1Adjusting CA19-9 values to predict malignancy in obstructive jaundice:Influence of bilirubin and C-reactive protein显示文摘AIM:To find a possible relationship between inflammation and CA19-9 tumor marker by analyzing data from patients with benign jaundice(BJ) and malignant jaundice(MJ).METHODS:All patients admitted for obstructive jaundice,in the period 2005-2009,were prospectively enrolled in the study,obtaining a total of 102 patients.On admission,all patients underwent complete standard blood test examinations including C-reactive protein(CRP),bilirubin,CA19-9.Patients were considered eligible for the study when they presented obstructive jaundice confirmed by instrumental examinations and increased serum bilirubin levels(total bilirubin > 2.0 mg/dL).The standard cut-off level for CA19-9 was 32 U/mL,whereas for CRP this was 1.5 mg/L.The CA19-9 level was adjusted by dividing it by the value of serum bilirubin or by the CRP value.The patients were divided into 2 groups,MJ and BJ,and after the adjustment a comparison between the 2 groups of patients was performed.Sensitivity,specificity and positive predictive values were calculated before and after the adjustment.RESULTS:Of the 102 patients,51 were affected by BJ and 51 by MJ.Pathologic CA19-9 levels were found in 71.7% of the patients.In the group of 51 BJ patients there were 29(56.9%) males and 22(43.1%) females with a median age of 66 years(range 24-96 years),whereas in the MJ group there were 24(47%) males and 27(53%) females,with a mean age of 70 years(range 30-92 years).Pathologic CA19-9 serum level was found in 82.3% of MJ.CRP levels were pathologic in 66.6% of the patients with BJ and in 49% with MJ.Bilirubin and CA19-9 average levels were significantly higher in MJ compared with BJ(P = 0.000 and P = 0.02),while the CRP level was significantly higher in BJ(P = 0.000).Considering a CA19-9 cut-off level of 32 U/mL,82.3% in the MJ group and 54.9% in the BJ group were positive for CA19-9(P = 0.002).A CA19-9 cut-off of 100 U/mL increases the difference between the two groups:35.3% in BJ and 68.6% in MJ(P = 0.0007).Adjusting the CA19-9 value by dividing it by serum bilirubin level meant that 21.5% in the BJ and 49% in the MJ group remained with a positive CA19-9 value(P = 0.003),while adjusting the CA19-9 value by dividing it by serum CRP value meant that 31.4% in the BJ group and 76.5% in the MJ group still had a positive CA19-9 value(P = 0.000004).Sensitivity,specificity,positive predictive values of CA19-9 > 32 U/mL were 82.3%,45% and 59.1%;when the cutoff was CA19-9 > 100 U/mL they were,respectively,68.6%,64.7% and 66%.When the CA19-9 value was adjusted by dividing it by the bilirubin or CRP values,these became 49%,78.4%,69.4% and 76.5%,68.6%,70.9%,respectively.CONCLUSION:The present study proposes CRP as a new and useful correction factor to improve the diag-nostic value of the CA19-9 tumor marker in patients with cholestatic jaundice.Gaetano La Greca Maria Sofia Rosario Lombardo Saverio Latteri Agostino Ricotta Stefano Puleo Domenico Russello 2012World Journal of Gastroenterology2012,18,31:20
2Laparo-endoscopic “Rendezvous” to treat cholecysto-choledocolithiasis: Effective,safe and simplifies the endoscopist’s work显示文摘AIM: To investigate our clinical experience with combined laparo-endoscopic Rendezvous (RV) for the treatment of patients affected by gallstones and common bile duct (CBD) stones and especially to study the never evaluated opinion of the endoscopist concerning the difficulty of the intraoperative endoscopic procedure during the RV in comparison with standard endoscopic retrograde cholangio-pancreatography (ERCP). METHODS: Eighty consecutive patients affected by cholecystolithiasis and diagnosed or suspected CBD stones were treated with a standardized 'tailored' RV. The relevant technical features, the feasibility, the effectiveness in stone