2Is autoimmune hepatitis a frequent finding among HCV patients with intense interface hepatitis?显示文摘AIM:To evaluate the overlap of autoimmune hepatitis in hepatitis C virus(HCV)-infected patients with intense interface hepatitis.METHODS:Among 1759 patients with hepatitis C submitted to liver biopsy,92(5.2%) presented intense interface hepatitis.These patients were evaluated regarding the presence of antinuclear antibody(ANA),anti-smooth muscle antibody(SMA) and anti-liver/kidney microsomal antibody(LKM-1),levels of γ-globulin and histological findings related to autoimmune hepatitis(plasma cell infiltrate and presence of rosettes).RESULTS:Among patients with hepatitis C and intense interface hepatitis there was a low prevalence of autoantibodies(ANA=12%,SMA=5%,LKM-1=0%) and the median γ-globulin level was within the normal range.Typical histological findings of autoimmune disease were observed in only two cases(2%).After applying the score for diagnosis of autoimmune hepatitis,only one patient was classified with a definitive diagnosis of autoimmune hepatitis.Since overlap with autoimmune hepatitis was not the explanation for the intense necroinflammatory activity in patients with chronic hepatitis C we sought to identify the variables associated with this finding.The presence of intense interface hepatitis was associated with more advanced age,both at the time of infection and at the time of the biopsy,and higher prevalence of blood transfusion and alcohol abuse.CONCLUSION:Although possible,overlap with autoimmune hepatitis is a very rare association in HCV-infected patients with intense interface hepatitis,an unusual presentation which seems to be related to other host variables.Rosilene G Badiani Vitória Becker Renata M Perez Carla AL Matos Lara B Lemos Valéria P Lanzoni Luis Eduardo C Andrade Alessandra Dellavance Antonio Eduardo B Silva Maria Lucia G Ferraz 2010World Journal of Gastroenterology2010,16,29:7
3Renin-angiotensin system in the kidney: What is new?显示文摘The renin-angiotensin system(RAS)has been known for more than a century as a cascade that regulates body fluid balance and blood pressure.AngiotensinⅡ(AngⅡ)has many functions in different tissues;however it is on the kidney that this peptide exerts its main functions.New enzymes,alternative routes for AngⅡformation or even active AngⅡ-derived peptides have now been described acting on AngⅡAT1or AT2receptors,or in receptors which have recently been cloned,such as Mas and AT4.Another interesting observation was that old members of the RAS,such as angiotensin converting enzyme(ACE),renin and prorenin,well known by its enzymatic activity,can also activate intracellular signaling pathways,acting as an outside-in signal transduction molecule or on the renin/(Pro)renin receptor.Moreover,the endocrine RAS,now is also known to have paracrine,autocrine and intracrine action ondifferent tissues,expressing necessary components for local AngⅡformation.This in situ formation,especially in the kidney,increases AngⅡlevels to regulate blood pressure and renal functions.These discoveries,such as the ACE2/Ang-(1-7)/Mas axis and its antangonistic effect rather than classical deleterious AngⅡeffects,improves the development of new drugs for treating hypertension and cardiovascular diseases.Fernanda M Ferr?o Lucienne S Lara Jennifer Lowe 2014World Journal of Nephrology2014,3,3:5

5Emerging role of caldesmon in cancer:A potential biomarker for colorectal cancer and other cancers显示文摘Colorectal cancer(CRC) is a devastating disease, mainly because of metastasis. As a result, there is a need to better understand the molecular basis of invasion and metastasis and to identify new biomarkers and therapeutic targets to aid in managing these tumors. The actin cytoskeleton and actin-binding proteins are known to play an important role in the process of cancer metastasis because they control and execute essential steps in cell motility and contractility as well as cell division. Caldesmon(CaD) is an actin-binding protein encoded by the CALD1 gene as multiple transcripts that mainly encode two protein isoforms: High-molecular-weight CaD, expressed in smooth muscle, and low-molecular weight CaD(l-CaD), expressed in nonsmooth muscle cells. According to our comprehensive review of the literature, CaD, particularly l-CaD, plays a key role in the development, metastasis, and resistance to chemoradiotherapy in colorectal, breast, and urinary bladder cancers and gliomas, among other malignancies. CaD is involved in many aspects of the carcinogenic hallmarks, including epithelial mesenchymal transition via transforming growth factor-beta signaling, angiogenesis, resistance to hormonal therapy, and immune evasion. Recent data show that CaD is expressed in tumor cells as well as in stromal cells, such as cancerassociated fibroblasts, where it modulates the tumor microenvironment to favor the tumor. Interestingly, CaD undergoes selective tumor-specific splicing, and the resulting isoforms are generally not expressed in normal tissues, making these transcripts ideal targets for drug design.
