| 1 | Sedation and analgesia in gastrointestinal endoscopy: What’s new?显示文摘Various types of sedation and analgesia technique have been used during gastrointestinal endoscopy procedures.The best methods for analgesia and sedation during gastrointestinal endoscopy are still debated.Providing an adequate regimen of sedation/analgesia might be considered an art,influencing several aspects of endoscopic procedures: the quality of the examination,the patient’s cooperation and the patient’s and physician’s satisfaction with the sedation.The properties of a model sedative agent for endoscopy would include rapid onset and offset of action,analgesic and anxiolytic effects,ease of titration to desired level of sedation,rapid recovery and an excellent safety prof ile.Therefore there is an impulse for development of new approaches to endoscopic sedation.This article provides an update on the methods of sedation today available and future directions in endoscopic sedation. | Lorella Fanti Pier Alberto Testoni | 2010 | World Journal of Gastroenterology2010,16,20: | 12 |
| 2 | Debris flow hazard assessment by means of numerical simulations:implications for the Rotolon creek valley(Northern Italy)显示文摘On 4^(th) November 2010, a debris flow detached from a large debris cover accumulated above the lowermost portion of the Rotolon landslide(Vicentine Pre-Alps, NE Italy) and channelized in the valley below within the Rotolon Creek riverbed. Such event evolved into a highly mobile and sudden debris flow, damaging some hydraulic works and putting at high risk four villages located along the creek banks. A monitoring campaign was carried out by means of a ground based radar interferometer(GB-In SAR) to evaluate any residual displacement risk in the affected area and in the undisturbed neighbouring materials.Moreover, starting from the current slope condition, a landslide runout numerical modelling was performed by means of DAN-3D code to assess the impacted areas, flow velocity, and deposit distribution of the simulated events. The rheological parameters necessary for an accurate modelling were obtained through the back analysis of the 2010 debris flow event. Back analysis was calibrated with all of the available terrain data coming from field surveys and ancillary documents, such as topographic,geomorphological and geological maps, with pre-and post-event Li DAR derived DTMs, and with orthophotos. Finally, to identify new possible future debris flow source areas as input data for the new modelling, all the obtained terrain data were reanalysed and integrated with the GB-In SAR displacement maps; consequently, new simulations were made to forecast future events. | 5 | Successful multidisciplinary clinical approach and molecular characterization by whole transcriptome sequencing of a cardiac myxofibrosarcoma: A case report显示文摘BACKGROUND Cardiac tumors are rare and complex entities.Surgery represents the cornerstone of therapy,while the role of adjuvant treatment remains unclear and,in case of relapse or metastatic disease,the prognosis is very poor.Lack of prospective,randomized clinical trials hinders the generation of high level evidence for the optimal diagnostic workup and multimodal treatment of cardiac sarcomas.Herein,we describe the multidisciplinary clinical management and molecular characterization of a rare case of cardiac myxofibrosarcoma in an elderly woman.CASE SUMMARY A 73-year-old woman presented signs and symptoms of acute left-sided heart failure.Imaging examination revealed a large,left atrial mass.With suspicion of a myxoma,she underwent surgery,and symptoms were promptly relieved.Histology showed a cardiac myxofibrosarcoma,a rare histotype of cardiac sarcoma.Eight months later,disease unfortunately relapsed,and after a multidisciplinary discussion,a chemotherapy with doxorubicin and then gemcitabine was started,achieving partial radiologic and complete metabolic response,which was maintained up to 2 years and is still present.This report is focused on the entire clinical path of our patient from diagnosis to follow-up,through surgery and strategies adopted at relapse.Moreover,due to their rarity,very little is known about the molecular landscape of myxofibrosarcomas.Thus,we also performed and described preliminary genome analysis of the tumor tissue to get further insight on mechanisms involved in tumor growth,and to possibly unveil new clinically actionable targets.CONCLUSION We report a case of cardiac myxofibrosarcoma that achieved a very good prognosis due to an integrated surgical,cardiac and oncologic treatment strategy. | Maristella Saponara Valentina Indio Carmine Pizzi Elena-Daniela Serban Milena Urbini Annalisa Astolfi Pasquale Paolisso Sofia Martin Suarez Margherita Nannini Davide Pacini Valentina Agostini Ornella Leone Valentina Ambrosini Giuseppe Tarantino Stefano Fanti Fabio Niro Francesco Buia Domenico Attinà Maria Abbondanza Pantaleo | 2019 | World Journal of Clinical Cases2019,7,19: | 2 |
