| 1 | Management of nonalcoholic fatty liver disease:An evidence-based clinical practice review显示文摘AIM:To build a consensus among Chilean specialists on the appropriate management of patients with nonalcoholic fatty liver disease(NAFLD)in clinical practice.METHODS:NAFLD has now reached epidemic proportions worldwide.The optimal treatment for NAFLD has not been established due to a lack of evidence-based recommendations.An expert panel of members of the Chilean Gastroenterological Society and the Chilean Hepatology Association conducted a structured analysis of the current literature on NAFLD therapy.The quality of the evidence and the level of recommendations supporting each statement were assessed according to the recommendations of the United States Preventive Services Task Force.A modified three-round Delphi technique was used to reach a consensus among the experts.RESULTS:A group of thirteen experts was established.The survey included 17 open-ended questions that were distributed among the experts,who assessed the articles associated with each question.The levels of agreement achieved by the panel were 93.8%in the first round and 100%in the second and third rounds.The final recommendations support the indication of lifestyle changes,including diet and exercise,for all patients with NAFLD.Proven pharmacological therapies include only vitamin E and pioglitazone,which can be used in nondiabetic patients with biopsy-proven nonalcoholic steatohepatitis(the progressive form of NAFLD),although the long-term safety and efficacy of these therapies have not yet been established.CONCLUSION:Current NAFLD management is rapidly evolving,and new pathophysiology-based therapies are expected to be introduced in the near future.All NAFLD patients should be evaluated using a three-focused approach that considers the risks of liver disease,diabetes and cardiovascular events. | Juan P Arab Roberto Candia Rodrigo Zapata Cristián Mu?oz Juan P Arancibia Jaime Poniachik Alejandro Soza Francisco Fuster Javier Brahm Edgar Sanhueza Jorge Contreras M Carolina Cuellar Marco Arrese Arnoldo Riquelme | 2014 | World Journal of Gastroenterology2014,20,34: | 11 |

| 5 | Keratoconus therapeutics advances显示文摘Keratoconus is a progressive, usually bilateral disease of the cornea that significantly diminishes visual acuity, secondary to a progressive corneal deformity which is characterized by corneal thinning, variable degrees of irregular astigmatism and specific abnormal topographic patterns. Normally it initiates during puberty and is progressive until the third or fourth decade of life, when normally the progression rate is diminished or waned. There are multiple scales to clinically classify keratoconus. One of the most commonly used is Amsler-Krumeich and recently with the development of morphometric and aberrometric techniques, additional scales have been created that allow keratoconus to be classified according to its severity. Despite certain etiology of keratoconus remains unknown, current treatment options are available in patients with ectatic corneas and they vary depending on the severity of the disease and they include spectacles, contact lenses, intrastromal rings, keratoplasty both penetrant or lamellar, cross-linking, refractive lens exchange withintraocular lens implant, phakic intraocular lenses and the combination of these alternatives. Some authors have been using excimer laser in patients with keratoconus but the safety of the procedure is controversial. | 6 | Intertwined leukocyte balances in tumours and peripheral blood as robust predictors of right and left colorectal cancer survival显示文摘BACKGROUND Colorectal cancer(CRC)accounts for 9.4%of overall cancer deaths,ranking second after lung cancer.Despite the large number of factors tested to predict their outcome,most patients with similar variables show big differences in survival.Moreover,right-sided CRC(RCRC)and left-sided CRC(LCRC)patients exhibit large differences in outcome after surgical intervention as assessed by preoperative blood leukocyte status.We hypothesised that stronger indexes than circulating(blood)leukocyte ratios to predict RCRC and LCRC patient outcomes will result from combining both circulating and infiltrated(tumour/peritumour fixed tissues)concentrations of leukocytes.AIM To seek variables involving leukocyte balances in peripheral blood and tumour tissues and to predict the outcome of CRC patients.METHODS Sixty-five patients diagnosed with colon adenocarcinoma by the Digestive Surgery Service of the La Paz University Hospital(Madrid,Spain)were enrolled in this study:43 with RCRC and 22 with LCRC.Patients were followed-up from January 2017 to March 2021 to record overall survival(OS)and recurrence-free survival(RFS)after surgical interventions.Leukocyte concentrations in peripheral blood were determined by routine