1口腔护理对危重症患者口腔健康状况和呼吸机相关性肺炎预防的影响显示文摘目的:本研究的目的是明确口腔护理措施对重症监护室患者口腔健康状况和呼吸机相关性肺炎发生率的影响。方法:本临床实验纳入2015-2016年间的100名患者,随机分为对照组(接受护士的口腔护理)和干预组(接受研究人员的系统口腔护理)。根据干预组患者的口腔健康状况,按照Beck口腔护理频次评价表(BOAS)确定需要接受口腔护理的频次。每次护理均包括调整气管内套管压力、用牙膏刷牙、使用抗菌药和嘴唇保湿。口腔检查采用BOAS和口腔黏膜菌斑量表评分,肺炎监测采用临床肺部感染评分。结果:第1~5天,两组患者的BOAS评分有统计学差异(P<0.001);第3~5天,两组患者的口腔黏膜菌斑评分有统计学差异(P<0.001);第3天和第5天,对照组肺炎发生率分别为10%(5例)和14%(7例),干预组肺炎发生率分别为4%(2例)和10%(5例),Fisher检验无统计学差异(P=0.538),但与对照组相比,干预组肺炎的发生率降低。结论:与传统的口腔护理措施比较,系统口腔护理虽然没有显著降低危重症患者呼吸机相关性肺炎的发生率,但显著改善了其口腔健康状况和口腔黏膜菌斑评分。Abdullah Haghighi Vida Shafipour Masoumeh Bagheri-Nesami Afshin Gholipour Baradari Jamshid Yazdani Charati 2017中国护理管理2017,17,3:14
2Applications of thin-film lithium niobate in nonlinear integrated photonics显示文摘Photonics on thin-film lithium niobate(TFLN)has emerged as one of the most pursued disciplines within integrated optics.Ultracompact and low-loss optical waveguides and related devices on this modern material platform have rejuvenated the traditional and commercial applications of lithium niobate for optical modulators based on the electro-optic effect,as well as optical wavelength converters based on secondorder nonlinear effects,e.g.,second-harmonic,sum-,and difference-frequency generations.TFLN has also created vast opportunities for applications and integrated solutions for optical parametric amplification and oscillation,cascaded nonlinear effects,such as low-harmonic generation;third-order nonlinear effects,such as supercontinuum generation;optical frequency comb generation and stabilization;and nonclassical nonlinear effects,such as spontaneous parametric downconversion for quantum optics.Recent progress in nonlinear integrated photonics on TFLN for all these applications,their current trends,and future opportunities and challenges are reviewed.Milad Gholipour Vazimali Sasan Fathpour 2022Advanced Photonics2022,4,3:6
3In vitro assay for the anti-brucella activity of medicinal plants against tetracycline-resistant Brucella melitensis显示文摘Brucellosis, a zoonosis caused by four species of brucella, has a high morbidity. Brucella melitensis is the main causative agent of brucellosis in both human and small ruminants. As an alternative to conventional antibiotics, medicinal plants are valuable resources for new agents against antibiotic-resistant strains. The aim of this study was to investigate the usage of native plants for brucellosis treatment. For this purpose, the anti-brucella activities of ethanolic and methanolic extracts of Salvia sclarea, Oliveria decumbens, Ferulago angulata, Vitex pseudo-negundo, Teucrium polium, Plantago ovata, Cordia myxa, and Crocus sativus were assessed. The activity against a resistant Br. melitensis strain was determined by disc diffusion method at various concentrations from 50-400 mg/ml. Antibiotic discs were also used as a control. Among the evaluated herbs, six plant (Salvia sclarea, Oliveria decumbens, Ferulago angulata, Vitex pseudo-negundo, Teucrium polium, and Crocus sativus) showed anti-brucella activity. Oliveria de- cumbens was chosen as the most effective plant for further studies. A tested isolate exhibited resistance to tetracycline, nafcillin, oxacillin, methicillin, and colistin. Minimal inhibitory concentration (MIC) and minimal bactericidal concentration (MBC) values for Oliveria decumbens against resistant Br. melitensis were the same (5 mg/ml), and for gentamicin they were both 2 mg/ml. Time-kill kinetics for a methanolic extract of Oliveria decumbens was 7 h whereas for an ethanolic extract it was 28 h. Also, Oliveria decumbens extracts showed a synergistic effect in combination with doxycycline and tetracycline. In general, the similar values of MIC and MBC for Oliveria decumbens suggest that these extracts could act as bactericidal agents against Br. melitensis. 