| 1 | Long-term outcomes after fractional flow reserve-guided percutaneous coronary intervention in patients with severe coronary stenosis显示文摘Objective To explore the safety and efficacy of FFR-guided percutaneous coronary intervention (PCI) in vessels with severe diameter stenosis. Methods & Results Of 1090 patients undergoing fractional flow reserve (FFR) assessment from 2002 to 2009,we identified 167 patients in whom FFR was measured in at least one 70%–89% stenotic lesion. These patients were subdivided into an FFR-defer group (n = 49) if PCI was deferred (FFR > 0.80),and an FFR-perform group (n = 118) if PCI was performed (FFR ≤ 0.80). Comparatively,an additional 1176 patients undergoing PCI in at least one lesion with 70%–89% stenosis but without measurement of FFR served as a control (angiography- guided) group. Clinical outcomes were compared during a median follow-up of 49.0 months. The 5-year Kaplan-Meier estimated revascularization rates were 16% in the FFR-defer group and 33% in the FFR-perform group (P = 0.046). The incidence of major adverse cardiac events were comparable in these two groups (HR = 0.82,95% CI: 0.37–1.82,P = 0.63). The number of stents placed was significantly lower in the FFR-guided group (0.9 ± 0.8 vs. 1.4 ± 0.8,P < 0.001). Conclusions Functional revascularization for lesions with visually severe stenosis is clinically safe and associated with fewer stents use. | 6 | Trauma and syncope-evidence for further sleep study? A case report显示文摘We report on an 83-year-old male with traumatic brain injury after syncope with a fall in the morning. He had a history of seizures, coronary artery disease and paroxysmal atrial fibrillation(AF). No medical cause for seizures and syncope was determined. During rehabilitation, the patient still complained of seizures, and also reported sleepiness and snoring. Sleep apnea diagnostics revealed obstructive sleep apnea(SA) with an apnea-hypopnoea index of 35/h, and sudden onset of tachycardia with variations of heart rate based on paroxysmal atrial fibrillation. Additional tests showed nocturnal AF which spontaneously converted to sinus rhythm mid-morning with an arrest of 5 s(sick sinus syndrome) and seizures. A DDD-pacer was implanted and no further seizures occurred. SA therapy with nasal continuous positive airway pressure was refused by the patient. | 14 | Probabilistic Tractable Models in Mixed Discrete-Continuou s Domains显示文摘We study the problem of the unsupervised learning of graphical models in mixed discrete-continuous domains.The problem of unsupervised learning of such models in discrete domains alone is notoriously challenging,compounded by the fact that inference is computationally demanding.The situation is generally believed to be significantly worse in discrete-continuous domains:estimating the unknown probability distribution of given samples is often limited in practice to a handful of parametric forms,and in addition to that,computing conditional queries need to carefully handle low-probability regions in safety-critical applications.In recent years,the regime of tractable learning has emerged,which attempts to learn a graphical model that permits efficient inference.Most of the results in this regime are based on arithmetic circuits,for which inference is linear in the size of the obtained circuit.In this work,we show how,with minimal modifications,such regimes can be generalized by leveraging efficient density estimation schemes based on piecewise polynomial approximations.Our framework is realized on a recent computational abstraction that permits efficient inference for a range of queries in the underlying language.Our empirical results show that our approach is effective,and allows a study of the trade-off between the granularity of the learned model and its predictive power. | Andreas Bueff Stefanie Speichert Vaishak Belle | 2021 | Data Intelligence2021，3，2： | 0 |
