fin, and Minke whales, which had different abundances during the study. The Fin Whale had the highest mean encounter rate for the 5 years(0.29 whales per nautical mile), followed by the Humpback Whale(0.09 whales per nautical mile). Annual dissimilarity in abundance of both seabirds and cetaceans occurred in conjunction with changes in the sea surface temperature and ice cover, showing the dependence of top predators on environmental changes.
