2Separate lateral parametrial lymph node dissection improves detection rate of parametrial lymph node metastasis in early-stage cervical cancer: 10-year clinical evaluation in a single center in China显示文摘Objective: To investigate the clinical significance of separate lateral parametrial lymph node dissection(LPLND) in improving parametrial lymph node(PLN) and its metastasis detection rate during radical hysterectomy for early-stage cervical cancer.Methods: From July 2007 to August 2017, 2,695 patients with cervical cancer in stage IB1-IIA2 underwent radical hysterectomy were included. Of these patients, 368 underwent separate dissection of PLNs using the LPLND method, and 2,327 patients underwent conventional radical hysterectomy(CRH). We compared the surgical parameters, PLN detection rate and PLN metastasis rate between the two groups.Results: Compared with CRH group, the rate of laparoscopic surgery was higher(60.3% vs. 15.9%, P<0.001),and the blood transfusion rate was lower(19.0% vs. 29.0%, P<0.001) in the LPLND group. PLNs were detected in 356 cases(96.7%) in the LPLND group, and 270 cases(11.6%) in the CRH group(P<0.001), respectively. The number of PLNs detected in the LPLND group was higher than that in the CRH group(median 3 vs. 1, P<0.001).The PLN metastases were detected in 25 cases(6.8%) in the LPLND group, and 18 cases(0.8%) in the CRH group(P<0.001), respectively. In multivariable analysis, LPLND is an independent factor not only for PLN detection [odds ratio(OR)=228.999, 95% confidence interval(95% CI): 124.661-420.664;P<0.001], but also for PLN metastasis identification(OR=10.867, 95% CI: 5.381-21.946;P<0.001).Conclusions: LPLND is feasible and safe. 3Limited energy parametrial resection/dissection during modified laparoscopic nerve-sparing radical hysterectomy显示文摘Objective: The aim of this study was to investigate the feasibility of using ultrasonic scalpel combined with vascular clip in parametrial management,called limited energy parametrial resection/dissection(LEPRD),in laparoscopic nerve plane-sparing radical hysterectomy(NPSRH),a modified nerve-sparing radical hysterectomy(NSRH); and to evaluate its effectiveness in pelvic autonomic nerve preservation.Methods: From July 2012 to January 2016,257 consecutive patients with stage IB1 to IIA2 cervical cancer who underwent NPSRH were included in this study.Patients were divided into three cohorts according to the different parametrial resection modality.The clinical,pathological and surgery-related parameters were compared between the three groups.Short-and long-term postoperative bladder functions were evaluated.Results: LEPRD was attempted in 94 patients,and was successful in 65(69.1%) patients(LEPRD group).The remaining 29(30.9%) patients required bipolar coagulation after failure of vascular clipping(combined modality group).Routine bipolar cautery was used in the other 163 patients during the parametrial resection(bipolar group).The blood loss in the LEPRD group was significantly lower than those in the other two groups(P<0.001).The rate of successful Foley removal on postoperative day 7 was significantly higher in the LEPRD group than in the bipolar group(P=0.022).The incidence of chronic voiding dysfunction was significantly lower in the LEPRD group than in the bipolar group(P=0.019).Conclusions: It is feasible to perform LEPRD in NPSRH for cervical cancers.This kind of limited energy surgical technique is associated with less blood loss,and leads to improved postoperative bladder function.Dan Zhao Bin Li Yating Wang Shuanghuan Liu Yanan Zhang Gongyi Zhang 2018Chinese Journal of Cancer Research2018,30,6:6
4Computation method of zero-stress state of pneumatic stressed membrane structure显示文摘The whole analysis process of pneumatic stressed membrane structure contains nine states and seven analysis processes.The zero-stress state is the corner-stone of analysis and design of pneumatic stressed structure,and has significant impact on the pre-stressed state and load state.According to the logical model of the whole numerical analysis process of pneumatic stressed structure,a numerical analysis method to solve the zero-stress state from the elasticized equilibrium state was firstly proposed,called linear compatibility matrix M-P inverse method.Firstly,the pneumatic membrane stressed structure was transferred into grid structure by using membrane link to simulate membrane