1Single center experience of capsule endoscopy in patients with obscure gastrointestinal bleeding显示文摘AIM:To identify optimum timing to maximize diagnostic yield by capsule endoscopy (CE) in patients with obscure gastrointestinal bleeding (OGIB).METHODS:We identified patients who underwent CE at our institution from August 2003 to December 2009.Patient medical records were reviewed to determine type of OGIB (occult,overt),CE results and complications,and timing of CE with respect to onset of bleeding.RESULTS:Out of 385 patients investigated for OGIB,284 (74%) had some lesion detected by CE.In 222 patients (58%),definite lesions were detected that could unequivocally explain OGIB.Small bowel ulcer/erosions secondary to Crohn's disease,tuberculosis or non-steroidal anti-inflammatory agent use were the commonest lesions detected.Patients with overt GI bleeding for < 48 h before CE had the highest diagnostic yield (87%).This was significantly greater (P < 0.05) compared to that in patients with overt bleeding prior to 48 h (68%),as well as those with occult OGIB (59%).CONCLUSION:We established the importance of early CE in management of OGIB.CE within 48 h of overt bleeding has the greatest potential for lesion detection.Mahesh Kumar Goenka Shounak Majumder Sanjeev Kumar Pradeepta Kumar Sethy Usha Goenka 2011World Journal of Gastroenterology2011,17,6:39

3Capsule endoscopy:Present status and future expectation显示文摘Video capsule endoscopy(CE)since its introduction 13years back,has revolutionized our approach to small intestinal diseases.Obscure gastrointestinal bleed(OGIB)continues to be the most important indication for CE with a high sensitivity,specificity as well as positive and negative predictive values.It is best performed during ongoing bleed or immediately thereafter.Overt OGIB has a higher diagnostic yield than occult OGIB.However,even in iron deficiency anemia,CE is emerging as important investigation after initial negative work up.In suspected Crohn’s disease(CD),CE has been shown superior to traditional imaging and endoscopic technique and should be considered after a negative ileocolonoscopy.Although CE has also been used for evaluating established CD,a high capsule retention rate precludes its use ahead of cross-sectional imaging.Celiac disease,particularly where gastro-duodenoscopy cannot be performed or is normal,can also be investigated by CE.Small bowel tumor,hereditary polyposis syndrome,and non-steroidal anti-inflammatory drugs induced intestinal damage are other indications for CE.Capsule retention is the only significant adverse outcome of CE and occurs mostly in presence of intestinal obstruction.This can be prevented by use of Patency capsule prior to CE examination.Presence of cardiac pacemaker and intracardiac devices continue to be relative contraindications for CE,though data do not suggest interference of CE with these devices.Major limitations of CE today include failure to control its movement from outside,inability of CE to acquire tissue for diagnosis,and lack of therapeutic help.With ongoing interesting and exciting developments taking place in these areas,these issues would be solved in all probability in near future.CE has the potential to become one of the most important tools in diagnostic and possibly in the therapeutic field of gastrointestinal disorder.Mahesh K Goenka Shounak Majumder Usha Goenka 2014World Journal of Gastroenterology2014,20,29:9
4Pre-treatment of rocks prior to comminution--A critical review of present practices显示文摘It has been an established fact that comminution processes, crushing and grinding, are most energy intensive processes which account for more than half of the total energy consumed in mineral industries.Various alternative pre-treatment methods have been tried by experts around the globe. Although these methods yielded positive results in terms of reduction in energy consumption in crushing and particularly, in grinding operations at laboratory scale, their industrial application still remains an unresolved issue and challenge. Present review paper describes each one of these methods along with outcome of earlier studies and issues that need to be addressed through further rigorous experimental investigation.It also suggests the direction in which future studies can be carried out to meet the primary objective of making comminution processes more energy efficient than today they are.Somani Aditya Nandi Tapas K. 5Inflammatory bowel disease-related colorectal cancer:Past,present and future perspectives显示文摘Inflammatory bowel disease-related colorectal cancer(IBD-CRC)is one of the most serious complications of IBD contributing to significant mortality in this cohort of patients.IBD is often associated with diet and lifestyle-related gut microbial dysbiosis,the interaction of genetic and environmental factors,leading