1老年非酒精性脂肪肝患者的血脂、脂蛋白和胱抑素C的水平变化显示文摘目的探讨老年非酒精性脂肪肝患者的血脂、脂蛋白和胱抑素C的水平。方法收集住院的老年非酒精性脂肪肝患者100例作为非酒精性脂肪肝组,同期住院的肝功能正常的老年患者100例作为对照组,分析两组患者血清甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、脂蛋白a(LPa)、载脂蛋白(Apo)A1、ApoB和胱抑素C的水平。结果非酒精性脂肪肝组患者TG、TC、LDL-C水平明显高于对照组(P<0.05),非酒精性脂肪肝组HDL-C水平明显低于对照组(P<0.05)。非酒精性脂肪肝组高血压、高脂血症和糖尿病的患病率均明显高于对照组(P<0.05),非酒精性脂肪肝组动脉粥样硬化的患病率和对照组比较没有差异(P>0.05)。非酒精性脂肪肝组LPa水平明显低于对照组(P<0.05),非酒精性脂肪肝组ApoA1和ApoB和对照组比较没有差异(P>0.05)。非酒精性脂肪肝组胱抑素C水平明显高于对照组(P<0.05)。结论老年非酒精性脂肪肝和血清TG、TC、LDL-C、HDL-C、LPa、和胱抑素C水平变化有一定关系,非酒精性脂肪肝增加高血压、高脂血症和糖尿病的患病率。叶咸德 2016中国老年学杂志2016,36,10:20
2WJH 6^(th) Anniversary Special Issues(7): Nonalcoholic fatty liver disease Pathogenesis and therapeutic approaches for non-alcoholic fatty liver disease显示文摘Non-alcoholic fatty liver disease affects approximately one-third of the population worldwide, and its incidence continues to increase with the increasing prevalence of other metabolic disorders such as type 2 diabetes. As non-alcoholic fatty liver disease can progress to liver cirrhosis, its treatment is attracting greater attention. The pathogenesis of non-alcoholic fatty liver disease is closely associated with insulin resistance and dyslipidemia, especially hypertriglyceridemia. Increased serum levels of free fatty acid and glucose can cause oxidative stress in the liver and peripheral tissue, leading to ectopic fat accumulation, especially in the liver. In this review, we summarize the mechanism underlying the progression of hepatic steatosis to steatohepatitis and cirrhosis. We also discuss established drugs that are already being used to treat non-alcoholic fatty liver disease, in addition to newly discovered agents, with respect to their mechanisms of drug action, focusing mainly on hepatic insulin resistance. 3Recent insights into farnesoid X receptor in non-alcoholic fatty liver disease显示文摘Non-alcoholic fatty liver disease(NAFLD) is the hepatic manifestation of metabolic syndrome and is one of the most prevalent liver disorders worldwide. NAFLD can gradually progress to liver inflammation, fibrosis, cirrhosis and even hepatocellular carcinoma. However, the pathogenesis of NAFLD is complex, and no efficient pharmaceutic treatments have yet been established for NAFLD. Accumulating data have shown that the farnesoid X receptor(FXR) plays important roles not only in bile acid metabolism, but also in lipid and carbohydrate homeostasis, inflammatory responses, among others. 4载脂蛋白A5(ApoA5)基因多态性与动脉粥样硬化性脑梗死关系的研究显示文摘目的研究脑梗死患者的动脉粥样硬化程度和载脂蛋白A5(ApoA5)-1131T/C和553G/T两位点的关系。方法选取2011-12—2013-12间我院医学影像科计算机体层扫描和磁共振确诊的脑梗死患者156例,分析其颈总动脉(CCA)和颈内动脉(ICA)内膜厚度(IMT)与载脂蛋白A5(ApoA5)-1131T/C和553G/T两位点关系。结果在-1131T/C位点基因型中,CC患者颈总动脉内膜为(1.12±0.27)mm,TT+TC患者为(1.07±0.12)mm,差异无统计学意义(P>0.05)。CC患者颈内动脉内膜为(1.04±0.15)mm,TT+TC患者为(0.92±0.17)mm,差异有统计学意义(P<0.05);另外,在553G/T位点基因型中,GG患者颈总动脉内膜为(1.06±0.19)mm,TT+GT患者为(1.15±0.16)mm,差异有统计学意义(P<0.05);GG患者颈内动脉内膜为(0.94±0.15)mm,TT+GT患者为(0.97±0.21)mm,差异无统计学意义(P>0.05)。对2组不同基因组的不同基因型患者进行相关血脂水平鉴定,发现-1131T/C位点基因型的两种不同基因型的TG水平差异有统计学意义(P<0.05),其余基因型相比差异均无统计学意义(P>0.05)。对载脂蛋白A5(ApoA5)基因的常见单倍体配型不同的患者进行CCA IMT和ICA IMT的测量发现,4组基因型中ICA IMT差异有统计学意义(P<0.05)。结论载脂蛋白A5(ApoA5)基因对动脉粥样硬化脑梗死关系相对较大,值得进一步研究。洪雁 周碧燕 李洁 梁智 张千 2015中国实用神经疾病杂志2015,18,10:5
