| 1 | Anti-TNF alpha in the treatment of ulcerative colitis:A valid approach for organ-sparing or an expensive option to delay surgery?显示文摘Ulcerative colitis(UC)is an inflammatory bowel disease affecting large bowel with variable clinical course.The history of disease has been modified by the introduction of biologic therapy,in particular Infliximab(IFX),that has demonstrated efficacy in inducing fast symptoms remission,promoting mucosal healing and maintaining long-term remission.However,surgery is still needed for UC patients:in case of failure of medical therapy and if acute complications or a malignancy occurred.Surgical treatment is associated with a short-term postoperative mortality and morbidity respectively of 0%-4%and 30%.In this study we systematically analyzed:the role of IFX in reducing the colectomy rate,the risk of post-operative morbidity in pre-operatively IFX-treated patients and the cost-effectiveness of IFX therapy.Four of 5 analyzed randomized controlled trials demonstrated that therapy with IFX significantly reduces the colectomy rate.Moreover,pre-operative treatment with IFX doesn’t seem to increase post-operative infectious complications.By an economic point of view,the cost-effectiveness of IFX-therapy was demonstrated for UC patients suffering from moderate to severe UC in a study based on a cost estimation of the National Health Service of England and Wales.However,the argument is debated. | Gianluca Rizzo Daniela Pugliese Alessandro Armuzzi Claudio Coco | 2014 | World Journal of Gastroenterology2014，20，17： | 5 |
| 2 | Current status of the adjuvant therapy in uterine sarcoma: A literature review显示文摘Uterine sarcomas (US) are rare mesenchymal tumours accounting approximately for 3%–7% of all uterine cancers. Histologically, US are classified into mesenchymal tumours or mixed epithelial and mesenchymal tumours. The group of mesenchymal tumours includes uterine leiomyosarcoma (uLMS, 65% of cases), endometrial stromal sarcoma (ESS, 21%)– traditionally divided into low grade (LG-ESS) and high grade–undifferentiated uterine sarcoma (5%) and other rare subtypes such as alveolar or embryonal rhabdomyosarcoma. Despite the fact that several drugs demonstrated clinical activity in advanced or metastatic settings, the role of postoperative therapy in US remains controversial. | 16 | Pancreatic mucinous cystadenocarcinoma in a patient harbouring BRCA1 germline mutation effectively treated with olaparib: A case report显示文摘BACKGROUND Pancreatic mucinous cystadenocarcinoma(MCAC)is a rare malignancy with a poor prognosis when it presents metastases at diagnosis.Due to its very low incidence,there are no clear recommendations for the treatment of advanced disease.Olaparib(an oral PARP inhibitor)has been approved for the maintenance treatment of patients with metastatic pancreatic adenocarcinoma harbouring germline BRCA1/2 mutations.Herein,we report the first case of a germline BRCA1 mutated unresectable MCAC which was effectively treated with olaparib.CASE SUMMARY A 41-year-old woman,without personal or family history of cancer,was diagnosed with ovarian and peritoneal metastases of MCAC.She underwent 12 cycles of gemcitabine plus oxaliplatin(GEMOX)obtaining a partial response and allowing radical surgery.One year later,local recurrence was documented,and other 12 cycles of GEMOX were administered obtaining a complete response.Seven years later,another local recurrence,not amenable to surgical resection,was diagnosed.She started FOLFIRINOX(oxaliplatin,irinotecan,leucovorin and fluorouracil),obtaining a partial response after 8 cycles.Given the excellent response to platinum-based chemotherapy,BRCA testing was performed,and a BRCA1 germline mutation was detected.She was switched to maintenance olaparib due to chemotherapy-related toxicities and achieved an almost complete metabolic response,with a reduction in the diameter of the lesion,after three months of therapy.CONCLUSION The current case suggests the beneficial effect of olaparib in BRCA mutated MCAC.However,further studies are required. | Mariacristina Di Marco Riccardo Carloni Stefania De Lorenzo Cristina Mosconi Andrea Palloni Elisa Grassi Daria Maria Filippini Angela Dalia Ricci Alessandro Rizzo Alessandro Di Federico Donatella Santini Daniela Turchetti Claudio Ricci Carlo Ingaldi Laura Alberici Francesco Minni Rita Golfieri Giovanni Brandi Riccardo Casadei | 2020 | World Journal of Gastrointestinal Oncology2020，12，12： | 0 |
| 17 | Efficiency in hydrocarbon degradation and biosurfactant production by Joostella sp.A8 when grown in pure culture and consortia显示文摘Joostella strains are emerging candidates for biosurfactant production. Here such ability was analyzed for Joostella strain A8 in comparison with Alcanivorax strain A53 and Pseudomonas strain A6, all previously isolated from hydrocarbon enrichment cultures made of polychaete homogenates. In pure cultures Joostella sp. A8 showed the highest stable emulsion percentage(78.33%), hydrophobicity rate(62.67%), and an optimal surface tension reduction during growth in mineral medium supplemented with diesel oil(reduction of about 12 mN/m), thus proving to be highly competitive with Alcanivorax and Pseudomonas strains. During growth in pure culture different level of biodegradation were detected for Alcanivorax strain A53(52.7%),Pseudomonas strain A6(38.2%) and Joostella strain A8(26.8%). When growing in consortia,isolates achieved similar abundance values, with the best efficiency that was observed for the Joostella-Pseudomonas co-culture. Gas-chromatographic analysis revealed an increase in the biodegradation efficiency in co-cultures(about 90%), suggesting that the contemporary action of different bacterial species could improve the process.
