| 1 | Management of granulomatous lobular mastitis: an international multidisciplinary consensus(2021 edition)显示文摘Granulomatous lobular mastitis(GLM) is a rare and chronic benign inflammatory disease of the breast. Difficulties exist in the management of GLM for many front-line surgeons and medical specialists who care for patients with inflammatory disorders of the breast. This consensus is summarized to establish evidence-based recommendations for the management of GLM. Literature was reviewed using PubMed from January 1, 1971 to July 31, 2020. Sixty-six international experienced multidisciplinary experts from 11 countries or regions were invited to review the evidence.Levels of evidence were determined using the American College of Physicians grading system, and recommendations were discussed until consensus. Experts discussed and concluded 30 recommendations on historical definitions,etiology and predisposing factors, diagnosis criteria, treatment, clinical stages, relapse and recurrence of GLM. GLM was recommended as a widely accepted definition. In addition, this consensus introduced a new clinical stages and management algorithm for GLM to provide individual treatment strategies. In conclusion, diagnosis of GLM depends on a combination of history, clinical manifestations, imaging examinations, laboratory examinations and pathology.The approach to treatment of GLM should be applied according to the different clinical stage of GLM. | 2 | Single hepatocellular carcinoma ≤ 3 cm in left lateral segment:Liver resection or radiofrequency ablation?显示文摘AIM:To evaluate the long-term results of radiofrequency ablation(RFA)compared to left lateral sectionectomy(LLS)in patients with Child-Pugh class A disease for the treatment of single and small hepatocellular carcinoma(HCC)in the left lateral segments.METHODS:We retrospectively reviewed the data of133 patients with single HCC(≤3 cm)in their left lateral segments who underwent curative LLS(n=66)or RFA(n=67)between 2006 and 2010.RESULTS:The median follow-up period was 33.5mo in the LLS group and 29 mo in the RFA group(P=0.060).Most patients had hepatitis B virus-related HCC.The hospital stay was longer in the LLS group than in the RFA group(8 d vs 2 d,P<0.001).The 1-,2-,and 3-year disease-free survival and overall survival rates were 80.0%,68.2%,and 60.0%,and 95.4%,92.3%,and 92.3%,respectively,for the LLS group;and 80.8%,59.9%,and 39.6%,and 98.2%,92.0%,and 74.4%,respectively,for the RFA group.The disease-free survival curve and overall survival curve were higher in the LLS group than in the RFA group(P=0.012 and P=0.013,respectively).Increased PIVKA-Ⅱlevels and small tumor size were associated with HCC recurrence in multivariate analysis.CONCLUSION:Liver resection is suitable for single HCC≤3 cm in the left lateral segments. | Jong Man Kim Tae Wook Kang Choon Hyuck David Kwon Jae-Won Joh Justin Sangwook Ko Jae Berm Park Hyunchul Rhim Joon Hyeok Lee Sung Joo Kim Seung Woon Paik | 2014 | World Journal of Gastroenterology2014,20,14: | 4 |

| 4 | Mooring Tension and Motion Characteristics of A Submerged Fish Reef with Net in Waves and Currents Using Numerical Analysis显示文摘A numerical model was used to analyze the motion response and mooring tension of a submerged fish reef system. The system included a net attached to a rigid structure suspended up from the bottom with a single, high tension mooring by fixed flotation. The analysis was performed by using a Morison equation type finite element model configured with truss elements. Input forcing parameters into the model consisted of both regular and irregular waves, with and without a steady current. Heave, surge and pitch dynamic calculations of the reef structure were made. Tension response results of the attached mooring line were also computed. Results were analyzed in both the time and frequency domain in which appropriate, linear transfer functions were calculated. The influence of the current was more evident in the tension and heave motion response data. This is most likely the result of the large buoyancy characteristics of the reef structure and the length of the mooring cable. Maximum mooring component tension was found to be 13.9 kN and occurred when the reef was subjected to irregular waves with a co-linear current of 1.0 m/s velocity. The results also showed that the system had little damping (in heave) with damped natural periods of 2.8 s. | 16 | Towards a Unified Single Analysis Framework Embedded with Multiple Spatial and Time Discretized Methods for Linear Structural Dynamics显示文摘We propose a novel computational framework that is capable of employing different time integration algorithms and different space discretized methods such as the Finite Element Method,particle methods,and other spatial methods on a single body sub-dividedintomultiple subdomains.This is in conjunctionwithimplementing thewell known Generalized Single Step Single Solve(GS4)family of algorithms which encompass the entire scope of Linear Multistep algorithms that have been developed over the past 50 years or so and are second order accurate into the Differential Algebraic Equation framework.In the current state of technology,the coupling of altogether different time integration algorithms has been limited to the same family of algorithms such as theNewmarkmethods and the coupling of different algorithms usually has resulted in reduced accuracy in one or more variables including the Lagrange multiplier.However,the robustness and versatility of the GS4 with its ability to accurately account for the numerical shifts in various time schemes it encompasses,overcomes such barriers and allows a wide variety of arbitrary implicit-implicit,implicit-explicit,and explicit-explicit pairing of the various time schemes while maintaining the second order accuracy in time for not only all primary variables such as displacement,velocity and acceleration but also the Lagrange multipliers used for coupling the subdomains.By selecting an appropriate spatialmethod and time scheme on the area with localized phenomena contrary to utilizing a single process on the entire body,the proposed work has the potential to better capture the physics of a given simulation.The method is validated by solving 2D problems for the linear second order systems with various combination of spatial methods and time schemes with great flexibility.The accuracy and efficacy of the present work have not yet been seen in the current field,and it has shown significant promise in its capabilities and effectiveness for general linear dynamics through numerical examples. | David Tae Kumar K.Tamma | 2023 | Computer Modeling in Engineering & Sciences2023,,5: | 0 |
