| 1 | Overexpression of the M2 isoform of pyruvate kinase is an adverse prognostic factor for signet ring cell gastric cancer显示文摘AIM:To investigate M2 isoform of pyruvate kinase(PKM2) expression in gastric cancers and evaluate its potential as a prognostic biomarker and an anticancer target.METHODS:All tissue samples were derived from gastric cancer patients underwent curative gastrectomy as a primary treatment.Clinical and pathological information were obtained from the medical records.Gene expression microarray data from 60 cancer and 19 noncancer gastric tissues were analyzed to evaluate the expression level of PKM2 mRNA.Tissue microarrays were constructed from 368 gastric cancer patients.Immunohistochemistry was used to measure PKM2 expression and PKM2 positivity of cancer was determined by proportion of PKM2-positive tumor cells and staining intensity.Association between PKM2 expression and the clinicopathological factors was evaluated and the correlation between PKM2 and cancer prognosis was evaluated.RESULTS:PKM2 mRNA levels were increased more than 2-fold in primary gastric cancers compared to adjacent normal tissues from the same patients(log transformed expression level:7.6 ± 0.65 vs 6.3 ± 0.51,P < 0.001).Moreover,differentiated type cancers had significantly higher PKM2 mRNA compared to undifferentiated type cancers(log transformed expression level:7.8 ± 0.70 vs 6.7 ± 0.71,P < 0.001).PKM2 protein was mainly localized in the cytoplasm of primary cancer cells and detected in 144 of 368(39.1%) human gastric cancer cases.PKM2 expression was not related with stage(P = 0.811),but strongly correlated with gastric cancer differentiation(P < 0.001).Differentiated type cancers expressed more PKM2 protein than did the undifferentiated ones.Well differentiated adenocarcinoma showed 63.6% PKM2-positive cells;in contrast,signet-ring cell cancers showed only 17.7% PKM2-positive cells.Importantly,PKM2 expression was correlated with shorter overall survival(P < 0.05) independent of stage only in signet-ring cell cancers.CONCLUSION:PKM2 expression might be an adverse prognostic factor for signet-ring cell carcinomas.Its function and potential as a prognostic marker should be further verified in gastric cancer. | Jae Yun Lim Sun Och Yoon So Young Seol Soon Won Hong Jong Won Kim Seung Ho Choi Jae Yong Cho | 2012 | World Journal of Gastroenterology2012,18,30: | 19 |
| 2 | Treatment strategies using adefovir dipivoxil for individuals with lamivudine-resistant chronic hepatitis B显示文摘AIM:To investigate retrospectively the long-term efficacy of various treatment strategies using adefovir dipivoxil(adefovir) in patients with lamivudine-resistant chronic hepatitis B.METHODS:We included 154 consecutive patients in two treatment groups:the 'add-on' group(n = 79),in which adefovir was added to ongoing lamivudine treatment due to lamivudine resistance,and the 'switch/combination' group(n = 75),in which lamivudine was first switched to adefovir and then re-added later as needed.The 'switch/combination' group was then divided into two subgroups depending on whether participants followed(group A,n = 30) or violated(group B,n = 45) a proposed treatment strategy that determined whether to add lamivudine based on the serum hepatitis B virus(HBV) DNA levels(< 60 IU/mL or not) after 6 mo of treatment(roadmap concept).RESULTS:The cumulative probability of virologic response(HBV DNA < 60 IU/mL) was higher in group A than in the 'add-on' group and in group B(P < 0.001).In contrast,the cumulative probability of virologic breakthrough was lower in the 'add-on' group than in group B(P = 0.002).Furthermore,the risk of virologic breakthrough in the multivariate analysis was significantly lower in the 'add-on' group than in group A(hazard ratio = 0.096;95%CI,0.015-0.629;P = 0.015).CONCLUSION:The selective combination of adefovir with lamivudine based upon early treatment responses increased the odds of virologic breakthrough relative to the use of uniform combination therapy from the beginning of treatment. | Tae Jung Yun Jin Yong Jung Chang Ha Kim Soon Ho Um Hyonggin An Yeon Seok Seo Jin Dong Kim Hyung Joon Yim Bora Keum Yong Sik Kim Yoon Tae Jeen Hong Sik Lee Hoon Jai Chun Chang Duck Kim Ho Sang Ryu | 2012 | World Journal of Gastroenterology2012,18,47: | 19 |