4Methane hydrate stability in the presence of water-soluble hydroxyalkyl cellulose显示文摘The effect of low-dosage water-soluble hydroxyethyl cellulose (approximate MW~90,000 and 250,000) as a member of hydroxyalkyl cellulosic polymer group on methane hydrate stability was investigated by monitoring hydrate dissociation at pressures greater than atmospheric pressure in a closed vessel. In particular, the influence of molecular weight and mass concentration of hydroxyethyl cellulose (HEC) was studied with respect to hydrate formation and dissociation. Methane hydrate formation was performed at 2℃ and at a pressure greater than 100 bar. Afterwards, hydrate dissociation was initiated by step heating from -10℃ at a mild pressure of 13 bar to 3℃, 0℃ and 2℃. With respect to the results obtained for methane hydrate formation/dissociation and the amount of gas uptake, we concluded that HEC 90,000 at 5000 ppm is suitable for long-term gas storage and transportation under a mild pressure of 13 bar and at temperatures below the freezing point.M. 5Oxidative coupling of methane over (Na_2WO_4+Mn or Ce)/SiO_2 catalysts:In situ measurement of electrical conductivity显示文摘The effects of manganese oxide or ceria promoters on the performance of Na2WO4/SiO2 catalysts for oxidative coupling of methane (OCM) are reported. The OCM reaction was performed in a continuous-flow microreactor at 800 ℃, atmospheric pressure and under GHSV = 13200 ml gC-1at h-1. Catalysts were characterized by in situ conductivity measurement, FT-IR spectroscopy, XRD, SEM and temperature programmed reduction analysis. Manganese oxide promoted Na2WO4/SiO2 is considered as one of the active and selective catalysts for OCM reaction. Ceria with high oxygen storage capacity is selected as a proper oxygen activator, providing a higher concentration of the oxy-anion species which is suitable for OCM reaction and compared with manganese oxide. Electrical conductivity of the catalysts was measured in OCM reaction under oxidizing atmosphere, i.e. in the absence of methane. It was found that the trimetallic catalysts, i.e. the catalysts having sodium, tungsten and Mn or Ce species, exhibited similar crystalline structures and morphologies, which lead to suitable bulk properties for the formation of an active and selective catalyst. However, tungsten had significant effect on the texture and redox properties of the catalysts. It was also shown that the crystalline structure of the bimetallic (Na+Mn or Ce)/SiO2 samples was quite different. This reveals that the metal oxides have significant effect on the extent of crystallization, taking place in the course of interaction of sodium with silica support. 6Evaluation of preventable trauma death in emergency department of Imam Reza hospital显示文摘BACKGROUND: Trauma is considered as a worldwide problem despite socio-economic development. Motor vehicle accidents(MVAs) are the most important cause of trauma. Trauma related deaths are mostly preventable. This study aimed to investigate the causes and prevention of death in trauma patients.METHODS: This retrospective, descriptive-analytic study assessed 100 trauma patients referred to our emergency department(ED) from January 2013 to Januanry 2015. The included patients were those with trauma died after arrival at our ED. Age, sex, cause of trauma, clinical causes of death, causes of death def ined by autopsy, way of transfer to the ED, time of ambulance arrival at the scene of trauma, and time elapsed to enter the ED from the scene of trauma were studied.RESULTS: In the 100 patients, 21(21%) patients were female and 79(79%) male. Fortythree patients were older than 60 years. Trauma was largely due to pedestrian accidents in 31% of the patients, and 33% had a hypo-volemic shock. About 80% of deaths were due to intra-cranial hemorrhage(ICH) or intra-ventricular hemorrhage(IVH), and spinal injuries were not preventable. Autopsy revealed that 28% of the patients suffered from internal injuries. Autopsy revealed that 19% of the deaths were not preventable and 81% were considered preventable. In our patients, 76 were transferred to the hospital by emergency medicine services(EMS). Analysis of time for ambulance arrival to the scene and frequency of death revealed that 52.2% of the deaths occurred between 11 and 15 minutes. Analysis of time for admission to the ED from the scene of trauma showed that 74.6% deaths occurred between 6 and 10 minutes.CONCLUSIONS: The rate of hospital preventable deaths is about 80%, a high mortality rate, which denotes a lack of proper diagnosis and treatment. 