| 1 | 胆囊癌的临床诊疗现状与挑战显示文摘胆囊癌在胃肠道癌症中位居第五位,同时它也是胆道最常见的恶性肿瘤,约占80%~95%。这种肿瘤预后不佳,总体5年生存率低于5%,平均中位生存期〈6个月。因为胆囊癌在进展时可以没有任何症状,所以早期诊断是至关重要。术前诊断为胆囊结石,而术后确诊为胆囊癌的发生率为0.5%~1.5%。如果术前怀疑患者胆囊癌,则不应通过腹腔镜手术。流行病学研究已经确定了胆囊癌的发病率有地理和种族的显著差异:美洲的印第安人高发,东南亚地区也比较高,但在美洲和世界的其他地区发病率较低。环境诱发因素在胆囊癌的发展过程中发挥关键作用,胆石症、胆道和寄生虫感染的慢性炎症是最好的例证。在过去的10年里,改进成像技术和改良根治的手术方法可改善患者的预后,并帮助延长胆囊癌患者的存活时间。对于R0切除的胆囊癌患者,其总体5年生存率为21%~69%。在将来,潜在的诊断标志物的发展将对那些种族易感性或已知胆道解剖异常的人群提供早期筛查诊断的机会。 | Mislav Raki? | 2 | Gardner's syndrome:Genetic testing and colonoscopy are indicated in adolescents and young adults with cranial osteomas:A case report显示文摘We present a case of a 25-year-old female with diagnosed familial adenomatous polyposis and elevated carcinoembryonic antigen with negative family history. The suspicion of Gardner's syndrome was raised because extirpation of an osteoma of the left temporo-occipital region was made 10 years ago. Restorative procto-colectomy and ileal pouch anal anastomosis was made but histology delineated adenocarcinoma of the rectum (Dukes C stage). We conclude that cranial osteomas often precede gastrointestinal manifestations of familial adenomatous polyposis or Gardner's syndrome and such patients should be evaluated with genetic testing followed by colonoscopy if results are positive to prevent the development of colorectal carcinoma. | 3 | Colonic duplication in an adult who presented with chronic constipation attributed to hypothyroidism显示文摘Gastrointestinal duplications are an uncommon congenital abnormality that manifest before the age of two in 80% of cases. Ileal duplication is the most common while colonic duplication,either cystic or tubular,occurs in 10%-15% of cases and remains asymptomatic and undiagnosed in most cases. Mostly occurring in pediatric patients,colonic duplication is encountered in adults in only a few cases. The most common clinical manifestations are abdominal pain and intestinal obstruction. Rarely,duplications present with signs of acute abdomen or acute bleeding. This study reports a case of colonic duplication in an adult who presented with chronic constipation. Complete diagnostic workup was made on several occasions during the previous eight year period,but no pathology was found and chronic constipation was attributed to hypothyroidism caused by long standing Hashimoto thyroiditis. Multislice CT,performed because of abdominal distension,defined colonic pathology but the definite diagnosis of duplication of the transversal colon was made at operation. The cystic duplication and the adjacent part of the ascending and transversal colon were excised en-block. | 5 | Esophagogastric anastomosis in rats: Improved healing by BPC 157 and L-arginine, aggravated by L-NAME显示文摘AIM To cure typically life-threatening esophagogastric anastomosis in rats, lacking anastomosis healing and sphincter function rescue, in particular. METHODS Because we assume esophagogastric fistulas represent a particular NO-system disability, we attempt to identify the benefits of anti-ulcer stable gastric pentadecapeptide BPC 157, which was in trials for ulcerative colitis and currently for multiple sclerosis, in rats with esophagocutaneous fistulas. Previously, BPC 157 therapies have promoted the healing of intestinal anastomosis and fistulas, and esophagitis and gastric lesions, along with rescued sphincter function. Additionally, BPC 157 particularly interacts with the NOsystem. In the 4 d after esophagogastric anastomosis creation, rats received medication(/kg intraperitoneallyonce daily: BPC 157(10 μg, 10 ng), L-NAME(5 mg), or L-arginine(100 mg) alone and/or combined or BPC 157(10 μg, 10 ng) in drinking water). For rats underwent esophagogastric anastomosis, daily assessment included progressive stomach damage(sum of the longest diameters, mm), esophagitis(scored 0-5), weak anastomosis(m L H2 O before leak), low pressure in esophagus at anastomosis and in the pyloric sphincter(cm H2O), progressive weight loss(g) and mortality. Immediate effect assessed blood vessels disappearance(scored 0-5) at the stomach surface immediately after anastomosis creation. RESULTS BPC 157(all regimens) fully counteracted the perilous disease course from the very beginning(i.e., with the BPC 157 bath, blood vessels remained present at the gastric surface after anastomosis creation) and eliminated mortality. Additionally, BPC 157 treatment in combination with L-NAME nullified any effect of L-NAME that otherwise intensified the regular course. Consistently, with worsening(with L-NAME administration) and amelioration(with L-arginine), either L-arginine amelioration prevails(attenuated esophageal and gastric lesions) or they counteract each other(L-NAME + L-arginine); with the addition of BPC 157(L-NAME + L-arginine + BPC 157), there was a marked beneficial effect. BPC 157 treatment for esophagogastric anastomosis, along with NOS-blocker L-NAME and/or NOS substrate L-arginine, demonstrated an innate NO-system disability(as observed with L-arginine effectiveness). | 7 | Test-retest reliability of the 30-15 Intermittent Fitness Test:A systematic review显示文摘Purpose:This review aimed to synthesize previous findings on the test-retest reliability of the 30-15 Intermittent Fitness Test(IFT).Methods:The literature searches were performed in 8 databases.Studies that examined the test-retest reliability of the 30-15 IFT and presented the intraclass correlation coefficient(ICC) and/or the coefficient of variation(CV) for maximal velocity and/or peak heart rate were included.The consensus-based standards for the selection of health measurement instruments(COSMIN) checklist was used for the assessment of the methodological quality of the included studies.Results:Seven studies,with a total of 10 study groups,explored reliability of maximal velocity assessed by the 30-15 IFT.ICCs ranged from0.80 to 0.99,where 70% of ICCs were≥0.90.CVs for maximal velocity ranged from 1.5% to 6.0%.Six studies,with a total of 7 study groups,explored reliability of peak heart rate as assessed by the 30-15 IFT.ICCs ranged from 0.90 to 0.97(i.e.,all ICCs were≥0.90).CVs ranged from 0.6% to 4.8%.All included studies were of excellent methodological quality.Conclusion:From the results of this systematic review,it can be concluded that the 30-15 IFT has excellent test-retest reliability for both maximal velocity and peak heart rate.The test may,therefore,be used as a reliable measure of fitness in research and sports practice. | Jozo Grgic Bruno Lazinica Zeljko Pedisic | 2021 | Journal of Sport and Health Science2021，10，4： | 2 |