| 3 | Prognostic value of inflammation-based markers in patients with pancreatic cancer administered gemcitabine and erlotinib显示文摘AIM: To evaluate the value of systemic inflammationbased markers as prognostic factors for advanced pancreatic cancer(PC). METHODS: Data from 82 patients who underwent combination chemotherapy with gemcitabine and erlotinib for PC from 2011 to 2014 were collected retrospectively. Data that included the neutrophil-to-lymphocyte ratio(NLR), the platelet-to-lymphocyte ratio, and the C-reactive protein(CRP)-to-albumin(CRP/Alb) ratio were analyzed. Kaplan-Meier curves, and univariate and multivariate Cox proportional hazards regression analyses were used to identify the prognostic factors associated with progression-free survival(PFS) and overall survival(OS). RESULTS: The univariate analysis demonstrated the prognostic value of the NLR(P = 0.049) and the CRP/Alb ratio(P = 0.047) in relation to PFS, and a positiverelationship between an increase in inflammation-based markers and a poor prognosis in relation to OS. The multivariate analysis determined that an increased NLR(hazard ratio = 2.76, 95%CI: 1.33-5.75, P = 0.007) is an independent prognostic factor for poor OS. There was no association between the PLR and the patients' prognoses in those who had received chemotherapy that comprised gemcitabine and erlotinib in combination. | 4 | Overexpression of miR-196b and HOXA10 characterize a poor-prognosis gastric cancer subtype显示文摘AIM:To identify molecular biologic differences between two gastric adenocarcinoma subgroups presenting different prognoses through the analysis of microRNA and protein expression.METHODS:Array technologies were used to generate1146 microRNAs and 124 proteins expression profiles of samples from 60 patients with gastric cancer.For the integrative analysis,we used established mRNA expression data published in our previous study.Whole mRNA expression levels were acquired from microarray data for 60 identical gastric cancer patients.Two gastric adenocarcinoma subgroups with distinct mRNA expression profiles presented distinctly different prognoses.MicroRNA and protein expression patterns were compared between gastric cancer tissue and normal gastric tissue and between two different prognostic groups.Aberrantly expressed microRNA,associated mRNA,and protein in patients with poor-prognosis gastric cancer were validated by quantitative reverse transcription polymerase chain reaction and immunochemistry in independent patients.RESULTS:We obtained the expression data of 1146microRNAs and 124 cancer-related proteins.Four microRNAs were aberrantly expressed in the two prognostic groups and in cancer vs non-cancer tissues(P<0.05).In the poor-prognosis group,miR-196b,miR-135b,and miR-93 were up-regulated and miR-29c*was down-regulated.miR-196b expression positively correlated with Homeobox A10(HOXA10)expression(r=0.726,P<0.001),which was significantly increased in poor-prognosis patients(P<0.001).Comparing gastric cancer with non-cancer tissues,46/124 proteins showed differential expression(P<0.05);COX2(P<0.001)and cyclin B1(P=0.017)were clearly overexpressed in the poor-prognosis group.CONCLUSION:Co-activation of miR-196b and HOXA10characterized a poor-prognosis subgroup of patients with gastric cancer.Elucidation of the biologic function of miR-196b and HOXA10 is warranted. | Jae Yun Lim Sun Och Yoon So-Young Seol Soon Won Hong Jong Won Kim Seung Ho Choi Ju-Seog Lee Jae Yong Cho | 2013 | World Journal of Gastroenterology2013,19,41: | 10 |
| 5 | Superior mechanical properties and strengthening mechanisms of lightweight AlxCrNbVMo refractory high-entropy alloys(x=0,0.5,1.0)fabricated by the powder metallurgy process显示文摘Light and strong AlxCrNbVMo(x=0,0.5,and 1.0)refractory high-entropy alloys(RHEAs)were designed and fabricated via a the powder metallurgical process.The microstructure of the AlxCrNbVMo alloys consisted of a single BCC crystalline structure with a sub-micron grain size of 2-3μm,and small amounts(<4 vol.