1Different profiles of cytokine expression during mild and severe acute pancreatitis显示文摘AIM:To study secretion patterns of proand anti-in-flammatory cytokines, and activation of various cellular subsets of leukocytes in peripheral blood.METHODS: We have conducted a prospective obser-vational study. One hundred and eight patients with a diagnosis of acute pancreatitis and onset of the disease within last 72 h were included in this study. The mRNA expression of 25 different types of cytokines in white blood cells was determined by quantitative real time polymerase chain reaction. Levels of 8 different cytokines in blood serum were measured by enzymelinked immunosorbent assay. Clinical data and cytokine expression results were subjected to statistical analysis.RESULTS: Severe and necrotizing acute pancreatitis (AP) is characterized by the significant depletion of circulating lymphocytes. Severe acute pancreatitis is as-sociated with a typical systemic inflammatory response syndrome and over-expression of pro-inflammatory cy-tokines [interleukin (IL)-6, IL-8, macrophage migration inhibitory factor (MIF)]. Serum IL-6 and MIF concentra-tions are the best discriminators of severe and necrotiz-ing AP as well as possible fatal outcome during the early course of the disease. CONCLUSION: Deregulation of cellular immune sys-tem is a key event leading to severe and necrotizing AP. 2Early recognition of abdominal compartment syndrome in patients with acute pancreatitis显示文摘AIM:To assess the value of widely used clinical scores in the early identification of acute pancreatitis(AP) patients who are likely to suffer from intra-abdominal hypertension(IAH)and abdominal compartment syndrome(ACS). METHODS:Patients(n=44)with AP recruited in this study were divided into two groups(ACS and non-ACS) according to intra-abdominal pressure(IAP)determined by indirect measurement using the transvesical route via Foley bladder catheter.On admission and at regular intervals,the severity of the AP and presence of organ dysfunction were assessed utilizing different multifactorial prognostic systems:Glasgow-Imrie score,Acute Physiology and Chronic Health Evaluation Ⅱ(APACHE-Ⅱ)score,and Multiorgan Dysfunction Score(MODS).The diagnostic performance of scores predicting ACS development,cut-off values and specificity and sensitivity were established using receiver operating characteristic(ROC)curve analysis. RESULTS:The incidence of ACS in our study population was 19.35%.IAP at admission in the ACS group was 22.0(18.5-25.0)mmHg and 9.25(3.0-12.4)mmHg in the non-ACS group(P<0.01).Univariate statistical analysis revealed that patients in the ACS group had significantly higher multifactorial clinical scores(APACHE Ⅱ,Glasgow-Imrie and MODS)on admission and highermaximal scores during hospitalization(P<0.01).ROC curve analysis revealed that APACHEⅡ,Glasgow-Imrie, and MODS are valuable tools for early prediction of ACS with high sensitivity and specificity,and that cut- off values are similar to those used for stratification of patients with severe acute pancreatitis(SAP). 3MicroRNAs as non-invasive diagnostic biomarkers for gastric cancer:Current insights and future perspectives显示文摘Non-invasive diagnostic biomarkers may contribute to an early identification of gastric cancer(GC) and improve the clinical management.Unfortunately,no sensitive and specific screening biomarkers are available yet and the currently available approaches are limited by the nature of the disease.GC is a heterogenic disease with various distinct genetic and epigenetic events that occur during the multifactorial cascade of carcinogenesis.Micro RNAs(mi RNAs) are commonly deregulated in gastric mucosa during the Helicobacter pylori infection and in stepwise manner from chronic gastritis,through preneoplastic conditions such as atrophic gastritis and intestinal