1Hepatocellular carcinoma in non-cirrhotic liver: A comprehensive review显示文摘Hepatocellular carcinoma(HCC) is the most common type of primary liver cancer, which in turns accounts for the sixth most common cancer worldwide.Despite being the 6 th most common cancer it is the second leading cause of cancer related deaths. HCC typically arises in the background of cirrhosis, however, about 20% of cases can develop in a non-cirrhotic liver. This particular subgroup of HCC generally presents at an advanced stage as surveillance is not performed in a non-cirrhotic liver. HCC in non-cirrhotic patients is clinically silent in its early stages because of lack of symptoms and surveillance imaging; and higher hepatic reserve in this population. Interestingly, F3 fibrosis in non-alcoholic fatty liver disease, hepatitis B virus and hepatitis C virus infections are associated with high risk of developing HCC. Even though considerable progress has been made in the management of this entity, there is a dire need for implementation of surveillance strategies in the patient population at risk, to decrease the disease burden at presentation and improve the prognosis of these patients. This comprehensive review details the epidemiology, risk factors, clinical features, diagnosis and management of HCC in non-cirrhotic patients and provides future directions for research.Aakash Desai Sonia Sandhu Jin-Ping Lai Dalbir Singh Sandhu 2019World Journal of Hepatology2019,11,1:19
2Gastrointestinal and hepatic manifestations of COVID-19:A comprehensive review显示文摘The severe acute respiratory syndrome-coronavirus-2(SARS-CoV-2)that causes coronavirus disease-2019(COVID-19)is a global pandemic, manifested by an infectious pneumonia.Although patients primarily present with fever, cough and dyspnea, some patients also develop gastrointestinal(GI)and hepatic manifestations. The most common GI symptoms reported are diarrhea, nausea, vomiting, and abdominal discomfort.Liver chemistry abnormalities are common and include elevation of aspartate transferase, alanine transferase, and total bilirubin.Studies have shown that SARS-CoV-2 infects the GI tract via its viral receptor angiotensin converting enzyme Ⅱ,which is expressed on enterocytes of the ileum and colon.Viral RNA has also been isolated from stool specimens of COVID-19 patients, which raised the concern for fecal-oral transmission in addition to droplet transmission.Although indirect evidence has suggested possible fecal-oral transmission of SARS-CoV-2,more effort is needed to establish the role of the fecal-oral transmission route.Further research will help elucidate the association between patients with underlying GI diseases, such as chronic liver disease and inflammatory bowel disease, and severity of COVID-19.In this review,we summarize the data on GI involvement to date,as well as the impact of COVID-19 on underlying GI diseases.Ming Han Cha Miguel Regueiro Dalbir S Sandhu 2020World Journal of Gastroenterology2020,26,19:4

5Responses of two cultivars of Trifolium repens L. to ethylene diurea in relation to ambient ozone显示文摘Three ethylene diurea(EDU) concentrations(0,150 and 300 mg/L) were used to evaluate the negative impact of ozone(O3) on two cultivars of Trifolium repens L.cv.Vardan and Bundel grown under natural field conditions in a suburban area of Varanasi,India.Mean O3 concentrations varied from 30.3 to 46.6 μg/L during the experimental period.Higher photosynthetic pigments and ascorbic acid concentrations were noticed in both EDU-treated cultivars over non-EDU-treated ones, but a reverse trend was found for lipid peroxidation.Growth parameters and biomass also showed increments under EDU treatment of both cultivars. The ratio of variable fluorescence to maximum fluorescence increased significantly in Vardan but not in Bundel upon EDU treatment. Results revealed that EDU concentration of 300 mg/L was more effective to combat the oxidative stress as well as protecting plants from O3 injury symptoms. The test cultivar Vardan is relatively sensitive to O3,thereby can be used as a bioindicator of O3 pollution in areas having higher O3 concentrations. Results also indicated that Bundel has more efficient antioxidant defense system than Vardan and hence was more tolerant to O3 stress.Shalini Singh Dalbir Kaur S.B.Agrawal M.Agrawal 2010Journal of Environmental Sciences2010,22,7:1

11Isolated colonic neurofibroma, a rare tumor: A case report and review of literature显示文摘BACKGROUND Neurofibromas are tumors comprised of peripheral nerve sheath and connective tissue components. They can occur sporadically or as part of familial syndromes such as neurofibromatosis type 1.Isolated colonic neurofibroma without systemic manifestations is a rarely reported clinical entity. Here we present a case of a 51 years old male with an isolated colonic neurofibroma seen on a screening colonoscopy.CASE SUMMARY Fifty-one years old male who was otherwise healthy without a significant family history of cancer underwent a screening colonoscopy and was found have a 2.3 cm×1.4 cm lesion in the colon.Tissue biopsy revealed a spindle cell tumor.Magnetic resonance imaging of the pelvis was negative for adenopathy. He underwent an endoscopic ultrasound that showed an ill-defined avascular lesion of mixed echogenicity measuring 2.8 cm×15.2 cm in the submucosa with no communication with muscularis mucosa or propria.Immunohistochemistry staining of the tumor was strongly positive for S100,with rare penetrating axons deep within the tumor.Tumor cells were negative for c-kit and desmin and had low Ki-67 index. These findings were consistent with a solitary colonic submucosal neurofibroma.A detailed history and physical examination did not reveal any evidence of extraintestinal neurofibromatosis. He underwent transanal surgical resection of the tumor. The patient tolerated the procedure well without any complications.CONCLUSION While neurofibromas have been well described in literature,an isolated colonic neurofibroma is a rare pathological entity.Malignant transformation of neurofibromas has been reported in patients with neurofibromatosis syndromes. We report a case of isolated colonic neurofibroma and highlight the importance of resection due to the increased risk of tumorigenesis.Sara Ghoneim Sonia Sandhu Dalbir Sandhu 2020World Journal of Clinical Cases2020,8,10:0