laboratory protocols.Paraffin-fixed samples of tumour and peritumoural tissues were assessed for leukocyte concentrations by immunohistochemical detection of CD4,CD8,and CD14 marker expression.Ratios of leukocyte concentration in blood and tissues were calculated and evaluated for their predictor values for OS and RFS with Spearman correlations and Cox univariate and multivariate proportional hazards regression,followed by the calculation of the receiver-operating characteristic and area under the curve(AUC)and the determination of Youden’s optimal cutoff values for those variables that significantly correlated with either RCRC or LCRC patient outcomes.RCRC patients from the cohort were randomly assigned to modelling and validation sets,and clinician-friendly nomograms were developed to predict OS and RFS from the respective significant indexes.The accuracy of the model was evaluated using calibration and validation plots.RESULTS The relationship of leukocyte ratios in blood and peritumour resulted in six robust predictors of worse OS in RCRC:CD8+lymphocyte content in peritumour(CD8pt,AUC=0.585,cutoff<8.250,P=0.0077);total lymphocyte content in peritumour(CD4CD8pt,AUC=0.550,cutoff<10.160,P=0.0188);lymphocyte-to-monocyte ratio in peritumour(LMRpt,AUC=0.807,cutoff<3.185,P=0.0028);CD8+LMR in peritumour(CD8MRpt,AUC=0.757,cutoff<1.650,P=0.0007);the ratio of blood LMR to LMR in peritumour(LMRb/LMRpt,AUC=0.672,cutoff>0.985,P=0.0244);and the ratio of blood LMR to CD8+LMR in peritumour(LMRb/CD8MRpt,AUC=0.601,cutoff>1.485,P=0.0101).In addition,three robust predictors of worse RFS in RCRC were found:LMRpt(AUC=0.737,cutoff<3.185,P=0.0046);LMRb/LMRpt(AUC=0.678,cutoff>0.985,P=0.0155)and LMRb/CD8MRpt(AUC=0.615,cutoff>1.485,P=0.0141).Furthermore,the ratio of blood LMR to CD4+LMR in peritumour(LMRb/CD4MRpt,AUC=0.786,cutoff>10.570,P=0.0416)was found to robustly predict poorer OS in LCRC patients.The nomograms showed moderate accuracy in predicting OS and RFS in RCRC patients,with concordance index of 0.600 and 0.605,respectively.CONCLUSION Easily obtainable variables at preoperative consultation,defining the status of leukocyte balances between peripheral blood and peritumoural tissues,are robust predictors for OS and RFS of both RCRC and LCRC patients. | Ramón Cantero-Cid Karla Marina Montalbán-Hernández Jenny Guevara Alejandro Pascual-Iglesias Elisa Pulido JoséCarlos Casalvilla Cristóbal Marcano Cristina Barragán Serrano Jaime Valentín Gloria Cristina Bonel-Pérez JoséAvendaño-Ortiz Verónica Terrón Roberto Lozano-Rodríguez Alejandro Martín-Quirós Elvira Marín Eva Pena Laura Guerra-Pastrián Eduardo López-Collazo Luis Augusto Aguirre | 2022 | World Journal of Gastrointestinal Oncology2022,14,1: | 1 |

| 10 | Dietary carbohydrate-to-protein ratio influences growth performance,hepatic health and dynamic of gut microbiota in atlantic salmon(Salmo salar)显示文摘Atlantic salmon(Salmo salar)fed a carbohydrate-rich diet exhibit suboptimal growth performance,along with other metabolic disturbances.It is well known that gut microbes play a pivotal role in influencing metabolism of the host,and these microbes can be modified by the diet.The main goal of the present study was to determine the effect of feeding graded levels of digestible carbohydrates to Atlantic salmon on the distal intestine digesta microbiota at 3 sampling times(i.e.,weeks 4,8 and 12),during a 12-week trial.A low carbohydrate-to-high protein diet(LC/HP,0%wheat starch),a medium carbohydrate-tomedium protein diet(MC/MP,15%wheat starch)or a high carbohydrate-to-low protein diet(HC/LP,30%wheat starch)was fed to triplicate fish tanks(27 to 28 fish per tank).We performed an in-depth characterization of the distal intestine digesta microbiota.Further,growth parameters,liver histology and the expression of genes involved in hepatic neolipogenesis in fish were measured.Fish fed a HC/LP diet showed greater hepatosomatic and viscerosomatic indexes(P=0.026 and P=0.018,respectively),lower final weight(P=0.005),weight gain(P=0.003),feed efficiency(P=0.033)and growth rate(P=0.003)compared with fish fed the LC/HP diet.Further,feeding salmon a high digestible carbohydrate diet caused greater lipid vacuolization,steatosis index(P=0.007)and expression of fatty acid synthase(fas)and delta-6 fatty acyl desaturase(d6fad)(P=0.001 and P=0.001,respectively)in the liver compared with fish fed the LC/HP diet.Although,the major impact of feeding a carbohydrate-rich diet to Atlantic salmon in beta diversity of distal intestine digesta microbiota was observed at week 4(HC/LP vs MC/MP and HC/LP vs LC/HP;P=0.007 and P=0.008,respectively)and week 8(HC/LP vs MC/MP;P=0.04),no differences between experimental groups were detected after 12 weeks of feeding.Finally,at the end of the trial,there was a negative correlation between lactic acid bacteria(LAB)members,including Leuconostoc and Lactobacillus,with hepatic steatosis level,the hepatosomatic and viscerosomatic indexes as well as the expression of fas and d6fad.Weissella showed negative correlation with hepatic steatosis level and the hepatosomatic index.Finally,further research to explore the potential use of LAB as probiotics to improve liver health in carnivorous fish fed fatty liver-induced diet is warranted. | Alejandro Villasante Carolina Ramírez Hector Rodríguez Patricio Dantagnan Adrian Hernandez Elías Figueroa Jaime Romero | 2022 | Animal Nutrition2022,,3: | 1 |