metaplasia,to early dysplasia and invasive cancer.Identification of mi RNAs in blood in 2008 led to a great interest on mi RNA-based diagnostic,prognostic biomarkers in GC.In this review,we provide the most recent systematic review on the existing studies related to mi RNAs as diagnostic biomarkers for GC.Here,we systematically evaluate 75 studies related to differential expression of circulating mi RNAs in GC patients and provide novel view on various heterogenic aspects of the existing data and summarize the methodological differences.Finally,we highlight several important aspects crucial to improve the future translational and clinical research in the field.Alexander Link Juozas Kupcinskas 2018World Journal of Gastroenterology2018,24,30:32
4Protective action of NADPH oxidase inhibitors and role of NADPH oxidase in pathogenesis of colon inflammation in mice显示文摘AIM:To investigate the role of nicotinamide adenine dinucleotide phosphate(NADPH) oxidase in colon epithelial cells in the pathogenesis of acute and chronic colon inflammation in a mouse model of dextran sulphate sodium(DSS)-induced colitis.METHODS:Balb/c mice were divided into three groups:8 mice with acute DSS-induced colitis(3.5% DSS solution;7 d),8 mice with chronic DSS-induced colitis(3.5% DSS solution for 5 d + water for 6 d;4 cycles;total:44 d) and 12 mice without DSS supplementation as a control group.Primary colonic epithelial cells were isolated using chelation method.The cells were cultivated in the presence of mediators(lipopolysaccharide(LPS),apocynin or diphenyleneiodonium).Viability of cells was assessed by fluorescent microscopy.Production of reactive oxygen species(ROS) by the cells was measured fluorometrically using Amplex Red.Production of tumour necrosis factor-alpha(TNF-α) by the colonic epithelial cells was analysed by ELISA.Nox1 gene expression was assessed by real-time PCR.RESULTS:Our study showed that TNF-α level was increased in unstimulated primary colonic cells both in the acute and chronic colitis groups,whereas decreased viability,increased ROS production,and expression of Nox1 was characteristic only for chronic DSS colitis mice when compared to the controls.The stimulation by LPS increased ROS generation via NADPH oxidase and decreased cell viability in mice with acute colitis.Treatment with NADPH oxidase inhibitors increased cell viability and decreased the levels of ROS and TNF-α in the LPS-treated cells isolated from mice of both acute and chronic colitis groups.CONCLUSION:Our study revealed the importance of NADPH oxidase in the pathogenesis of both acute and chronic inflammation of the colon.Rima Ramonaite Jurgita Skieceviciene Simonas Juzenas Violeta Salteniene Juozas Kupcinskas Paulius Matusevicius Vilmante Borutaite Limas Kupcinskas 2014World Journal of Gastroenterology2014,20,35:4
5Fatty acids of erythrocyte membrane in acute pancreatitis patients显示文摘AIM:To evaluate changes in the fatty acid composition of erythrocyte membrane phospholipids during severe and mild acute pancreatitis(AP)of alcoholic and nonalcoholic etiology.METHODS:All consecutive patients with a diagnosis of AP and onset of the disease within the last 72 h admitted to the Hospital of Lithuanian University of Health Sciences between June and December 2007 were included.According to the Acute Physiology and ChronicHealth Evaluation(APACHEⅡ)scale,the patients were subdivided into the mild(APACHEⅡscore<7,n=22)and severe(APACHEⅡscore≥7,n=17)AP groups.Healthy individuals(n=26)were enrolled as controls.Blood samples were collected from patients on admission to the hospital.Fatty acids(FAs)were extracted from erythrocyte phospholipids and expressed as percentages of the total FAs present in the chromatogram.The concentrations of superoxide dismutase and glutathione peroxidase were measured in erythrocytes.RESULTS:We found an increase