to chronic gut inflammation.According to the“common ground hypothesis”,microbial dysbiosis and intestinal barrier impairment are at the core of the chronic inflammatory process associated with IBD-CRC.Among the many underlying factors known to increase the risk of IBD-CRC,perhaps the most important factor is chronic persistent inflammation.The persistent inflammation in the colon results in increased proliferation of cells necessary for repair but this also increases the risk of dysplastic changes due to chromosomal and microsatellite instability.Multiple pathways have been identified,regulated by many positive and negative factors involved in the development of cancer,which in this case follows the‘inflammation-dysplasia-carcinoma’sequence.Strategies to lower this risk are extremely important to reduce morbidity and mortality due to IBD-CRC,among which colonoscopic surveillance is the most widely accepted and implemented modality,forming part of many national and international guidelines.However,the effectiveness of surveillance in IBD has been a topic of much debate in recent years for multiple reasons—cost-benefit to health systems,resource requirements,and also because of studies showing conflicting long-term data.Our review provides a comprehensive overview of past,present,and future perspectives of IBD-CRC.We explore and analyse evidence from studies over decades and current best practices followed globally.In the future directions section,we cover emerging novel endoscopic techniques and artificial intelligence that could play an important role in managing the risk of IBD-CRC.Snehali Majumder Uday Nagesh Shivaji Rangarajan Kasturi Alben Sigamani Subrata Ghosh Marietta Iacucci 2022World Journal of Gastrointestinal Oncology2022,14,3:5
6A hypothesis for treating inflammation and oxidative stress with hydrogen sulfide during age-related macular degeneration显示文摘Age-related macular degeneration(AMD) is a leading cause of blindness and is becoming a global crisis since affected people will increase to 288 million by 2040.Genetics,age,diabetes,gender,obesity,hypertension,race,hyperopia,iris-color,smoking,sun-light and pyroptosis have varying roles in AMD,but oxidative stress-induced inflammation remains a significant driver of pathobiology.Eye is a unique organ as it contains a remarkable oxygengradient that generates reactive oxygen species(ROS) which upregulates inflammatory pathways.ROS becomes a source of functional and morphological impairments in retinal pigment epithelium(RPE),endothelial cells and retinal ganglion cells.Reports demonstrated that hydrogen sulfide(H_2S) acts as a signaling molecule and that it may treat ailments.Therefore,we propose a novel hypothesis that H_2S may restore homeostasis in the eyes thereby reducing damage caused by oxidative injury and inflammation.Since H_2S has been shown to be a powerful antioxidant because of its free-radicals' inhibition properties in addition to its beneficial effects in age-relatedconditions,therefore,patients may benefit from H_2S salubrious effects not only by minimizing their oxidant and inflammatory injuries to retina but also by lowering retinal glutamate excitotoxicity.Akash K George Mahavir Singh Rubens Petit Homme Avisek Majumder Harpal S SANDhu Suresh C Tyagi 2018International Journal of Ophthalmology(English edition)2018,11,5:3

8In vivo modulation of androgen receptor by androgens显示文摘Aim:To study the effect of androgen and antiandrogen on the level of androgen receptor(AR)mRNA.Methods:The totalRNA was extracted from the prostate and analyzed by slot blot analysis,The blots were hybrid-ized with ARcDNA probe and 1Aprobe(internal control)and autoradionraphy was performed.The intensity of signal was measured with a densitometer and the ratio of AR RNAand1ARNAwas calculated.Results:Androgenic deprivation produced by castration decreased the weight of the prostate and increased the levels of ARmRNA.Treatment of the castrated rats with testostrone increased the weight of prostate and decreased the levels of ARmRNA.Treatment of normal rats with flutamide decreased the weight of the gland and increased the levels of AR mRNA.Conclusion:Androgens produce proliferative effect on the prostate and negatively regulate the AR transcription.V. 9Molecular detection of pancreatic neoplasia: Current status and future promise显示文摘Pancreatic cancer is usually diagnosed at an advanced stage and curative resection is feasible in only a small minority of patients at the time of diagnosis. Diagnosis at an early stage is unequivocally associated with better long-term survival. Several candidate molecular markers for early detection are currently under investigation in different phases of discovery and validation. Recent advances in the technology for whole genome,methylome,ribonucleome,and proteome interrogation has enabled rapid advancements in the field of biomarker discovery. 