| 1 | Comparison of visual acuity outcomes between ranibizumab and bevacizumab treatment in neovascular age-related macular degeneration显示文摘AIM:To compare visual acuity(VA) outcomes between intravitreal injection of bevacizumab and ranibizumab in the treatment of neovascular age-related macular degeneration(AMD).· METHODS:We conducted a consecutive,retrospective case series study in patients with newly diagnosed all type choroidal neovascularization(CNV) secondary to AMD who received an intravitreal injection of bevacizumab(1.25mg) or ranibizumab(0.3mg) at Lions Eye Institute,Western Australia from Mar.2006 to May 2008.All patients received injection at baseline with additional monthly injections given at the discretion of the treating physician.Main outcome measures were changes in VA.· RESULTS:There were 371 consecutive patients received injection at least in one eye with at least 6 months of follow up(median of 12.0 months).Bevacizumab treatment prevented 221 out of 278(79.5%) patients from losing <15 letters in VA compared with 79 out of 93(84.9%) of ranibizumab treated patients(P=0.25).While 68(24.5%) of bevacizumab treated patients gained ≥15 letters of VA compared with 24(25.8%) of ranibizumab treated patients(P=0.79).75.3% and 66.2% patients benefited from ranibizumab and bevacizumab respectively with final VA better than 6/60(P=0.10).Multivariate analysis showed that pre-treatment VA was negatively associated with benefit outcome.Assignment of injection was not associated with VA outcome of benefit after adjusting the covariate(P=0.857).CONCLUSION:There are no difference in treatment efficacy in terms of VA between bevacizumab and ranibizumab in routine clinical condition. | Ian J Constable Ian L McAllister Timothy Isaacs | 2011 | International Journal of Ophthalmology(English edition)2011，4，1： | 3 |
| 2 | Diabetes mellitus, revascularization and outcomes in elderly patients with myocardial infarction-related cardiogenic shock显示文摘Background The prognostic role of diabetes mellitus(DM)in elderly patients with myocardial infarction-related cardiogenic shock(MI-CS)remains controversial.Little information exists about the impact of intensive cardiac care unit(ICCU)and revascularization on outcomes of elderly patients with MI-CS.We aimed to assess the prognostic impact of DM according to age in patients with MI-CS,and to analyze the impact ICCU management and revascularization on in-hospital mortality in MI-CS patients at older ages.Methods Discharge episodes with diagnosis of CS associated with MI were selected from the Spanish National Health System’s Basic Data Set.Centers were classified according to their availability of ICCU.Main outcome measured was in-hospital mortality.Results A total of 23,590 episodes of MI-CS were identified,of whom 12,447(52.8%)were in patients aged≥75 years.The impact of DM on in-hospital mortality was different among age subgroups.While in younger patients,DM was associated to a higher mortality risk(0.52 vs.0.47,OR=1.12,95%CI:1.06–1.18,χ^2<0.001),this association became non-significant in older patients(0.76 vs.0.81,χ^2=0.09).Adjusted mortality rate of MI-CS aged≥75 years was lower in patients admitted to hospitals with ICCU(adjusted mortality rate:74.2%vs.77.7%,P<0.001)and in patients undergoing revascularization(74.9%vs.77.3%,P<0.001).Conclusions Prognostic impact of DM in patients with MI-CS was different according to age,with a significantly lower impact at older ages.The availability of ICCU and revascularization were associated with better outcomes in these complex patients. | Miquel Gual Albert Ariza-Solé María García Márquez Cristina Fernández JoséL Bernal Francesc Formiga María-Isabel Barrionuevo JoséC Sánchez-Salado Victòria Lorente Júlia Pascual Isaac Llaó Oriol Alegre Angel Cequier Javier Elola | 2020 | Journal of Geriatric Cardiology2020，17，10： | 2 |
| 3 | Diagnostic and therapeutic role of endoscopic retrograde cholangiopancreatography in biliary rhabdomyosarcoma显示文摘Biliary rhabdomyosarcoma (BRMS) is an uncommon childhood malignancy which has been managed surgically. We present a case of a 3-year-old boy with BRMS, in whom endoscopic retrograde cholangiopancreatography (ERCP) was successfully used both diagnostically and therapeutically, thus obviating the need for surgery and its attendant risks of morbidity and mortality.