in the percentages of saturated and monounsaturated FAs,a decrease in the percentages of total polyunsaturated FAs(PUFAs)and n-3 PUFAs in erythrocyte membrane phospholipids of AP patients compared with healthy controls.Palmitic(C16:0),palmitoleic(C16:1n7cis),arachidonic(C20:4n6),docosahexaenoic(DHA,C22:6n3),and docosapentaenoic(DPA,C22:5n3)acids were the major contributing factors.A decrease in the peroxidation and unsaturation indexes in AP patients as well as the severe and mild AP groups as compared with controls was observed.The concentrations of antioxidant enzymes in the mild AP group were lower than in the control group.In severe AP of nonalcoholic etiology,the percentages of arachidic(C20:0)and arachidonic(C20:4n6)acids were decreased as compared with the control group.The patients with mild AP of nonalcoholic etiology had the increased percentages of total saturated FAs and gama linoleic acid(C18:3n6)and the decreased percentages of elaidic(C18:1n9t),eicosapentaenoic acid(EPA,C20:5n3),DPA(C22:5n3),DHA(C22:6n3)as well as total and n-3 PUFAs in erythrocyte membrane phospholipids.CONCLUSION:The composition of FAs in erythrocyte membranes is altered during AP.These changes are likely to be associated with alcohol consumption,inflammatory processes,and oxidative stress.Irma Kuliaviene Antanas Gulbinas Johannes Cremers Juozas Pundzius Limas Kupcinskas Zilvinas Dambrauskas Eugene Jansen 2013World Journal of Gastroenterology2013,19,34:3
6Laccase-catalyzed bisphenol A oxidation in the presence of 10-propyl sulfonic acid phenoxazine显示文摘The kinetics of the Coriolopsis byrsina laccase-catalyzed bisphenol A(BisA) oxidation was investigated in the absence and presence of electron-transfer mediator 3-phenoxazin-10-ylpropane-1-sulfonic acid(PPSA) at pH 5.5 and 25°C. It was shown that oxidation rate of the hardly degrading compound BisA increased in the presence of the highly reactive substrate PPSA. The increase of reaction rate depends on PPSA and BisA concentrations as well on their ratio, e.g., at0.2 mmol/L of BisA and 2 μmol/L of PPSA the rate increased 2 times. The kinetic data were analyzed using a scheme of synergistic laccase-catalyzed BisA oxidation. The calculated constant, characterizing reactivity of PPSA with laccase, is almost 1000 times higher than the constant, characterizing reactivity of BisA with laccase. 7Effects of rhamnolipid biosurfactant JBR425 and synthetic surfactant Surfynol465 on the peroxidase-catalyzed oxidation of 2-naphthol显示文摘The kinetics of the recombinant Coprinus cinereus peroxidase-catalyzed2-naphthol oxidationwas investigated in the presence of rhamnolipid biosurfactant JBR425 and synthetic surfactant Surfynol465 at pH 5.5 and25°C,with concentrations of (bio)surfactants both less than critical micelle concentrations (CMC) and larger than CMC. Itwas shown that monomers of JBR425 aswell as monomers of Surfynol465 had an enhancing effect on the conversion of2-naphthol indose response manner anddid not influence the initial rate of 2-naphthol oxidation. The resultswere accounted by a scheme,which contains a stadium of enzyme inhibition by oligomeric2- naphthol oxidation products. The action of the biosurfactant's (or synthetic surfactant's) monomerswas explained by avoidance of the enzyme active center clothingwith oligomers. Similar results havedemonstrated the potential of rhamnolipid biosurfactant JBR425due to its biodegradability. 