clearance, the safety but also the simple evaluation of difficulty and agreement of the endoscopist were analyzed with a questionnaire. RESULTS: The feasibility was 97.5% and the effectiveness 100% concerning CBD clearance and solution of coexisting problems at the papilla. Minor morbidity was 3.3%, the operating time was prolonged by a mean of 14 min, the mean hospital stay was 3.8 d and only one stone’s recurrence occurred. The endoscopist evaluated the procedure to be simpler than standard ERCP-ES in 81.2% of the cases.CONCLUSION: Simultaneous RV carries higheffectiveness and safety at least comparable to those reported for other options. The endoscopist is very often satisfied with this approach because of the minimization of some steps of the endoscopic procedure and avoidance of relevant iatrogenic risk factors. 5Effects of percutaneous varicocele repair on testicular volume:results from a 12-month follow-up显示文摘Varicocele is a comm on fin ding in men. Varicocele correcti on has bee n advocated for young patients with testicular hypotrophy, but there is a lack of morphofunctional follow-up data. We assessed whether percutaneous treatment of left varicocele is associated with testicular 'catch-up growth' in the following 12 months by retrospectively reviewing data from an electronic database of 10 656 patients followed up in our clinic between 2006 and 2016. We selected all young adults (<35 years) with left varicocele who un derwe nt percuta neous treatment, had a minimum of 12 mon ths' ultraso und imagi ng follow-up, and had no other con ditions affecting testicular volume. One hundred and fourteen men (mean±standard deviation [s.d.] of age: 22.8 ± 5.4 years) met the inclusion and exclusion criteria. Left testicular hypotrophy (LTH), defined as a >20% difference between left and right testicular volume at baseline, was observed in 26 (22.8%) men. Participants with LTH (mean±s.d.: 14.5 ± 2.7 ml) had lower baseline testicular volume compared to those without LTH (mean±s.d.: 15.7 ± 3.8 ml;P= 0.032). Repeated measures mixed models showed a sign ifica nt in teraction betwee n LTH and time posttreatme nt when correcting for baseli ne left testicular volume (β= 0.114, 95% confidence interval [Cl]: 0.018-0.210, P=0.020), resulting in a catch-up growth of up to 1.37 ml per year (95% Cl: 0.221- 2.516). Age at intervention was also associated with reduced testicular volume (-0.072 ml per year, 95% Cl:-0.135--0.009;P = 0.024). Percutaneous treatment of left varicocele in young adults with LTH can result in catch-up growth over 1 year of follow-up. 6Mesenchymal stem cells as a therapeutic tool to treat sepsis显示文摘Sepsis is a clinical syndrome caused by a deregulated host response to an infection. Sepsis is the most frequent cause of death in hospitalized patients. Although knowledge of the pathogenesis of sepsis has increased substantially during the last decades, attempts to design effective and specific therapiestargeting components of the derailed host response have failed. Therefore, there is a dramatic need for new and mechanistically alternative therapies to treat this syndrome. Based on their immunomodulatory properties, adult mesenchymal stem or stromal cells(MSCs) can be a novel therapeutic tool to treat sepsis. Indeed, MSCs reduce mortality in experimental models of sepsis by modulating the deregulated inflammatory response against bacteria through the regulation of multiple inflammatory networks, the reprogramming of macrophages and neutrophils towards a more antiinflammatory phenotype and the release of antimicrobial peptides. 7催化一步转化木质素单体为苯酚:建立木质素到高附加值化学品的通道(英文)显示文摘木质素是地球上产量最大的芳香类有机高分子,其有效转化利用在近年来备受关注.催化降解木质素制备酚类单体在过去十年取得了长足进步,目前已开发出氢解、水解、热解、氧化、光解等一系列方法.通过加氢脱氧法可以将木质素的降解产物转化为烃类燃料,但该过程耗氢量大,并且芳香环在加氢气氛下被破坏.另一个可能的应用是将木质素衍生物进一步转化为高附加值的芳香族化合物,但解聚产物成分复杂,成为木质素高效转化为单一高附加值化学品的瓶颈.在加氢条件下,木质素解聚产物主要为酚类混合物,多在羟基临位带有一至两个甲氧基,并在对位带有C2或C3的取代基(多为烷基).针对这一结构特点,我们设计了新反应路径,通过分别去除甲氧基和烷基得到苯酚.该过程保留了苯酚的基本结构而将其他取代基去除,原理上可以有效的将木质素降解的混合物转化为单一产物苯酚.通过催化剂的筛选和优化,Pt/C催化剂对脱甲氧基显示出最好的活性和选择性,在400°C,常压下脱除效率>80%.在流动气氛下连续工作4 h,Pt/C催化剂无失活迹象.H-ZSM-5为最有效的脱烷基催化剂,最优效率83%左右.H-ZSM-5在反应过程中逐渐失活.通过热重差热及红外光谱分析,失活主要原因为积碳.在400°C空气中煅烧后,催化剂可以再生.通过简单的物理混合,Pt/C和H-ZSM-5一步将木质素单体转化为60%的苯酚,显示了该方法直接转化木质素到高附加值苯酚的巨大潜力.这是同时将木质素中甲氧基、烷基选择性脱除的首例报道.经过估算,从原生木质素出发,通过加氢解聚,耦合本文开发的一步脱甲氧基、烷基路径,可将木质素转化为约25%的苯酚.木质素中的甲氧基、烷基将分别转化为甲醇和烯烃,提高了木质素碳资源的利用效率.张佳光 Loris Lombardo Gokalp Gozaydin Paul J.Dyson 颜宁 2018Chinese Journal of Catalysis2018,39,9:5
