19系统性回顾儿童重症监护期间的误诊显示文摘目的总结关于PICU内误诊发生率、后续影响和原因的文献。数据来源检索PubMed、EMBASE和Cochrane数据库,期限至2019年12月。研究选择纳入关于儿童重症监护误诊和诊断过程的研究。没有翻译的非英语研究、病例报告/系列、没有提供误诊信息的研究、重点为非PICU人群的研究、单一条件/疾病或单一诊断试验/工具均被排除。数据抽取从每项研究中提取研究设计、目标、研究样本数据以及结果中误诊流行率、后续影响和原因的数据。数据整合采用独立分层评价法筛选396篇摘要,最终纳入17篇研究(全文14篇,摘要3篇)。88%(15/17)的研究采用观察性研究设计。尸检研究(尸检率为20%~47%)显示主要诊断漏诊率为10%~23%;5%~16%的尸检发现的误诊对患儿生存有潜在不良影响并可改变诊疗。回顾性病历分析报告了误诊率的差异,从普通PICU患儿的8%到意外危重症入院患儿的12%,以及在PICU发病率和病死率会议上讨论患儿的21%~25%。心血管疾病、感染性疾病、先天性疾病和神经系统疾病最常被误诊。系统因素(40%~67%)、认知因素(20%~33%)以及系统联合认知因素(40%)与误诊有关。关于误诊影响的资料有限。结论对PICU内误诊所知有限。下一步工作应平衡关注不同疾病诊断研究,并发掘常见误诊的系统和过程决定因素,以期对误诊有更深入了解。Cifra CL Custer JW Singh H 方伯梁(译) 钱素云(校) 2021中国小儿急救医学2021,28,12:0
