| 1 | 证据指导临床决策显示文摘澳大利亚循证护理中心(Joanna Briggs Institute,JBI)建立于1996年,为国际性循证实践合作机构,主要致力于护理及医学相关科学领域的研究。JBI目前在全球31个国家中设立有分中心,其中包括美国、英国、加拿大、西班牙、瑞典、南非、泰国、新西兰等,上海复旦大学护理学院在2004年11月成立了复旦大学循证护理中心,是JBI的第21个分中心。JBI志立于为广大医护专业人员提供最佳证据并促进其投入临床实践,成立12年来开展了系统、丰富的循证护理学术活动。包括组织进行专题的文献系统回顾、举办“循证护理”国际年会、举行培训和研讨会、开展根据文献系统回顾引出的相关研究、资助培训“循证护理”实践活动、编辑发行((BestPractice:EvidenceBasedPracticeInformationSheets)(《最佳护理实践》)刊物,为临床护理实践提供实证,以倡导循证护理在实践中的作用。目前JBI已公开发表的循证护理专题成果有36项,例如:“压疮的危险因素评估和预防”、“压疮所致组织损伤的处理”、“外周血管插管装置的管理和感染控制”、“医院跌倒和坠床的预防和处理”、“癌症化疗病人口腔溃疡的预防和处理”、“冠心病患者的心理社会干预”、“手术病人的术前教育”、“急性疼痛的非药物干预法”等等。JBI通过组织世界各地的专题小组进行为期至少半年的文献系统回顾,总结归纳实证依据,并开展相应研究,发布专题刊物,对循证护理在全球的推广,进而提高护理实践的科学性和有效性发挥了重要的作用。本刊将通过此栏目陆续发表JBI提供的部分《最佳护理实践》的中文译文,以便更好地将循证护理的理念和最新成果提供给广大的护理工作者,促进临床护理的规范性和科学性。《证据指导临床决策》是在本栏目发表的第一篇文章,文中介绍了:何谓证据、循证实践,及JBI循证实践的模式。 | Jordan Zoe Court Anthea Xue Yi-fan | 2009 | 中华护理杂志2009，44，4： | 8 |
| 2 | Ionospheric longitudinal variations at midlatitudes: Incoherent scatter radar observation at Millstone Hill显示文摘Incoherent scatter radar (ISR) extra-wide coverage experiments during the period of 1978-2011 at Millstone Hill are used to investigate longitudinal differences in electron density. This work is motivated by a recent finding of the US east-west coast difference in TEC suggesting a combined effect of changing geomagnetic declination and zonal winds. The current study provides strong supporting evidence of the longitudinal change and the plausible mechanism by examining the climatology of electron density Ne on both east and west sides of the radar with a longitude separation of up to 40o for different heights within 300-450 km. Main findings include: 1) The east-west difference can be up to 60% and varies over the course of the day, being positive (East side Ne > West side Ne) in the late evening, and negative (West side Ne > East side Ne) in the pre-noon. 2) The east-west difference exists throughout the year. The positive (relative) difference is most pronounced in winter; the negative (relative) difference is most pronounced in early spring and later summer. 3) The east-west difference tends to enhance toward decreasing solar activity, however, with some seasonal dependence; the enhancements in the positive and negative differences do not take place simultaneously. 4) Both times of largest positive and largest negative east-west differences in Ne are earlier in summer and later in winter. The two times differ by 12-13 h, which remains constant throughout the year. 5) Variations at different heights from 300-450 km are similar. Zonal wind climatology above Millstone Hill is found to be perfectly consistent with what is expected based on the electron density difference between the east and west sides of the site. The magnetic declination-zonal wind mechanism is true for other longitude sectors as well, and may be used to understand longitudinal variations elsewhere. | 15 | Lymphoproliferative disorders in inflammatory bowel disease patients on immunosuppression: Lessons from other inflammatory disorders显示文摘Immunosuppressive agents, such as thiopurines, methotrexate, and biologics, have revolutionized the treatment of inflammatory bowel disease(IBD). However, a number of case reports, case control studies and retrospective studies over the last decade have identified a concerning link between immunosuppression and lymphoproliferative disorders(LPDs), the oncological phenomenon whereby lymphocytes divide uncontrollably. These LPDs have been associated with Epstein-Barr virus(EBV) infection in which the virus provides the impetus for malignant transformation while immunosuppression hampers the immune system's ability to detect and clear these malignant cells. As such, the use of immunosuppressive agents may come at the cost of increased risk of developing LPD. While little is known about the LPD risk in IBD, more is known about immunosuppression in the post-transplantation setting and the development of EBV associated posttransplantation lymphoproliferative disorders(PTLD). In review of the PTLD literature, evidence is available to demonstrate that certain immune suppressants such as cyclosporine and T-lymphocyte modulators in particular are associated with an increased risk of PTLD development. As well, high doses of immunosuppressive agents and multiple immunosuppressive agent use are also linked to increased PTLD development.