8Climatic influences on cardiovascular diseases显示文摘Classical risk factors only partially account for variations in cardiovascular disease incidence;therefore,also other so far unknown features,among which meteorological factors,may influence heart diseases(mainly coronary heart diseases,but also heart failure,arrhythmias,aortic dissection and stroke)rates.The most studied phenomenon is ambient temperature.The relation between mortality,as well as cardiovascular diseases incidence,and temperature appears graphically as a'U'shape.Exposure to cold,heat and heat waves is associated with an increased risk of acute coronary syndromes.Other climatic variables,such as humidity,atmospheric pressure,sunlight hours,wind strength and direction and rain/snow precipitations have been hypothesized as related to fatal and nonfatal cardiovascular diseases incidence.Main limitation of these studies is the unavailability of data on individual exposure to weather parameters.Effects of weather may vary depending on other factors,such as population disease profile and age structure.Climatic stress may increase direct and indirect risks to human health via different,complex pathophysiological pathways and exogenous and endogenous mechanisms.These data have attracted growing interest because of the recent earth’s climate change,with consequent increasing ambient temperatures and climatic fluctuations.This review evaluates the evidence base for cardiac health consequences of climate conditions,and it also explores potential further implications.Maurizio Giuseppe Abrignani Alberto Lombardo Annabella Braschi NicolòRenda Vincenzo Abrignani 2022World Journal of Cardiology2022,14,3:5
9Blunt diaphragmatic lesions: Imaging findings and pitfalls显示文摘Blunt diaphragmatic lesions(BDL) are uncommon in trauma patients, but they should be promptly recognized as a delayed diagnosis increases morbidity and mortality. It is well known that BDL are often overlooked at initial imaging, mainly because of distracting injuries to other organs. Sonography may directly depict BDL only in a minor number of cases. Chest X-ray has low sensitivity in detecting BDL and lesions can be reliably suspected only in case of intra-thoracic herniation of abdominal viscera. Thanks to its wide availability, timeeffectiveness and spatial resolution, multi-detector computed tomography(CT) is the imaging modality of choice for diagnosing BDL; several direct and indirect CT signs are associated with BDL. 10Adipokines and C-reactive protein in relation to bone mineralization in pediatric nonalcoholic fatty liver disease显示文摘AIM: To investigate bone mineral density (BMD) in obese children with and without nonalcoholic fatty liver disease (NAFLD); and the association between BMD and serum adipokines, and high-sensitivity C-reactive protein (HSCRP). METHODS: A case-control study was performed. Cases were 44 obese children with NAFLD. The diagnosis of NAFLD was based on magnetic resonance imaging (MRI) with high hepatic fat fraction (≥ 5%). Other causes of chronic liver disease were ruled out. Controls were selected from obese children with normal levels of aminotransferases, and without MRI evidence of fatty liver as well as of other causes of chronic liver diseases. Controls were matched (1-to 1-basis) with thecases on age, gender, pubertal