| 3 | Implantless patellar fixation in medial patellofemoral ligament reconstruction显示文摘Purpose:The medial patellofemoral ligament(MPFL)acts as primary restraint to lateral patellar dislocation and its rupture has been reported in almost all cases of acute patellar dislocation.Various surgical techniques have been described for MPFL reconstruction,using many femoral and patellar fixation techniques and different grafts.This article details our technique for MPFL reconstruction using semitendinosus graft which avoids the use of implant at patellar end.Methods:Twenty patients(8 males and 12 females)with complaints regarding acute and chronic lateral patellar instability were evaluated and treated by MPFL reconstruction procedure.The mean age of patients was 21 years(range 17-34 years).MPFL reconstruction was performed using semitendinosus graft passing through two parallel,obliquely directed tunnels created in patella.Fixation of graft was done with an interference screw only at the femoral end.Mean follow-up period after intervention was 26.4 months(range 23-30 months).Results were evaluated using Kujala score.Results:All patients gained adequate patellar stability and full arc of motion.No incidence of patella fracture was noted.There were no postoperative complications related to the procedure.There was no recurrence of instability in patella at final follow-up.Conclusion:Passing the graft through the tunnels in patella without use of any implant has given excellent functional outcome and moreover has the advantages of less implant-related complications and cost-effectiveness. | Ashish Devgan Umesh Yadav Pankaj Sharma Rajesh Rohilla Radhika Devgan Pravesh Mudgil Aman Verma Vasudha Dhupper | 2019 | Chinese Journal of Traumatology2019,22,5: | 1 |

| 5 | High Phenotypic Variation in Morus alba L. along an Altitudinal Gradient in the Indian Trans-Himalaya显示文摘Ten quantitative morphological characters were studied in 56 Morus alba L. trees representing three natural populations from the trans-Himalayan Ladakh region. The altitude of collection sites ranged from 2815 to 3177 m above the sea level(asl). Coefficient of variation(CV) showed high phenotypic variation in M. alba. Linear regression analysis revealed that leaf and fruit size decreases with an increase in altitude. High CV was observed for leaf length, leaf width, petiole length, leaf area, internodal distance, number of nodes, bud length, fruit length, fruit width and fruit weight. Similarly, a high phenotypic plasticity index was observed for bud length, leaf length, leaf width, petiole length, leaf area, inter-nodal distance, number of nodes, fruit length, fruit width and fruit weight. For every 100 m increase in altitude, leaf length, leaf width and leaf area decreased by 1 cm, 0.8 cm and 16.6 cm2, respectively. Analysis of covariance showed a predominant altitudinal effect on the morphological characters in comparison to the population effect. A small change in the altitude caused significant change in the plant morphological characteristics. | 6 | 幽门螺杆菌根除治疗对非酒精性脂肪肝病患者肝脂肪变化的影响:一项随机对照先导研究显示文摘背景:幽门螺杆菌(Hp)感染可导致胰岛素抵抗和非酒精性脂肪肝病(NAFLD)。本研究旨在评估Hp根除治疗(HPET)与标准治疗(SMT)对NAFLD患者的影响。方法:80例伴有Hp感染的NAFLD患者随机分入SMT组(36例,饮食和锻炼)和HPET组(44例,除饮食和锻炼标准治疗外,予以阿莫西林+克拉红霉素+泮托拉唑三联疗法对Hp进行根除治疗)。检测受控衰减参数(CAP)、体脂参数、肝酶、血脂及包括稳态模型胰岛素抵抗指数(HOMA-IR)在内的血糖参数,比较治疗前及治疗后24周两组患者上述指标的差异。结果:64例患者纳入改良意向性分析,其中SMT组28例,HPET组36例。治疗后24周,SMT组和HPET组患者CAP评分均显著下降(P=0.002和P<0.001),但CAP变化值两组差异无统计学意义(P=0.213)。24周时,HPET组有68%的患者成功根除Hp,与治疗无应答者及SMT组患者相比,Hp根除者HOMA-IR显著改善(P=0.007)。治疗后24周,两组患者肝酶水平均显著下降,但变化值两组差异无统计学意义;血脂水平在治疗前后无显著改变,且两组间差异亦无统计学意义。HPET组患者谷胱甘肽显著下降,但变化值两组差异无统计学意义。结论:与标准治疗相比,辅以Hp根除治疗并不能进一步减轻NAFLD患者的肝脂肪变性和进一步降低肝酶水平,但成功根除Hp可以使HOMA-IR获得显著改善(临床注册号:CTRI/2017/05/008608)。 | Vikas Maharshi Pooja Gupta Vijay L.Kumar Ashish Datt Upadhyay Prasenjit Das Rajni Yadav Baibaswata Nayak Ramesh Kumar Shalimar | 2020 | Gastroenterology Report2020,8,2: | 1 |