5Non-alcoholic fatty liver disease-a chronic disease of the 21^(st) century显示文摘Non-alcoholic fatty liver disease(NAFLD) comprises a spectrum of metabolic states ranging from simple steatosis to inflammation with associated fibrosis to cirrhosis. Though accumulation of hepatic fat is not associated with a significant increase in mortality rates, hepatic inflammation is, as this augments the risk of terminal liver disease, i.e.,cirrhosis, hepatic decompensation(liver failure) and/or hepatocellular carcinoma. Disease progression is usually slow, over a decade or more and, for the most part, remains asymptomatic. Recent estimates suggest that the global prevalence of NAFLD is high, about one in four. In most cases, NAFLD overlaps with overweight, obesity,cardiovascular disease and the metabolic syndrome with numerous contributing parameters including a dysregulation of adipose tissue, insulin resistance, type 2 diabetes, changes in the gut microbiome, neuronal and hormonal dysregulation and metabolic stress. NAFLD is diagnosed incidentally, despite its high prevalence. Non-invasive imaging techniques have emerged, making it possible to determine degree of steatosis as well asfibrosis. Despite this,the benefit of routine diagnostics remains uncertain. A better understanding of the(molecular) pathogenesis of NAFLD is needed combined with long-term studies where benefits of treatment can be assessed to determine costbenefit ratios. 7Efficacy of polyphenolic ingredients of Chinese herbs in treating dyslipidemia of metabolic syndromes显示文摘There is an increasing interest and popularity of Chinese herbal medicine worldwide, which is accompanied by increasing concerns about its effectiveness and potential toxicity.Several ingredients, such as polyphenolic compounds berberine, flavonoids, and curcumin, have been studied extensively by using various animal models.Effectiveness of treatment and amelioration of metabolic syndromes, including insulin resistance and dyslipidemia, has been demonstrated.This review summarizes the major checkpoints and contributing factors in regulation of exogenous and endogenous lipid metabolism, with particular emphasis centered on triglyceride-rich and cholesterol-rich lipoproteins.Available experimental evidence demonstrating the lipid-lowering effect of berberine, fl avonoids and curcumin in cell culture and animal models is compiled, and the strengths and shortcomings of experimental designs in these studies are discussed.Zemin Yao Li Zhang Guang Ji 2014Journal of Integrative Medicine2014,12,3:3
8游离脂肪酸与2型糖尿病合并心肌梗死患者胰岛素抵抗的相关性显示文摘目的探讨游离脂肪酸和2型糖尿病合并心肌梗死患者胰岛素抵抗的关系。方法对60例心肌梗死患者进行回顾分析,30例2型糖尿病合并心肌梗死患者为糖尿病组,30例非糖尿病的心肌梗死患者为非糖尿病组,并选取30wgqj非心肌梗死住院患者作为健康对照组。结果非糖尿病组和糖尿病组患者第1天空腹血糖水平均高于健康对照组,第12天餐后2 h血糖、胰岛素及C肽水平均高于健康对照组,差异有统计学意义(P<0.05)。非糖尿病组患者第1、12天空腹血糖水平及第12天餐后2 h血糖、C肽水平均低于糖尿病组同期(P<0.05)。糖尿病组患者第12天餐后2 h血糖、胰岛素及C肽水平均高于同组第1天餐后2 h(P<0.05)。结论游离脂肪酸、餐后血糖及其胰岛素水平升高与心肌梗死相关的胰岛素抵抗有关。张佩 范海涛 寇雪燕 2018宁夏医学杂志2018,40,4:1