stage and as closely as possible on body mass index-SD score. All participants underwent clinical examination, laboratory tests, and whole body (WB) and lumbar spine (LS) BMD by dual energy X-ray absorptiometry. BMDZ-scores were calcu- lated using race and gender specific LMS curves. RESULTS: Obese children with NAFLD had a significantly lower LS BMDZ-score than those without NAFLD [mean, 0.55 (95%CI: 0.23-0.86) vs 1.29 (95%CI: 0.95-1.63); P < 0.01]. WB BMD Z-score was also decreased in obese children with NAFLD compared to obese children with no NAFLD, though borderline significance was observed [1.55 (95%CI: 1.23-1.87) vs 1.95 (95%CI: 1.67-2.10); P = 0.06]. Children with NAFLD had significantly higher HSCRP, lower adiponectin, but similar leptin levels. Thirty five of the 44 children with MRI-diagnosed NAFLD underwent liver biopsy. Among the children with biopsy-proven NAFLD, 20 (57%) had nonalcoholic steatohepatitis (NASH), while 15 (43%) no NASH. Compared to children without NASH, those with NASH had a significantly lower LS BMD Z-score [mean, 0.27 (95%CI: -0.17-0.71) vs 0.75 (95%CI: 0.13-1.39); P < 0.05] as well as a significantly lower WB BMD Z-score [1.38 (95%CI: 0.89-1.17) vs 1.93 (95%CI: 1.32-2.36); P < 0.05]. In multiple regression analysis, NASH (standardized β coefficient, -0.272; P < 0.01) and HSCRP (standardized β coefficient, -0.192; P < 0.05) were significantly and independently associated with LS BMD Z-score. Similar results were obtained when NAFLD (instead of NASH) was included in the model. WB BMD Z-scores were significantly and independently associated with NASH (standardized β coefficient, -0.248;P < 0.05) and fat mass (standardized β coefficient, -0.224;P < 0.05). 13Comparison of machine learning models for gully erosion susceptibility mapping显示文摘Gully erosion is a disruptive phenomenon which extensively affects the Iranian territory,especially in the Northern provinces.A number of studies have been recently undertaken to study this process and to predict it over space and ultimately,in a broader national effort,to limit its negative effects on local communities.We focused on the Bastam watershed where 9.3%of its surface is currently affected by gullying.Machine learning algorithms are currently under the magnifying glass across the geomorphological community for their high predictive ability.However,unlike the bivariate statistical models,their structure does not provide intuitive and quantifiable measures of environmental preconditioning factors.To cope with such weakness,we interpret preconditioning causes on the basis of a bivariate approach namely,Index of Entropy.And,we performed the susceptibility mapping procedure by testing three extensions of a decision tree model namely,Alternating Decision Tree(ADTree),Naive-Bayes tree(NBTree),and Logistic Model Tree(LMT).We dichotomized the gully information over space into gully presence/absence conditions,which we further explored in their calibration and validation stages.Being the presence/absence information and associated factors identical,the resulting differences are only due to the algorithmic structures of the three models we chose.Such differences are not significant in terms of performances;in fact,the three models produce outstanding predictive AUC measures(ADTree=0.922;NBTree=0.939;LMT=0.944).However,the associated mapping results depict very different patterns where only the LMT is associated with reasonable susceptibility patterns.This is a strong indication of what model combines best performance and mapping for any natural hazard-oriented application.Alireza Arabameri Wei Chen Marco Loche Xia Zhao Yang Li Luigi Lombardo Artemi Cerda Biswajeet Pradhan Dieu Tien Bui 2020Geoscience Frontiers2020,11,5:3