8Atrophic gastritis and gastric cancer tissue miRNome analysis reveals hsa-miR-129-1 and hsa-miR-196a as potential early diagnostic biomarkers显示文摘BACKGROUND Gastric cancer(GC)is one of the most frequently diagnosed tumor globally.In most cases,GC develops in a stepwise manner from chronic gastritis or atrophic gastritis(AG)to cancer.One of the major issues in clinical settings of GC is diagnosis at advanced disease stages resulting in poor prognosis.Micro RNAs(mi RNAs)are small noncoding molecules that play an essential role in a variety of fundamental biological processes.However,clinical potential of mi RNA profiling in the gastric cancerogenesis,especially in premalignant GC cases,remains unclear.AIM To evaluate the AG and GC tissue mi RNomes and identify specific mi RNAs’potential for clinical applications(e.g.,non-invasive diagnostics).METHODS Study included a total of 125 subjects:Controls(CON),AG,and GC patients.All study subjects were recruited at the Departments of Surgery or Gastroenterology,Hospital of Lithuanian University of Health Sciences and divided into the profiling(n=60)and validation(n=65)cohorts.Total RNA isolated from tissue samples was used for preparation of small RNA sequencing libraries and profiled using next-generation sequencing(NGS).Based on NGS data,deregulated mi RNAs hsa-mi R-129-1-3 p and hsa-mi R-196 a-5 p were analyzed in plasma samples of independent cohort consisting of CON,AG,and GC patients.Expression level of hsa-mi R-129-1-3 p and hsa-mi R-196 a-5 p was determined using the quantitative real-time polymerase chain reaction and 2-ΔΔCt method.RESULTS Results of tissue analysis revealed 20 differentially expressed mi RNAs in AG group compared to CON group,129 deregulated mi RNAs in GC compared to CON,and 99 altered mi RNAs comparing GC and AG groups.Only 2 mi RNAs(hsa-mi R-129-1-3 p and hsa-mi R-196 a-5 p)were identified to be step-wise deregulated in healthy-premalignant-malignant sequence.Area under the curve(AUC)-receiver operating characteristic analysis revealed that expression level of hsa-mi R-196 a-5 p is significant for discrimination of CON vs AG,CON vs GC and AG vs GC and resulted in AUCs:88.0%,93.1%and 66.3%,respectively.Comparing results in tissue and plasma samples,hsa-mi R-129-1-3 p was significantly down-regulated in GC compared to AG(P=0.0021 and P=0.024,tissue and plasma,respectively).Moreover,analysis revealed that hsa-mi R-215-3 p/5 p and hsa-mi R-934 were significantly deregulated in GC based on Helicobacter pylori(H.pylori)infection status[log2 fold change(FC)=-4.52,P-adjusted=0.02;log2 FC=-4.00,P-adjusted=0.02;log2 FC=6.09,P-adjusted=0.02,respectively].CONCLUSION Comprehensive mi RNome study provides evidence for gradual deregulation of hsa-mi R-196 a-5 p and hsa-mi R-129-1-3 p in gastric carcinogenesis and found hsami R-215-3 p/5 p and hsa-mi R-934 to be significantly deregulated in H.pylori carrying GC patients.Greta Varkalaite Evelina Vaitkeviciute Ruta Inciuraite Violeta Salteniene Simonas Juzenas Vytenis Petkevicius Rita Gudaityte Antanas Mickevicius Alexander Link Limas Kupcinskas Marcis Leja Juozas Kupcinskas Jurgita Skieceviciene 2022World Journal of Gastroenterology2022,28,6:2
9Polymorphisms of micro RNA target genes IL12B, INSR, CCND1 and IL10 in gastric cancer显示文摘AIM To evaluate associations between mi RNA target genes IL12B,INSR,CCND1 and IL10 polymorphisms and gastric cancer(GC)in European population.METHODS Gene polymorphisms were analyzed in 508 controls and474 GC patients from 3 tertiary centers in Germany,Lithuania and Latvia.Controls were patients from the out-patient departments,who were referred for upper endoscopy because of dyspeptic symptoms and had no history of previous malignancy.Gastric cancer(GC)patients had histopathological verification of gastric adenocarcinoma.Genomic DNA was extracted using salting out method from peripheral blood mononuclear cells.IL12B T>G(rs1368439),INSR T>C(rs1051690),CCND1 A>C(rs7177)and IL10 T>C(rs3024498)SNPs were genotyped by the real-time polymerase chain reaction.Associations between gene polymorphism and GC were evaluated using multiple logistic regression analysis with adjustment for sex,age and country of birth.RESULTS We observed similar distribution of genotypes and allelic frequencies of all