1Antidiabetic therapy and increased risk of hepatocellular carcinoma in chronic liver disease显示文摘AIM:To explore the association between hepatocellular carcinoma(HCC) and type 2 diabetes mellitus, describe the temporal relations between the onset of diabetes and the development of HCC and evaluate the possible effects of antidiabetic therapy on HCC risk.METHODS:We recruited 465 HCC patients, 618 with cirrhosis and 490 control subjects.We evaluated the odds ratio(OR) for HCC by univariate and multivariate analysis.Moreover, OR for HCC in diabetic subjects treated with insulin or sulphanylureas and with metformin were calculated.RESULTS:The prevalence of diabetes mellitus was 31.2% in HCC, 23.3% in cirrhotic patients and 12.7% in the Control group.By univariate and multivariate analysis, the OR for HCC in diabetic patients were respectively 3.12(CI 2.2-4.4, P < 0.001) and 2.2(CI 1.2-4.4, P = 0.01).In 84.9% of cases, type 2 diabetes mellitus was present before the diagnosis of HCC.Moreover, we report an OR for HCC of 2.99(CI 1.34-6.65, P = 0.007) in diabetic patients treated with insulin or sulphanylureas, and an OR of 0.33(CI 0.1-0.7, P = 0.006) in diabetic patients treated with metformin.CONCLUSION:Our study confi rms that type 2 diabetes mellitus is an independent risk factor for HCC and pre-exists in the majority of HCC patients.Moreover, in male patients with type 2 diabetes mellitus, our data shows a direct association of HCC with insulin and sulphanylureas treatment and an inverse relationship with metformin therapy.Valter Donadon Massimiliano Balbi Michela Ghersetti Silvia Grazioli Antonio Perciaccante Giovanni Della Valentina Rita Gardenal Maria Dal Mas Pietro Casarin Giorgio Zanette Cesare Miranda 2009World Journal of Gastroenterology2009,15,20:23

13Potential strategies offered by animals to implement in buildings' energy performance: Theory and practice显示文摘The strategies for thermal regulation and envir on mental control found in nature are countless. In this article, a parallelism between animals and building energy systems is defined in order to identJfy and emphasize the immediate opportunities that biomimicry offers for future research. The motivation was the need to find alternative solutions to tackle problems mainly in the efficiency of heating, ventilation and cooling systems. Due to the wide range of possibilities offered by animals, this study is largely limited to the strategies that cold-blooded animals have developed through evolutionary adaptation to the environment. The method used for the an a lysis is based on a soluti on-based approach. Firstly, differe nt animal thermoregulation strategies are defined (biological domain). Then the strategy is an a lyzed and classified into three categories. This classification is esse ntial in order to formulate the parallelism with building systems (transfer phase). The final step is to identify the potential implementation (technological domain).This approach has been seen to be useful in creating new research opportunities based on biomimicry. 15Minimally invasive tenodesis for peroneus longus tendon rupture: A case report and review of literature显示文摘BACKGROUND Peroneal tendon disorders are common causes of lateral hindfoot pain.However,total rupture of the peroneal longus tendon is rare.Surgical treatment for this condition is usually a side-to-side tenodesis of the peroneal longus tendon to the peroneal brevis tendon.While the traditional procedure involves a long lateral curved incision,this approach is associated with damage to the lateral soft tissues(up to 24%incidence).CASE SUMMARY A 50-year-old female had developed pain at the lateral aspect of the hindfoot 1 mo after an ankle sprain while walking in the street.Previous treatments were anti-inflammatory drugs,ice,rest and Cam-walker boot.At physical exam,there was pain and swelling over the course of the peroneal tendons.Ankle instability and cavovarus foot deformity were ruled out.Eversion strength was weak(4/5).Imaging showed complete rupture of the peroneal longus tendon associated with a sharp hypertrophic peroneal tubercle.Surgical repair was indicated after failure of conservative treatment(physiotherapy,rest,analgesics,and ankle stabilizer).A less invasive approach was performed for peroneal longus tendon debridement and side-to-side tenodesis to the adjacent peroneal brevis tendon,with successful clinical and functional outcomes.CONCLUSION Peroneus longus tendon tenodesis can be performed through a less invasive approach with preservation of the lateral soft tissue integrity.Danilo Ryuko Cândido Nishikawa Fernando Aires Duarte Guilherme Honda Saito Cesar de Cesar Netto Fábio Correia Paiva Fonseca Bruno Rodrigues de Miranda Augusto César Monteiro Marcelo Pires Prado 2020World Journal of Orthopedics2020,11,2:0

17Surgical treatment of cementoblastoma associated with apicoectomy and endodontic therapy:Case report显示文摘This case report describes the surgical removal of cementoblastoma associated with apicoectomy and endodontic therapy. The patient, an 18-year-old man, presented pain in the region of the mandibular body on the right side. On clinical exam, bone expansion was observed in the region at the bottom of the vestibular sulcus, pain on palpation, slight extrusion of tooth 46 with presence of pulp vitality. Radiographic exams demonstrated the presence of a radiopaque area and discrete radiolucent halo associated with the root of tooth 46, suggesting the diagnosis of cementoblastoma. Endodontic treatment of tooth 46 was performed and exeresis of the lesion by apicoectomy. Twelve months after the first surgery, recurrence of the lesion was observed, and a new apicoectomy was necessary, this time up to the middle third of the root.