15Treatment of chronic hepatitis C with direct-acting antivirals: The role of resistance显示文摘The use of direct-acting antivirals(DAAs) to treat chronic hepatitis C has resulted in a significant increase in rates of sustained viral response(around 90%-95%) as compared with the standard treatment of peginterferon/ribavirin. Despite this, however, the rates of therapeutic failure in daily clinical practice range from 10%-15%. Most of these cases are due to the presence of resistant viral variants, resulting from mutations produced by substitutions of amino acids in the viral target protein that reduce viral sensitivity to DAAs, thus limiting the efficacy of these drugs. The high genetic diversity of hepatitis C virus has resulted in the existence of resistance-associated variants(RAVs), sometimes even before starting treatment with DAAs, though generally at low levels. These preexisting RAVs do not appear to impact on the sustained viral response, whereas those that appear after DAA therapy could well be determinant in virological failure with future treatments. As well as the presence of RAVs, virological failure to treatment with DAAs is generally associated with other factors related with a poor response, such as the degree of fibrosis, the response to previous therapy, the viral load or the viral genotype. 16Study of borohydride ionic liquids as hydrogen storage materials显示文摘Stability of borohydrides is determined by the localization of the negative charge on the boron atom.Ionic liquids(ILs) allow to modify the stability of the borohydrides and promote new dehydrogenation pathways with a lower activation energy. The combination of borohydride and IL is very easy to realize and no expensive rare earth metals are required. The composite of the ILs with complex hydrides decreases the enthalpy and activation energy for the hydrogen desorption. The Coulomb interaction between borohydride and IL leads to a destabilization of the materials with a significantly lower enthalpy for hydrogen desorption. Here, we report a simple ion exchange reaction using various ILs, such as vinylbenzyltrimethylammonium chloride([VBTMA][Cl]), 1-butyl-3-methylimidazolium chloride([bmim][Cl]), and 1-ethyl-1-methylpyrrolidinium bromide([EMPY][Br]) with NaBH4 to decrease the hydrogen desorption temperature. Dehydrogenation of 1-butyl-3-methylimidazolium borohydride([bmim][BH4]) starts below 100℃. The quantity of desorbed hydrogen ranges between 2.4 wt% and 2.9 wt%, which is close to the theoretical content of hydrogen. 18Thermonuclear ^19F(p,α0)^16O reaction rate显示文摘The thermonuclear^(19)F(p,α_0)16O reaction rate in the temperature region 0.007–10 GK has been derived by re-evaluating the available experimental data, together with the low-energy theoretical R-matrix extrapolations.Our new rate deviates by up to about 30% compared to the previous results, although all rates are consistent within the uncertainties. At very low temperature(e.g. 0.01 GK) our reaction rate is about 20% lower than the most recently published rate, because of a difference in the low energy extrapolated S-factor and a more accurate estimate of the reduced mass used in the calculation of the reaction rate. At temperatures above ~1 GK, our rate is lower, for instance, by about 20% around 1.75 GK, because we have re-evaluated the previous data(Isoya et al., Nucl. Phys.7, 116(1958)) in a meticulous way. The present interpretation is supported by the direct experimental data. The uncertainties of the present evaluated rate are estimated to be about 20% in the temperature region below 0.2 GK,and are mainly caused by the lack of low-energy experimental data and the large uncertainties in the existing data.Asymptotic giant branch(AGB) stars evolve at temperatures below 0.2 GK, where the^(19)F(p,α)16O reaction may play a very important role. However, the current accuracy of the reaction rate is insufficient to help to describe, in a careful way, the fluorine over-abundances observed in AGB stars.
