| 1 | WONCA研究论文摘要汇编——抑郁症初级保健心理治疗效果:系统回顾和Meta分析显示文摘目的系统回顾抑郁症患者初级保健心理治疗有效性的现有可查证据,与一般护理或控制组进行对比,并对疗法类型和实施模式进行研究。方法通过检索MEDLINE、Embase、CENTRAL和Psyc INFO数据库,搜集对比成人抑郁症患者初级保健心理治疗与常规治疗或控制组的随机对照试验,检索时间设定为2013年12月之前。至少两人从纳入研究中进行信息提取,并进行误差风险评估。以治疗后抑郁得分作为指标进行随机效果Meta分析。结果总共30项研究符合纳入标准,涉及5 159例患者。与控制组相比,治疗结束时效果(标准平均差异)得分如下:面对面认知行为疗法(CBT)得分为-0.30〔95%CI(-0.48,-0.13)〕,面对面问题解决疗法得分为-0.14〔95%CI(-0.40,0.12)〕,面对面人际心理疗法得分为-0.24〔95%CI(-0.47,-0.02)〕,其他面对面心理干预得分为-0.28〔95%CI(-0.44,-0.12)〕,远程治疗师引导CBT得分为-0.43〔95%CI(-0.62,-0.24)〕,远程治疗师引导问题解决疗法得分为-0.56〔95%CI(-1.57,0.45)〕,自助引导CBT得分为-0.40〔95%CI(-0.69,-0.11)〕,无接触或最低限度接触CBT得分为-0.27〔95%CI(-0.44,-0.10)〕。结论心理疗法在抑郁症患者初级保健中起到有效作用。与需要大量资源的疗法相比,面对面认知行为疗法能够以更少的资源获得相似效果。 | Linde K Sigterman K Kriston L 本刊编辑部 | 2015 | 中国全科医学2015，18，10： | 9 |