%)of fine oxide dispersoids.This homogeneous microstructure,without chemical segregation or micropores was achieved via high-energy ball milling and spark-plasma sintering.The alloys exhibited superior mechanical properties at 25 and 1000℃compared to those of other RHEAs.Here,CrNbVMo alloy showed a yield strength of 2743 MPa at room temperature.Surprisingly,the yield strength of the CrNbVMo alloy at 1000℃was 1513 MPa.The specific yield strength of the CrNbVMo alloy was increased by 27%and 87%at 25 and 1000℃,respectively,compared to the AlMo_(0.5) NbTa_(0.5)TiZr RHEA,which exhibited so far the highest specific yield strength among the cast RHEAs.The addition of Al to CrNbVMo alloy was advantageous in reducing its reduce density to below 8.0 g/cm^(3),while the elastic modulus decreased due to the much lower elastic modulus of Al compared to that of the CrNbVMo alloy.Quantitative analysis of the strengthening contributions,showed that the solid solution strengthening,arising from a large misfit effect due to the size and modulus,and the high shear modulus of matrix,was revealed to predominant strengthening mechanism,accounting for over 50%of the yield strength of the AlxCrNbVMo RHEAs. | Byungchul Kang Taeyeong Kong Ho Jin Ryu Soon Hyung Hong | 2021 | Journal of Materials Science & Technology2021,,10: | 9 |
| 6 | Endoscopic submucosal tunnel dissection salvage technique for ulcerative early gastric cancer显示文摘Endoscopic submucosal dissection is an effective treatment modality for early gastric cancer(EGC), though the submucosal fibrosis found in ulcerative EGC is an obstacle for successful treatment. This report presents two cases of ulcerative EGC in two males, 73- and 80-year-old, with severe fibrosis. As endoscopic ultrasonography suggested that the EGCs had invaded the submucosal layer, the endoscopic submucosal tunnel dissection salvage technique was utilized for complete resection of the lesions. Although surgical gastrectomy was originally scheduled, the two patients had severe coronary heart disease, and surgeries were refused because of the risks associated with their heart conditions. The endoscopic submucosal tunnel dissection salvage technique procedures described in these cases were performed under conscious sedation, and were completed within 30 min. The complete en bloc resection of EGC using endoscopic submucosal tunnel dissection salvage technique was possible with a free resection margin, and no other complications were noted during the procedure. This is the first known report concerning the use of the endoscopic submucosal tunnel dissection salvage technique salvage technique for treatment of ulcerative EGC. | 7 | In-situ synthesis of TiC/Fe alloy composites with high strength and hardness by reactive sintering显示文摘Fe alloy composites reinforced with in-situ titanium carbide(Ti C) particles were fabricated by reactive sintering using different reactant C/Ti ratios of 0.8,0.9,1 and 1.1 to investigate the microstructure and mechanical properties of in-situ Ti C/Fe alloy composites.The microstructure showed that the in-situ synthesized Ti C particles were spherical with a size of 1–3 μm,irrespective of C/Ti ratio.The stoichiometry of in-situ Ti C increased from 0.85 to 0.88 with increasing C/Ti ratio from 0.8 to 0.9,but remained almost unchanged for C/Ti ratios between 0.9 and 1.1 due to the same driving force for carbon diffusion in Ti Cxat the common sintering temperature.The in-situ Ti C/Fe alloy composite with C/Ti = 0.9 showed improved mechanical properties compared with other C/Ti ratios because the presence of excess carbon(C/Ti = 1 and 1.1) resulted in unreacted carbon within the Fe alloy matrix,while insufficient carbon(C/Ti = 0.8)caused the depletion of carbon from the Fe alloy matrix,leading to a significant decrease in hardness.This study presents that the maximized hardness and superior strength of in-situ Ti C/Fe alloy composites can be achieved by microstructure control and stoichiometric analysis of the in-situ synthesized Ti C particles,while maintaining the ductility of the composites,compared to those of the unreinforced Fe alloy.Therefore,we anticipate that the in-situ synthesized Ti C/Fe alloy composites with enhanced mechanical properties have great potential in cutting tool,mold and roller material applications. | Junho Lee Dongju Lee Myung Hoon Song Wonhyuk Rhee Ho Jin Ryu Soon Hyung Hong | 2018 | Journal of Materials Science & Technology2018,34,8: | 8 |

