| 2 | Multicentre collaborative cohort study of the use of Kirschner wires for the management of supracondylar fractures in children显示文摘Purpose:Supracondylar fractures of the humerus cause significant morbidity in children.Nerve damage and loss of fracture reduction are common recognised complications in patients with this injury.Uncertainty surrounds the optimal Kirschner wire configuration and diameter for closed reduction and pinning of these fractures.This study describes current practice and examined the association between wire configuration or diameter and outcomes(clinical and radiological)in the operative management of paediatric supracondylar fractures.Methods:Children presenting w让h GartlandⅡorⅢsupracondylar fractures at five hospitals in southwest England were eligible for inclusion.Collaborators scrutinised paper and electronic case notes.Outcome measures were maintenance of reduction and iatrogenic nerve injury.Results:Altogether 209 patients were eligible for inclusion:15.7%had a documented neurological deficit at presentation;3.9%who were neurologically intact at presentation sustained a new deficit caused by treatment and 13.4%experienced a clinically significant loss of reduction following fixation.Maintenance of reduction was significantly better in patients treated specifically with crossed×3 Kirschner wire configuration compared to all other configurations.The incidence of iatrogenic nerve injury was not significantly different between groups treated with different wire configurations.Conclusion:We present a large multicentre cohort study showing that crossed×3 Kirschner wires are associated with better maintenance of reduction than crossed×2 or lateral entry wires.Greater numbers would be required to properly investigate nerve injury relating to operative management of supracondylar fractures.We found significant variations in practice and compliance w让h the British Orthopaedic Association Standard for Trauma(BOAST)11 guidelines. | Henry Claireaux Richard Goodall Joshua Hill Elizabeth Wilson Philippa Coull Sebastian Green James Schuster-Bruce Diana Lim Joanna Miles Payam Tarassoli | 2019 | Chinese Journal of Traumatology2019,22,5: | 7 |

| 9 | Do medical students studying in the United Kingdom have an adequate factual knowledge of basic life support?显示文摘BACKGROUND: Healthcare professionals have a duty to maintain basic life support(BLS) skills. This study aims to evaluate medical students' factual knowledge of BLS and the training they receive.METHODS: A cross-sectional, closed-response questionnaire was distributed to the fi rst-and fourth-year students studying at institutions in the United Kingdom. The paper questionnaire sought to quantify respondent's previous BLS training, factual knowledge of the BLS algorithm using five multiple choice questions(MCQs), and valuate their desire for further BLS training. Students received 1 point for each correctly identifi ed answer to the 5 MCQ's.RESULTS: A total of 3,732 complete responses were received from 21 medical schools. Eighty percent(n=2,999) of students completed a BLS course as part of their undergraduate medical studies. There was a signifi cant difference(P<0.001) in the percentage of the fourth-year students selecting the correct answer in all the MCQ's compared to the fi rst-year students except in identifyingthe correct depth of compressions required during CPR(P=0.095). Overall 10.3%(95% CI 9.9% to 10.7%) of respondents correctly identified the answer to 5 MCQ's on BLS: 9% of the first-year students(n=194) and 12% of the fourth-year students(n=190). On an institutional level the proportion of students answering all MCQ's correctly ranged from 2% to 54% at different universities. Eighty-one percent of students(n=3,031) wished for more BLS training in their curriculum.CONCLUSION: Factual knowledge of BLS is poor among medical students in the UK. The numerical simulation of the geochemical response to the ventilation experiment (VE) is the object of the present paper, focusing on the transport of chloride as a conservative species and sulphate as a reactive species. Utilising the validated hydro-mechanical (HM) results from earlier steps of the DECOVALEX task, reactive and non-reactive transport models, incorporating the current understanding of the geochemistry at the site, were successfully constructed for the whole experimental period. The associated parametric and process uncertainty analyses clearly demonstrate that the basic HM understanding must be sound.