polymorphisms between GC patients and controls except of INSR rs1051690.The frequency of the T allele of INSR gene was significantly higher in GC patients than in controls(23.26%and 19.19%respectively,P=0.028).CT genotype was also more prevalent in patients compared to control group(38.48%and 30.12%respectively,P<0.021).Logistic regression analysis revealed that only one polymorphism(rs1051690 in INSR gene)was associated with increased risk of GC.Carriers of CT genotype had higher odds of GC when compared to CC genotype(OR=1.45,95%PI:1.08-1.95,P=0.01).Similar association was observed in a dominant model for INSR gene,where comparison of TT+CT vs CC genotypes showed an increased risk of GC(OR=1.44,95%PI:1.08-1.90,P=0.01).Other analyzed SNPs were not associated with the presence of GC.CONCLUSION INSR rs1051690 SNP is associated with increased risk of GC,while polymorphisms in IL12B,CCND1 and IL10genes are not linked with the presence of GC.Vytenis Petkevicius Violeta Salteniene Simonas Juzenas Thomas Wex Alexander Link Marcis Leja Ruta Steponaitiene Jurgita Skieceviciene Limas Kupcinskas Laimas Jonaitis Gediminas Kiudelis Peter Malfertheiner Juozas Kupcinskas 2017World Journal of Gastroenterology2017,23,19:2

13Endogenous motion of liver correlates to the severity of portal hypertension显示文摘BACKGROUND Degree of portal hypertension(PH)is the most important prognostic factor for the decompensation of liver cirrhosis and death,therefore adequate care for patients with liver cirrhosis requires timely detection and evaluation of the presence of clinically significant PH(CSPH)and severe PH(SPH).As the most accurate method for the assessment of PH is an invasive direct measurement of hepatic venous pressure gradient(HVPG),the search for non-invasive methods to diagnose these conditions is actively ongoing.AIM To evaluate the feasibility of parameters of endogenously induced displacements and strain of liver to assess degree of PH.METHODS Of 36 patients with liver cirrhosis and measured HVPG were included in the casecontrol study.Endogenous motion of the liver was characterized by derived parameters of region average tissue displacement signal(dantero,dretro,dRMS)and results of endogenous tissue strain imaging using specific radiofrequency signal processing algorithm.Average endogenous strainμand standard deviationσof strain were assessed in the regions of interest(ROI)(1 cm×1 cm and 2 cm×2 cm in size)and different frequency subbands of endogenous motion(0-10 Hz and 10-20 Hz).RESULTS Four parameters showed statistically significant(P<0.05)correlation with HVPG measurement.The strongest correlation was obtained for the standard deviation of strain(estimated at 0-10 Hz and 2 cm×2 cm ROI size).Three parameters showed statistically significant differences between patient groups with CSPH,but only dretro showed significant results in SPH analysis.According to ROC analysis area under the curve(AUC)of theσROI[0…10Hz,2 cm×2 cm]parameter reached 0.71(P=0.036)for the diagnosis of CSPH;with a cut-off value of 1.28μm/cm providing 73%sensitivity and 70%specificity.AUC for the diagnosis of CSPH forμROI[0…10Hz,1 cm×1 cm]was 0.78(P=0.0024);with a cut-off value of 3.92μm/cm providing 73%sensitivity and 80%specificity.Dretro parameter had an AUC of 0.86(P=0.0001)for the diagnosis of CSPH and 0.84(P=0.0001)for the diagnosis of SPH.A cut-off value of-132.34μm yielded 100%sensitivity for both conditions,whereas specificity was 80%and 72%for CSPH and SPH respectively.CONCLUSION The parameters of endogenously induced displacements and strain of the liver correlated with HVPG and might be used for non-invasive diagnosis of PH.Sigita Gelman Andrius Sakalauskas Romanas Zykus Andrius Pranculis Rytis Jurkonis Irma Kuliavienė Arūnas Lukoševičius Limas Kupčinskas Juozas Kupčinskas 2020World Journal of Gastroenterology2020,26,38:1