| 9 | Primary signet ring cell carcinoma of the appendix:A rare case report and our 18-year experience显示文摘Primary adenocarcinoma of the appendix is a rare ma-lignancy that constitutes < 0.5% of all gastrointestinal neoplasms. Moreover, primary signet ring cell carci-noma of the appendix is an exceedingly rare entity. We have encountered 15 cases of primary appendiceal cancer among 3389 patients who underwent appen-dectomy over the past 18 years. In the present report, we describe a rare case of primary signet ring cell carcinoma of the appendix with ovarian metastases and unresectable peritoneal dissemination occurring in a 67-year-old female patient. She underwent ap-pendectomy and bilateral salpingo-oophorectomy with a laparoscopy procedure. She then received palliative systemic chemotherapy with 12 cycles of oxaliplatin, 5-flurorouracil, and leucovorin (FOLFOX-4). | 10 | Computed tomography findings for predicting severe acute hepatitis with prolonged cholestasis显示文摘AIM: To evaluate the significance of computed tomography (CT) findings in relation to liver chemistry and the clinical course of acute hepatitis. METHODS: Four hundred and twelve patients with acute hepatitis who underwent enhanced CT scanning were enrolled retrospectively. Imaging findings were analyzed for the following variables: gallbladder wall thickness (GWT), arterial heterogeneity, periportal tracking, number and maximum size of lymph nodes, presence of ascites, and size of spleen. The serum levels of alanine aminotransferase, alkaline phosphatase, bilirubin, albumin, and prothrombin time were measured on the day of admission and CT scan, and laboratory data were evaluated every 2-4 d for all subjects during hospitalization. RESULTS: The mean age of patients was 34.4 years, and the most common cause of hepatitis was hepatitis A virus (77.4%). The mean GWT was 5.2 mm. The number of patients who had findings of arterial heterogeneity, periportal tracking, lymph node enlargement > 7 mm, and ascites was 294 (80.1%), 348 (84.7%), 346 (84.5%), and 56 (13.6%), respectively. On multivariate logistic regression, male gender [odds ratio (OR) = 2.569, 95%CI: 1.477-4.469, P = 0.001], toxic hepatitis (OR = 3.531, 95%CI: 1.444-8.635, P = 0.006), level of albumin (OR = 2.154, 95%CI: 1.279-3.629, P = 0.004), and GWT (OR = 1.061, 95%CI: 1.015-1.110, P = 0.009) were independent predictive factors for severe hepatitis. The level of bilirubin (OR = 1.628, 95%CI: 1.331-1.991, P < 0.001) and GWT (OR = 1.172, 95%CI: 1.024-1.342,P = 0.021) were independent factors for prolonged cholestasis in multivariate analysis. | 11 | Thioredoxin and thioredoxin-interacting protein as prognostic markers for gastric cancer recurrence显示文摘AIM:To evaluate the potential of thioredoxin (TXN) and thioredoxin-interacting protein (TXNIP) expression as biomarkers for predicting gastric cancer recurrence. METHODS:TXN and TXNIP expression levels were acquired from gene expression microarray data for 65 human gastric cancer tissues. We determined whether each gene expression level was associated with cancer recurrence and investigated the relationship between the two genes. For validation, the expression levels of TXN and TXNIP were measured by quantitative real- time reverse transcription polymerase chain reaction in 68 independent stage Ⅲ gastric cancer patients. The correlation between gene expression and cancer prognosis was evaluated. Immunohistochemical staining was performed to investigate the protein expression levels of TXN and TXNIP and to characterize the expression patterns of each protein. RESULTS:TXN was a prognosis-related gene (P = 0.009), whereas TXNIP, a TXN inhibitor, demonstrated a negative correlation with TXN in the gene expression microarray data. In the 68 stage Ⅲ patients, the expression levels of both TXN and TXNIP had a statistically significant effect on recurrence-free survival (RFS, P = 0.008 and P = 0.036, respectively). The low TXN and high TXNIP expression group exhibited a better prognosis than the other groups, and the high TXN and low TXNIP expression group exhibited a poorer prognosis (P < 0.001 for RFS and P = 0.001 for overall survival). More than half of the patients in the simulta-neously high TXN and low TXNIP expression group ex- perienced a recurrence within 1 year after curative surgery, and the 5-year survival rate of the patients in this group was 29%, compared with 89% in the low TXN and high TXNIP expression group. The TXN protein was overexpressed in 65% of the gastric cancer tissues, whereas the TXNIP protein was underexpressed in 85% of the cancer cells. In a correlation analysis, TXN and TXNIP were highly correlated with many oncogenes and tumor suppressors as well as with genes related to energy, protein synthesis and autophagy. | 12 | Study on SCR De NO_x mechanism through in situ electrical conductivity measurements on V_2O_5-WO_3/TiO_2 catalysts显示文摘V2O5-WO3/TiO2 catalysts were prepared by