10Dengue epidemiology and pathogenesis:images of the future viewed through a mirror of the past显示文摘Every year, millions of individuals throughout the world are seriously affected by dengue virus. The unavailability of a vaccine and of anti-viral drugs has made this mosquito-borne disease a serious health concern. Not only does dengue cause fatalities but it also has a profoundly negative economic impact. In recent decades, extensive research has been performed on epidemiology, vector biology, life cycle, pathogenesis, vaccine development and prevention. Although dengue research is still not at a stage to suggest definite hopes of a cure, encouraging significant advances have provided remarkable progress in the fight against infection. 12马来西亚一所私立大学使用视觉显示装置的学生非斜视双眼视觉异常显示文摘目的:评估马来西亚一所私立大学使用视觉显示装置(VDU)的学生非斜视双眼视觉异常(NSBVA)情况。方法:对2019-01/2019-05期间每天使用3h及以上VDU的大学生进行横断面研究。使用一种简便的样本收集方法,所有受试者均通过初级视力检查且满足入选标准,对符合入选标准的学生进一步进行NSBVA评估。采用描述性分析排除NSBVA的百分比,采用Chi-square独立性检验,观察NSBVA与年龄、性别、VDU使用时间的相关性。结果:研究共纳入140名学生,其中女88名(62.9%),男52名(37.1%)。受试者平均年龄为22.54±1.48a,平均VDU使用时间为5.76±2.49h。使用VDU学生的NSBVA百分比为40%。在VDU使用者中,调节异常和倾斜异常的发生率分别为17.86%和22.14%。性别与NSBVA之间存在适度关联(P=0.010)。但是,年龄(P=0.334)和VDU使用时间(P=0.835)与NSBVA无显著相关。结论:马来西亚一所私立大学的学生中,有40%的人使用VDU。在所有NSBVA中,VDU用户调节异常(15%)和会聚功能异常(10%)更为常见。Chiranjib Majumder Toh Ching Ling 2020国际眼科杂志2020,20,6:2
13Bearing and uplift capacities of under-reamed piles in soft clay underlaid by stiff clay using lower-bound finite element limit analysis显示文摘Ensuring a safe foundation design in soft clay is always a challenging task to engineers.In the present study,the effectiveness of under-reamed piles in soft clay underlaid by stiff clay is numerically studied using the lower-bound finite element limit analysis(LB FELA).The bearing and uplift capacities of under-reamed piles are estimated through non-dimensional factors N_(cul)and Fcul,respectively.These factors increased remarkably and marginally compared to N_(cul)and Fcul of the piles without bulbs when the bulb is placed in stiff and soft clay,respectively.For a given ratio of undrained cohesion of stiff to soft clay(c_(2)/c_(1)),the factors N_(cul)and Fcul moderately increased with the increase in the length-to-shaft-diameter ratio(Lu/D)and adhesion factors in soft clay(α_(s1))and stiff clay(αs2).The variation of radial stress along the pile-soil interface,distribution of axial force in the under-reamed piles,and state of plastic shear failure in the soil are also studied under axial compression and tension.The results of this study are expected to be useful for the estimation of the bearing and uplift capacities of under-reamed piles in uniform clay and soft clay underlaid by stiff clay.Mantu MAJUMDER Debarghya CHAKRABORTY 2021Frontiers of Structural and Civil Engineering2021,15,2:2
14Hydrogen sulfide intervention in cystathionine-β-synthase mutant mouse helps restore ocular homeostasis显示文摘AIM: To investigate the applications of hydrogen sulfide(H2 S) in eye-specific ailments in mice.METHODS: Heterozygous cystathionine-β-synthase(CBS+/-) and wild-type C57 BL/6 J(WT) mice fed with or without high methionine diet(HMD) were administered either phosphate buffered saline(PBS) or the slow-release H2 S donor: GYY4137. Several analyses were performed to study GYY4137 effects by examining retinal lysates for key protein expressions along with plasma glutamate and glutathione estimations. Intraocular pressure(IOP) was monitored during GYY4137 treatment; barium sulfate and bovine serum albumin conjugated fluorescein isothiocyanate(BSA-FITC) angiographies were performed for examining vasculature and its permeability post-treatment. Visionguided behavior was also tested employing novel object recognition test(NORT) and light-dark box test(LDBT) recordings.RESULTS: CBS deficiency(CBS+/-) coupled with HMD led disruption of methionine/homocysteine(Hcy) metabolism leading to hyperhomocysteinemia(HHcy) in CBS+/-mice as reflected by increased Hcy, and s-adenosylhomocysteine hydrolase(SAHH) levels. Unlike CBS, cystathionine-γ lyase(CSE), methylenetetrahydrofolate reductase(MTHFR) levels which were reduced but compensated by GYY4137intervention. Heightened oxidative and endoplasmic reticulum(ER) stress responses were mitigated by GYY4137 effects along with enhanced glutathione(GSH) levels. Increased glutamate levels in CBS+/-strain were prominent than WT mice and these mice also exhibited higher IOP that was lowered by GYY4137 treatment. CBS deficiency also resulted in vision-guided behavioral impairment as revealed by NORT and LDBT findings. Interestingly, GYY4137 was able to improve CBS+/-mice behavior together with lowering their glutamate levels. Blood-retinal barrier(BRB) appeared compromised in CBS+/-with vessels' leakage that was mitigated in GYY4137 treated group.
