1Is human hepatocellular carcinoma a hormone-responsive tumor?显示文摘Before the positive results recently obtained with multitarget tyrosine kinase inhibitor sorafenib,there was no standard systemic treatment for patients with advanced hepatocellular carcinoma(HCC).Sex hormones receptors are expressed in a significant proportion of HCC samples.Following preclinical and epidemiological studies supporting a relationship between sex hormones and HCC tumorigenesis,several randomized controlled trials (RCTs)tested the efficacy of the anti-estrogen tamoxifen as systemic treatment.Largest among these trials showed no survival advantage from the administration of tamoxifen,and the recent Cochrane systematic review produced a completely negative result.This questions the relevance of estrogen receptor-mediated pathways in HCC.However,a possible explanation for these disappointing results is the lack of proper patients selection according to sex hormones receptors expression,but unfortunately the interaction between this expression and efficacy of tamoxifen has not been studied adequately.It has been also proposed that negative results might be explained if tamoxifen acts in HCC via an estrogen receptor-independent pathway,that requires higher doses than those usually administered, but an Asian RCT conducted to assess dose-response effect was completely negative.Interesting,preliminaryresults have been obtained when hormonal treatment (tamoxifen or megestrol)has been selected according to the presence of wild-type or variant estrogen receptors respectively,but no large RCTs are available to support this strategy.Negative results have been obtained also with anti-androgen therapy.In conclusion,there is no robust evidence to consider HCC a hormone-responsive tumor.Hormonal treatments should not be part of the current management of HCC.Massimo Di Maio Bruno Daniele Sandro Pignata Ciro Gallo Ermelinda De Maio Alessandro Morabito Maria Carmela Piccirillo Francesco Perrone 2008World Journal of Gastroenterology2008,14,11:2
2Surgical treatment of femoral deformities in polyostotic fibrous dysplasia and McCune-Albright syndrome:A literature review显示文摘BACKGROUND Surgical correction of femoral deformities in polyostotic fibrous dysplasia(PFD)or McCune-Albright syndrome(MAS),such as coxa vara or shepherd’s crook deformity,is a challenge.AIM To evaluate the treatment of patients with femoral deformities caused by PDF or MAS treated by osteotomies and stabilized with different methods,by analyzing the most relevant studies on the topic.METHODS A literature search was performed in Medline database(PubMed).Articles were screened for patients affected by PFD or MAS surgically managed by osteotomies and stabilized with different methods.RESULTS The initial search produced 184 studies,with 15 fulfilling the eligibility criteria of our study.Selected articles(1987-2019)included 111 patients overall(136 femurs).CONCLUSION Based on our results,the preferred method to stabilize corrective osteotomies is intramedullary nailing with neck cross pinning.When the deformity is limited to the proximal part of the femur,a screw or blade plate may be used,although there is a high risk of fracture below the plate.When the femur is entirely involved,a two-stage procedure may be considered.Giulio Gorgolini Alessandro Caterini Lorenzo Nicotra Fernando De Maio Kristian Efremov Pasquale Farsetti 2022World Journal of Orthopedics2022,13,3:1
3Lattice Boltzmann Methods for Multiphase Flow Simulations across Scales显示文摘The simulation of multiphase flows is an outstanding challenge,due to the inherent complexity of the underlying physical phenomena and to the fact that multiphase flows are very diverse in nature,and so are the laws governing their dynamics.In the last two decades,a new class of mesoscopic methods,based on minimal lattice formulation of Boltzmann kinetic equation,has gained significant interest as an efficient alternative to continuum methods based on the discretisation of the NS equations for non ideal fluids.In this paper,three different multiphase models based on the lattice Boltzmann method(LBM)are discussed,in order to assess the capability of the method to deal with multiphase flows on a wide spectrum of operating conditions and multiphase phenomena.In particular,the range of application of each method is highlighted and its effectiveness is qualitatively assessed through comparison with numerical and experimental literature data.Giacomo Falcucci Stefano Ubertini Chiara Biscarini Silvia Di Francesco Daniele Chiappini Silvia Palpacelli Alessandro De Maio Sauro Succi 2011Communications in Computational Physics2011,9,2:0