surface.Secondly,on the basis of equilibrium matrix theory of pin joint structure and small deformation assumption,compatibility equation of system was established.Thirdly,the unstressed length and elongation of links were calculated from the tension and material parameters of elasticized equilibrium state.Finally,using compatibility matrix M-P inverse,the nodal displacement was calculated by solving compatibility equation,the configuration of zero-stress state could be obtained through reverse superposition,and the stress was released.According to the algorithm,the program was coded with MATLAB.The correctness and efficiency of this method were verified by several numerical examples,and it could be found that one elasticized equilibrium state corresponded to one configuration of the zero-stress state.The work has theoretical significance and practical guidance value for pneumatic membrane structural design.ZHAO JunZhao CHEN WuJun FU GongYi LI RuiXiong 2012Science China(Technological Sciences)2012,55,3:4
5Fabrication and properties of ultraviolet photo-detectors based on SiC nanowires显示文摘A new type of ultraviolet photo-detectors (UVPDs) based on a bundle of highly aligned SiC nanowires was fabricated and the photo-electric properties of the UVPDs including I-V characteristics and time response were studied in this work. SiC nanowires were prepared by pyrolysis of a polymer precursor with ferrocene as the catalyst by a CVD route. The diameters of SiC nanowires varied from 100 to 200 nm while they were some centimeters long and the SiC nanowires were with zinc blended cubic form ( -SiC) tested by X-ray diffraction. A bundle of nanowires was fixed onto two legs' base by conductive silver paste to form the UVPDs. The electrical measurement of the device showed a significant increase of current when the device was exposed to 254 nm UV light, and the rising time of the device is very short, but the falling time is relatively long. 9The basis of a more contagious 501Y.V1 variant of SARS-CoV-2显示文摘Dear Editor,The SARS-CoV-2 virus has infected over one hundred million people(COVID-19 patients)and caused more than two million deaths to date.The number of infected people continues to grow quickly,emphasizing the need for rapid use of effective vaccines.Although two mRNA vaccines based on Spike protein(produced by Pfizer-BioNTech and MODERNA)have been approved for emergency use in the US,1,2 the increasing Spike variants that have appeared around the world raise concerns about the continued efficacy of the vaccines.3 Monoclonal antibodies,developed by Regeneron and Eli Lilly,specifically targeting the native form of Spike have been approved by the FDA for emergency use.4,5 An N501Y variant(Y501)of the Spike protein of SARS-CoV-2(B.1.1.7,20I/501Y.V1),first emerged in UK and has now spread to the rest of the world.This variant appears to be much more contagious than the original N501 version.3 Furthermore,Y501 mutation is also found in a variant(B.1.351,20H/501Y.V2)from South Africa and a variant(P1,20J/501Y.V3)from Brazil.3 Unfortunately,this mutation is located at the interaction surface between the RBD and human Angiotensin Converting Enzyme 2(ACE2).6 Thus,the Y501 variation present in B.1.1.7,20I/501Y.V1 might affect the binding ability of the RBD to bind ACE2.We therefore compared the binding affinity of N501 and Y501 RBD for ACE2,uncovering that the affinity for ACE2 of the Y501-RBD was~10 fold higher than that of the N501 version.This may account,at least in part,for the greater infectivity of SARS-CoV-2 with this mutation.Structural modeling data showed that Y501-RBD can form an additional aromatic ring–ring interaction and an additional hydrogen bond with ACE2 by comparison with the RBD of the wild type.In spite of this,sera from individuals immunized with the Pfizer-BioNTech vaccine still efficiently block ACE2 binding to Y501-RBD.Furthermore,Bamlanivimab,the recently FDA approved therapeutic antibody drug for treatment of COVID-19 patients4 still binds the variant Y501-RBD as efficiently as it binds the N501-RBD,giving hope that treatment with the existing monoclonal antibodies may still help COVID-19 patients.Haolin Liu Qianqian Zhang Pengcheng Wei Zhongzhou Chen Katja Aviszus John Yang Walter Downing Chengyu Jiang Bo Liang Lyndon Reynoso Gregory PDowney Stephen KFrankel John Kappler Philippa Marrack Gongyi Zhang 2021Cell Research2021,31,6:1
