| 1 | 香港华人妇女产后抑郁危险因素的前瞻性研究显示文摘目的:产后抑郁是一种常见的精神障碍,对产妇、新生儿和其它家庭成员导致广泛的伤害。产后抑郁的病因学研究大多是基于西方样本,对其中的社会文化原因也缺乏实证研究。本研究从人口学、心理社会及种族文化因素等方面探讨中国妇女产后抑郁症状学的决定因素。方法:在大学附属综合医院产前门诊连续收集登记的中国孕妇959例,从她们第一次产前检查开始评估(基线),以后分别在孕期的最后3个月、产后即刻和产后3个月时评估。调查涉及的危险因素分6个方面:人口学和社会经济背景,躯体疾病及精神疾病病史, 产前抑郁情绪,围生期应激源,人际关系,和种族文化环境。采用等级回归进行分析。因变量是产后3个月时的抑郁评分。结果:决定产后抑郁症状学的因素有:目前的生活事件,缺乏社会支持,婚姻不满意,既往有故意自伤史,以及产前存在抑郁情绪。婆媳关系不好和没有陪月在产后阶段也与抑郁评分高有关联。结论:西方研究所确立的产后抑郁危险因素大体适用于中国妇女。产褥期的社会文化特点同样对产妇情绪有影响。 | Dominic TS Lee Alexander SK Yip Sandra SM Chan Fiona FY Lee Tony YS Leung Tony KH Chung | 2005 | 中国心理卫生杂志2005，19，9： | 50 |
| 2 | Hepatic adenoma mimicking a metastatic lesion on computed tomography-positron emission tomography scan显示文摘Positron emission tomography (PET) using 18F-fluorodeoxyglucose ( 18F-FDG) is an imaging modality which reflects cellular glucose metabolism. Most malignant cells accumulate and trap 18F-FDG, allowing the visualisation of increased uptake. It is hence widely used to differentiate malignant from benign lesions. 'False positive' findings of hepatic lesions have been described in certain instances such as hepatic abscesses, but are rare in cases involving hepatocellular adenomas. To our knowledge, there have been only 7 reports in the English literature documenting PET-avid hepatocellular adenomas; 6 of the 7 reports were published in the last 3 years with the first report by Patel et al. We report the case of a 44-year-old Chinese female patient with a history of cervical adenocarcinoma, referred for a hepatic lesion noted on a surveillance computed tomography (CT) scan. A subsequent CT-PET performed showed a hypermetabolic lesion (standardized uptake value 7.9) in segment Ⅳb of the liver. After discussion at a multidisciplinary hepato-pancreato-biliary conference, the consensus was that of a metastatic lesion from her previous cervical adenocarcinoma, and a resection of the hepatic lesion was performed. | 3 | Dynamic phenotypic heterogeneity and the evolution of multiple RNA subtypes in hepatocellular carcinoma:the PLANET study显示文摘Intra-tumor heterogeneity(ITH) is a key challenge in cancer treatment, but previous studies have focused mainly on the genomic alterations without exploring phenotypic(transcriptomic and immune)heterogeneity. Using one of the largest prospective surgical cohorts for hepatocellular carcinoma(HCC)with multi-region sampling, we sequenced whole genomes and paired transcriptomes from 67 HCC patients(331 samples). We found that while genomic ITH was rather constant across stages, phenotypic ITH had a very different trajectory and quickly diversified in stage Ⅱ patients. Most strikingly, 30% of patients were found to contain more than one transcriptomic subtype within a single tumor. Such phenotypic ITH was found to be much more informative in predicting patient survival than genomic ITH and explains the poor efficacy of single-target systemic therapies in HCC. | 4 | Rare case of intussusception in an adult with acute myeloid leukemia显示文摘Intussusception is rarely reported in adult patients with acute leukemia. We report a case of intussusception in a 29-year-old woman with acute myeloid leukemia(AML). She developed right lower quadrant pain, fever, and vomiting on day 16 of induction chemotherapy.Physical examination showed tenderness and guarding at the right lower quadrant of the abdomen. Abdominal computed tomography(CT) showed distension of the cecum and ascending colon, which were filled with loops of small bowel, and herniation of the ileocecal valve into the cecum. We proceeded to laparotomy and revealed ileocecal intussusception with the ileocecal valve as the leading point. The terminal ileum was thickened and invaginated into the cecum, which showed gangrenous changes. Right hemicolectomy was performed and microscopic examination of the colonic tissue showed infiltration of leukemic cells. The patient recovered after the operation and was subsequently able to continue treatment for AML. This case demonstrates that the diagnosis of intussusception is difficult because the presenting symptoms can be non-specific, but abdominal CT can be informative for preoperative diagnosis. Resection of the involved bowel is recommended when malignancy is suspected or confirmed. Intussusception should be considered in any leukemia patients presenting with acute abdomen.
