| 1 | Serum uric acid level in newly diagnosed essential hypertension in a Nepalese population:A hospital based cross sectional study显示文摘Objective:To develop the missing link between hyperuricemia and hypertension.Methods:The study was conducted in Department of Biochemistry in collaboration with Nephrology Unit of Internal Medicine Department.Hypertension was defined according to blood pressure readings by definitions of the Seventh Report of the Joint National Committee.Totally 205newly diagnosed and untreated essential hypertensive cases and age-sex matched nonnotensive controls were enrolled in the study.The potential confounding factors of hyperuricemia and hypertension in both cases and controls were controlled.Uric acid levels in all participants were analyzed.Results:Renal function between newly diagnosed hypertensive cases and nonnotensive healthy controls were adjusted.The mean serum uric acid observed in newly diagnosed hypertensive cases and in nonnotensive healthy controls were(290.05±87.03)μmol/L and(245.24±09.38)μmol/L respectively.A total of 59(28.8%)participants of cases and 28(13.7%)participants of controls had hyperuricemia(odds ratio 2.555(95%CI:1.549-4.213),P<0.00l).Conclusions:The mean serum uric acid leveb and number of hyperuricemic subjects were found to be significantly higher in cases when compared to controls. | Bibek Poudel Binod Kumar Yadav Arun Kumar Bharat Jha Kanak Bahadur Raut | 2014 | Asian Pacific Journal of Tropical Biomedicine2014,4,1: | 10 |

| 8 | First jejunal artery, an alternative graft for right hepatic artery reconstruction显示文摘Common bile duct cancer invading right hepatic artery is sometimes diagnosed intraoperatively. Excision andsafe reconstruction of the artery with suitable graft is essential. Arterial reconstruction with autologous saphenous vein graft is the preferred method practiced routinely. However the right hepatic artery reconstruction has also been carried out with several other vessels like gastroduodenal artery, right gastroepiploic artery or the splenic artery. We report a case of 63-year-old man presenting with history of progressive jaundice, pruritus and impaired appetite. Following various imaging modalities including computed tomography, endoscopic retrograde cholangiopancreatography, magnetic resonance cholangiopancreatography, intraductal ultrasound extrahepatic bile duct cancer was diagnosed; however, none of those detected vessel invasion. Intraoperatively, right hepatic artery invasion was revealed. Right hepatic artery was resected and reconstructed with a graft harvested from the first jejunal artery(JA). Postoperative outcome was satisfactory with a long-term graft patency. | 13 | A Study of Prospective Barriers,Benefits and Measures for Building Information Modeling(BIM)Adoption in Nepalese AEC Industry显示文摘The construction industry needs modern construction methodology and technology to improve sustainability and production performance.Building Information Modelling(BIM)technology supports improving the quality of products by reducing design and construction defects,risks to the health and safety of workers,and reduce overall project cost and delivery time.The BIM has capabilities,but it is still undiscovered and unable to exploit the full scale of its benefits in the Architectural Engineering and Construction(AEC)industry.There is a trend to adopt the BIM level 1,which is limited to 2D and only in a few cases 3D models uses in the design and construction of residential and commercial buildings,particularly in Nepal.Hence,this paper focuses on providing insight into the BIM benefits and identifies the potential barriers while adopting BIM Level 3 in Nepal.This was accomplished by developing a 4DBIM model of a multi-story residential building in Nepal and conducting the industry survey via focus group with the AEC professionals based on the developed 4DBIM model.A comprehensive literature review was conducted and presented the findings of the BIM benefits and barriers while adopting BIM.The study found that commercial and governmental projects can immediately be adopted BIM technology.It is concluded that the unavailability of skilled BIM users and the lack of proper policies for BIM adoption are key barriers in Nepal.Hence,the new policy is required to achieve and exploit the full scale of the BIM benefits and improve the project delivery in terms of quality,cost and time including the health and safety of workers and the sustainability of the AEC industry. | Raj Shah Bibek Shrestha | 2023 | Journal of Smart Buildings and Construction Technology2023,5,1: | 0 |
| 14 | 过度通气能够改善幕上开颅术的手术条件吗?一项多中心随机交叉试验研究显示文摘背景过度通气作为神经外科手术麻醉中的重要方法,其有效性还缺乏验证。我们在由手术医师评估脑肿胀分级和测定颅内压(ICP)的条件下,选择中度低碳酸血症和正常血液碳酸数值两种情况对接受开颅手术的幕上脑肿瘤患者进行一项双周期随机交叉试验研究。方法275例幕上脑肿瘤患者随机接受以下治疗程序:一是过度通气(动脉二氧化碳分压,PaCO2=25±2mmHg)继以正常通气(动脉二氧化碳分压,PaCO2=37±2mmHg);二是正常通气继以过度通气。通气和呼气末CO2分压保持稳定20分钟。患者随机接受丙泊酚输注或异氟烷吸入麻醉。在每一试验阶段末由不了解治疗程序的手术医师测定ICP,并以4级评定方法评估脑肿胀分级。结果以通用估计方程模型分析,我们发现过度通气降低了45%脑肿胀加重的风险,P=0.004,95%可信区间为22%-61%,需要治疗患者数为8。过度通气期间ICP均数±标准差为12.3±8.1mmHg,低于正常通气时的16.2±9.6mmHg,P〈0.001。麻醉方式对脑肿胀评估和ICP无影响。结论对幕上脑肿瘤患者而言,手术中过度通气可改善与ICP降低相关的由手术医师评估的脑肿胀程度。 | Adrian W. | 16 | Recent advancements on novel approaches of insulin delivery显示文摘A large number of populations are living with diabetes, which is the most common endocrine disorder which isclassified into Type-I and Type-II diabetes. Diabetes is one of the listed diseases specified in Section 3 of the Drugand Magic Remedies Act, 1954, where advertisement of drugs for certain diseases has been prohibited. It can bemanaged or controlled with proper diagnosis and treatment and by changing food habits. In addition to therapywith oral hypoglycemic agents, patients are also given subcutaneous insulin injections which is traditionally usedin diabetes therapy. This multi-dose injectable therapy has relatively low patient compliance. To avoid theassociated drawbacks with this therapy, several other delivery techniques, including oral, buccal, rectal, transdermal and pulmonary routes were investigated for effective insulin delivery.