impregnation method and in situ electrical conductivity measurements were carried out to investigate the reaction mechanism for ammonia SCR (selective catalytic reduction) of NOx. The electrical conductivity change with ammonia supply and the increase of electrical conductivity were mainly caused by reduction of the labile surface oxygen. The electrical conductivity change of catalysts shows close relationship with the conversion rate of NOx. Variation of conversion rate in atmosphere without gaseous oxygen also supports that the labile lattice oxygen is indispensable in the initial stage of the de NOx reaction. These results suggest that the liable lattice oxygen acts decisive role in the de NOx mechanism. They also support that De NOx reaction occurs through the Eley-Rideal type mechanism. | 14 | Self-expandable metal stents in patients with postoperative delayed gastric emptying after distal gastrectomy显示文摘AIM To investigate the efficacy and safety of endoscopic stent insertion in patients with delayed gastric emptying after gastrectomy.METHODS In this study, we prospectively collected data from patients who underwent stent placement for delayed gastric emptying(DGE) after distal gastrectomy between June 2010 and April 2017, at a tertiary referral academic center. Clinical improvement, complications, and consequences after stent insertion were analyzed.RESULTS Technical success was achieved in all patients(100%). Early symptom improvement was observed in 15 of 20 patients(75%) and clinical success was achieved in all patients. Mean follow-up period was 1178.3 ± 844.1 d and median stent maintenance period was 51 d(range 6-2114 d). During the follow-up period, inserted stents were passed spontaneously per rectum without any complications in 14 of 20 patients(70%). Symptom improvement was maintained after stent placement without the requirement of any additional intervention in 19 of 20 patients(95%).CONCLUSION Endoscopic stent placement provides prompt relief of obstructive symptoms. | 16 | Clinical outcomes of endoscopic papillectomy of ampullary adenoma: A multi-center study显示文摘BACKGROUND Ampullary adenoma is a rare premalignant lesion,but its incidence is increasing.Endoscopic papillectomy has become the first treatment of choice for ampullary adenomas due to its safety and effectiveness,thereby replacing surgical resection.However,recurrence rates and adverse events after endoscopic papillectomy were reported in up to 30%of cases.AIM To review the long-term outcomes of endoscopic papillectomy and investigate the factors that affect these outcomes.METHODS We retrospectively analyzed the data of patients who underwent endoscopic papillectomy for ampullary adenoma at five tertiary hospitals between 2013 and 2020.We evaluated clinical outcomes and their risk factors.The definitions of outcomes were as follow:(1)curative resection:complete endoscopic resection without recurrence;(2)endoscopic success:treatment of ampullary adenoma with endoscopy without surgical intervention;(3)early recurrence:reconfirmed adenoma at the first endoscopic surveillance;and(4)late recurrence:reconfirmed adenoma after the first endoscopic surveillance.RESULTS A total of 106 patients were included for analysis.Of the included patients,81(76.4%)underwent curative resection,99(93.4%)had endoscopic success,showing that most patients with noncurative resection were successfully managed with endoscopy.Sixteen patients(15.1%)had piecemeal resection,22 patients(20.8%)had shown positive/uncertain resection margin,11 patients(16.1%)had an early recurrence,13 patients(10.4%)had a late recurrence,and 6 patients(5.7%)had a re-recurrence.In multivariate analysis,a positive/uncertain margin[Odds ratio(OR)=4.023,P=0.048]and piecemeal resection(OR=6.610,P=0.005)were significant risk factors for early and late recurrence,respectively.Piecemeal resection was also a significant risk factor for non-curative resection(OR=5.424,P=0.007).Twenty-six patients experienced adverse events(24.5%).CONCLUSION Endoscopic papillectomy is a safe and effective treatment for ampullary adenomas.Careful selection and follow-up of patients is mandatory,particularly in cases with positive/uncertain margin and piecemeal resection. | Seong Ji Choi Hong Sik Lee Jiyeong Kim Jung Wan Choe Jae Min Lee Jong Jin Hyun Jai Hoon Yoon Hyo Jung Kim Jae Seon Kim Ho Soon Choi | 2022 | World Journal of Gastroenterology2022,28,17: | 2 |