11Evaluation of the risk factors associated with emergency department boarding:A retrospective cross-sectional study显示文摘Purpose Boarding is a common problem in the emergency department(ED)and is associated with poor health care and outcome.Imam Khomeini Hospital is the main healthcare center in Urmia,a metropolis in the northwest of Iran.Due to the overcrowding and high patient load,we aim to characterize the rate,cause and consequence of boarding in the ED of this center.Methods All medical records of patients who presented to the ED of Imam Khomeini Hospital from August 1,2017 to August 1,2018 were retrospectively analyzed.Patients with uncompleted records were excluded.Boarding was defined as the inability to transfer the admitted ED patients to a downstream ward in≥2 h after the admission order.Demographic data,boarding rate,mortality and triage levels(1–5)assessed by emergency severity index were collected and analyzed.The first present time of patients was classified into 4 ranges as 0:00–5:59,6:00–11:59,12:00–17:59 and 18:00–23:59.Descriptive,parametric and non-parametric statistical tests were performed and the risk of boarding was determined by Pearson Chi-square test.Results Demographic data analysis showed that 941(58.5%)male and 667(41.5%)female,altogether 1608 patients were included in this study.Five patients(0.3%)died.The distribution of patients with the triage levels 1–5 was respectively 79(4.9%),1150(71.5%),374(23.3%),4(0.2%)and 0(0%).Most patients were of level 2.Only 75(4.7%)patients required intensive care.The majority of patients(84.2%)were presented at weekdays.The maximum patient load was observed between 12:00–17:59.Of the 1608 patients,340(21.1%)experienced boarding within a mean admission time of 13.70 h.Among the 340-boarded patients,20.1%belonged to surgery,12.1%to orthopedics,10.9%to neurosurgery and 10.3%to neurology.The boarding rate was higher in females,patients requiring intensive care and those with low triage levels.Compared with the non-boarded,the boarded patients had a higher mean age.Conclusion The boarding rate is higher in the older and female patients.Moreover,boarding is dependent on the downstream ward sections:patients requiring surgical management experience the maximum boarding rate.Yousef Nouri Changiz Gholipour Javad Aghazadeh Shahriar Khanahmadi Talayeh Beygzadeh Danial Nouri Mehryar Nahaei Reza Karimi Elnaz Hosseinalipour 2020Chinese Journal of Traumatology2020,23,6:1

14Robust Control of Robotic Manipulators in the Task-Space Using an Adaptive Observer Based on Chebyshev Polynomials显示文摘In this paper,an adaptive observer for robust control of robotic manipulators is proposed.The lumped uncertainty is estimated using Chebyshev polynomials.Usually,the uncertainty upper bound is required in designing observer-controller structures.However,obtaining this bound is a challenging task.To solve this problem,many uncertainty estimation techniques have been proposed in the literature based on neuro-fuzzy systems.As an alternative,in this paper,Chebyshev polynomials have been applied to uncertainty estimation due to their simpler structure and less computational load.Based on strictly-positive-rea Lyapunov theory,the stability of the closed-loop system can be verified.The Chebyshev coefficients are tuned based on the adaptation rules obtained in the stability analysis.Also,to compensate the truncation error of the Chebyshev polynomials,a continuous robust control term is designed while in previous related works,usually a discontinuous term is used.An SCARA manipulator actuated by permanent magnet DC motors is used for computer simulations.Simulation results reveal the superiority of the designed method.GHOLIPOUR Reza FATEH Mohammad Mehdi 2020Journal of Systems Science & Complexity2020,33,5:1
