2Proton pump inhibitor use increases mortality and hepatic decompensation in liver cirrhosis显示文摘BACKGROUND Proton pump inhibitors(PPIs)are widely prescribed,often without clear indications.There are conflicting data on its association with mortality risk and hepatic decompensation in cirrhotic patients.Furthermore,PPI users and PPI exposure in some studies have been poorly defined with many confounding factors.AIM To examine if PPI use increases mortality and hepatic decompensation and the impact of cumulative PPI dose exposure.METHODS Data from patients with decompensated liver cirrhosis were extracted from a hospital database between 2013 to 2017.PPI users were defined as cumulative defined daily dose(cDDD)≥28 within a landmark period,after hospitalisation for hepatic decompensation.Cox regression analysis for comparison was done after propensity score adjustment.Further risk of hepatic decompensation was analysed by Poisson regression.RESULTS Among 295 decompensated cirrhosis patients,238 were PPI users and 57 were non-users.PPI users had higher mortality compared to non-users[adjusted HR=2.10,(1.20-3.67);P=0.009].Longer PPI use with cDDD>90 was associated with higher mortality,compared to non-users[aHR=2.27,(1.10-5.14);P=0.038].PPI users had a higher incidence of hospitalization for hepatic decompensation[aRR=1.61,(1.30-2.11);P<0.001].CONCLUSION PPI use in decompensated cirrhosis is associated with increased risk of mortality and hepatic decompensation.Longer PPI exposure with cDDD>90 increases the risk of mortality.Marianne Anastasia De Roza Lim Kai Jia Wen Kam Yiong Huak Chan Andrew Kwek Tiing Leong Ang John Chen Hsiang 2019World Journal of Gastroenterology2019,25,33:6

5Proteomic and genomic studies of non-alcoholic fatty liver disease-clues in the pathogenesis显示文摘Non-alcoholic fatty liver disease(NAFLD)is a widely prevalent hepatic disorder that covers wide spectrum of liver pathology.NAFLD is strongly associated with liver inflammation,metabolic hyperlipidaemia and insulin resistance.Frequently,NAFLD has been considered as the hepatic manifestation of metabolic syndrome.The pathophysiology of NAFLD has not been fully elucidated.Some patients can remain in the stage of simple steatosis,which generally is a benign condition;whereas others can develop liver inflammation and progress into non-alcoholic steatohepatitis,fibrosis,cirrhosis and hepatocellular carcinoma.The mechanism behind the progression is still not fully understood.Much ongoing proteomic researches have focused on discovering the unbiased circulating biochemical markers to allow early detection and treatment of NAFLD.Comprehensive genomic studies have also begun to provide new insights into the gene polymorphism to understand patientdisease variations.Therefore,NAFLD is considered a complex and mutifactorial disease phenotype resulting from environmental exposures acting on a susceptible polygenic background.This paper reviewed the current status of proteomic and genomic studies that have contributed to the understanding of NAFLD pathogenesis.For proteomics section,this review highlighted functional proteins that involved in:(1)transportation;(2)metabolic pathway;(3)acute phase reaction;(4)antiinflammatory;(5)extracellular matrix;and(6)immune system.In the genomic studies,this review will discuss genes which involved in:(1)lipolysis;(2)adipokines;and(3)cytokines production.Jun Wei Lim John Dillon Michael Miller 2014World Journal of Gastroenterology2014,20,26:3

7Metabolic positron emission tomography imaging of cancer:Pairing lipid metabolism with glycolysis显示文摘The limitations of fluorine-18 fluorodeoxy-D-glucose(FDG)in detecting some cancers has prompted a longstandin search for other positron emission tomography(PET tracers to complement the imaging of glycolysis i oncology, with much attention paid to lipogenesis base on observations that the production of various lipid an lipid-containing compounds is increased in most cancers Radiolabeled analogs of choline and acetate have now been used as oncologic PET probes for over a decade showing convincingly improved detection sensitivit over FDG for certain cancers. However, neither cholin nor acetate have been thoroughly validated as lipogeni biomarkers, and while acetyl-Co A produced from acetat is used in de-novo lipogenesis to synthesize fatty acids acetate is also consumed by various other syntheti and metabolic pathways, with recent experimenta observations challenging the assumption that lipogenesi is its predominant role in all cancers. Since tumor detected by acetate PET are also frequently detected b choline PET, imaging of choline metabolism might serv as an alternative albeit indirect marker of lipogenesis particularly if the fatty acids produced in cancer cell are mainly destined for membrane synthesis throug incorporation into phosphatidylcholines. Aerobic glycolysi may or may not coincide with changes in lipid metabolism resulting in combinatorial metabolic phenotypes that ma have different prognostic or therapeutic implications Consequently, PET imaging using dual metabolic tracers in addition to being diagnostically superior to imagin with individual tracers, could eventually play a greate role in supporting precision medicine, as efforts t develop small-molecule metabolic pathway inhibitors ar coming to fruition. 14Taxonomic revisionofthegenus Lactarius(Russulales,Basidiomycota)in Korea显示文摘The genus Lactarius Pers.(Russulales)is a cosmopolitan group of Basidiomycota that forms ectomycorrhizal relationships primarily with both deciduous and coniferous trees.Although the genus has been well-studied in Europe and North America,only fragmentary researches have been carried out on Asian species.In particular,the distribution of Lactarius species in South Korea is poorly understood due to insufficient morphological descriptions and a lack of DNA sequence data.In addition,the misuse of European and North American names has added to confusion regarding the taxonomy of Asian Lactarius species.In this study,the diversity of Lactarius in South Korea was evaluated by employing both morphological and phylogenetic approaches.A multi-locus phylogenetic analysis of 729 Lactarius specimens collected between 1960 and 2017 was performed using the internal transcribed spacer(ITS)region,partial nuclear ribosomal large subunit(nrLSU),partial second largest subunit of RNA polymerase II(rpb2),and minichromosome maintenance complex component 7(mcm7).49 Lactarius species were identified in three Lactarius subgenera:L.subg.Russularia(17 spp.),L.subg.Lactarius(22 spp.),and L.subg.Plinthogalus(10 spp.).Among them,28 Lactarius species were identified as new to science,while just 17 were previously described Lactarius species.Four of the taxa remain un-named due to paucity of materials.A key to Korean Lactarius species,molecular phylogenies,a summary of diversity,and detailed description are provided.Hyun Lee Komsit Wissitrassameewong Myung Soo Park Annemieke Verbeken John Eimes Young Woon Lim 2019Fungal Diversity2019,,2:1