Statisticaltreatment is applied to data obtained and the results are analyzed. Results: The expense per patient during the period ofDecember/15-May/16 was statistically less than the one during the same period on the year before (December/14-May/15), having astate dispensing saving per patient of US$ 1.34 per month. And a saving per patient in the pharmaceutical care is approximatelyUS$ 2.82 per month. | 17 | Unmanned Aerial Vehicle Multi-Access Edge Computing as Security Enabler for Next-Gen 5G Security Frameworks显示文摘5G/Beyond 5G(B5G)networks provide connectivity to many heterogeneous devices,raising significant security and operational issues and making traditional infrastructure management increasingly complex.In this regard,new frameworks such as Anastacia-H2020 or INSPIRE-5GPlus automate the management of next-generation infrastructures,especially regarding policy-based security,abstraction,flexibility,and extensibility.This paper presents the design,workflow,and implementation of a security solution based on Unmanned Aerial Vehicles(UAVs),able to extend 5G/B5G security framework capabilities with UAV features like dynamic service provisioning in specific geographic areas.The proposed solution allows enforcing UAV security policies in proactive and reactive ways to automate UAV dynamic deployments and provisioning security Virtual Network Functions(VNFs)in the onboard Multi-access Edge Computing(MEC)node.A UAV has been ensembled from scratch to validate the proposal,and a raspberry-pi has been onboarded as compute node.The implementation provides a VNF for dynamic UAV management,capable of dynamically loading waypoints into the flight controller to address reactive autonomous flights,and anMLbased VNF capable of detecting image patterns.According to the security policies,the onboard VNFs can be dynamically configured to generate alerts to the framework and apply local reactions depending on the detection made.In our experiments,we measured the time it takes for the solution to be ready after receiving a security policy for detecting patterns in a specific geographical area.The time it takes for the solution to react automatically was also measured.The results show that the proactive flow configuration considering ten waypoints can be enforced in less than 3 s,and a local reactive flow can be enforced in around 1 s.We consider that the results are promising and aligned with other security enabler solutions as part of existing 5G/6G security frameworks. | Jaime Ortiz Córdoba Alejandro Molina Zarca Antonio Skármeta | 2023 | Intelligent Automation & Soft Computing2023,37,8: | 0 |

| 20 | Detection of KRAS G12D in colorectal cancer stool by droplet digital PCR显示文摘AIM To assess KRAS G12 D mutation detection by droplet digital PCR(dd PCR) in stool-derived DNA from colorectal cancer(CRC) patients.METHODS In this study, tumor tissue and stool samples were collected from 70 patients with stage Ⅰ-Ⅳ CRC diagnosed by preoperative biopsy. KRAS mutational status was determined by pyrosequencing analysis of DNA obtained from formalin-fixed paraffinembedded(FFPE) tumor tissues. The KRAS G12 D mutation was then analyzed by dd PCR in FFPE tumors and stool-derived DNA from patients with this point mutation. Wild-type(WT) tumors, as determined by pyrosequencing, were included as controls; analysis of FFPE tissue and stool-derived DNA by dd PCR was performed for these patients as well.RESULTS Among the total 70 patients included, KRAS mutations were detected by pyrosequencing in 32(45.71%), whereas 38(54.29%) had WT tumors. The frequency of KRAS mutations was higher in left-sided tumors(11 located in the right colon, 15 in the left, and 6 in the rectum). The predominant point mutation was KRAS G12 D(14.29%, n = 10), which was more frequent in early-stage tumors(I-IIA, n = 7). In agreement with pyrosequencing results, the KRAS G12 D mutation was detected by dd PCR in FFPE tumor-derived DNA, and only a residual number of mutated copies was found in WT controls. The KRAS G12 D mutation was also detected in stool-derived DNA in 80% of all fecal samples from CRC patients with this point mutation. CONCLUSION dd PCR is a reliable and sensitive method to analyze KRAS G12 D mutation in stool-derived DNA from CRC patients, especially at early stages.
