| 2 | Steroid hyperglycemia: Prevalence, early detection and therapeutic recommendations: A narrative review显示文摘Steroids are drugs that have been used extensively in a variety of conditions. Although widely prescribed for their anti-inflammatory and immunosuppressive properties, glucocorticoids have several side effects, being hyperglycemia one of the most common and representative. In the present review, we discuss the main epidemiologic characteristics associated with steroid use, with emphasis on the identification of high risk populations. Additionally we present the pathophysiology of corticosteroid induced hyperglycemia as well as the pharmacokinetics and pharmacodynamics associated with steroid use. We propose a treatment strategy based on previous reports and the understanding of the mechanism of action of both, the different types of glucocorticoids and the treatment options, in both the ambulatory and the hospital setting. | 3 | A review of feed efficiency in swine:biology and application显示文摘Feed efficiency represents the cumulative efficiency with which the pig utilizes dietary nutrients for maintenance,lean gain and lipid accretion.It is closely linked with energy metabolism,as the oxidation of carbon-containing components in the feed drive all metabolic processes.While much is known about nutrient utilization and tissue metabolism,blending these subjects into a discussion on feed efficiency has proven to be difficult.For example,while increasing dietary energy concentration will almost certainly increase feed efficiency,the correlation between dietary energy concentration and feed efficiency is surprisingly low.This is likely due to the plethora of non-dietary factors that impact feed efficiency,such as the environment and health as well as individual variation in maintenance requirements,body composition and body weight.Nonetheless,a deeper understanding of feed efficiency is critical at many levels.To individual farms,it impacts profitability.To the pork industry,it represents its competitive position against other protein sources.To food economists,it means less demand on global feed resources.There are environmental and other societal implications as well.Interestingly,feed efficiency is not always reported simply as a ratio of body weight gain to feed consumed.This review will explain why this arithmetic calculation,as simple as it initially seems,and as universally applied as it is in science and commerce,can often be misleading due to errors inherent in recording of both weight gain and feed intake.This review discusses the importance of feed efficiency,the manner in which it can be measured and reported,its basis in biology and approaches to its improvement.It concludes with a summary of findings and recommendations for future efforts. | John F.Patience Mariana C.Rossoni-Seräo Nestor A.Gutiérrez | 2015 | Journal of Animal Science and Biotechnology2015,6,4: | 10 |
| 4 | Pancreatic and pulmonary mast cells activation during experimental acute pancreatitis显示文摘AIM:To study the activation of pancreatic and pulmonary mast cells and the effect of mast cell inhibition on the activation of peritoneal and alveolar macrophages during acute pancreatitis.METHODS:Pancreatitis was induced by intraductal infusion of 5% sodium taurodeoxycholate in rats.The mast cell inhibitor cromolyn was administered intraperitoneally(i.p.) 30 min before pancreatitis induction.The pancreatic and pulmonary tissue damage was evaluated histologically and mast cells and their state of activation were evaluated.Peritoneal and alveolar macrophages were obtained and the expression of tumor necrosis factor α was determined.Myeloperoxidase activity was measured to evaluate the effect of mast cell inhibition on the progression of the inflammatory process.Finally,the effect of plasma on cultured mast cells or macrophages was evaluated in vitro.RESULTS:The mast cell stabilizer signif icantly reduced inflammation in the pancreas and lung and the activation of alveolar macrophages but had no effect on peritoneal macrophages.Mast cell degranulation was observed in the pancreas during pancreatitis but no changes were observed in the lung.Plasma from rats with pancreatitis could activate alveolar macrophages but did not induce degranulation of mast cells in vitro.CONCLUSION:Pancreatic mast cells play an important role in triggering the local and systemic inflammatory response in the early stages of acute pancreatitis.In contrast,lung mast cells are not directly involved in the inflammatory response related to pancreatic damage. | Inmaculada Lopez-Font Sabrina Gea-Sorlí Enrique de-Madaria Luis M Gutiérrez Miguel Pérez-Mateo Daniel Closa | 2010 | World Journal of Gastroenterology2010,16,27: | 10 |
| 5 | Management of occult hepatitis B virus infection:An update for the clinician显示文摘Occult hepatitis B virus(HBV) infection(OBI) is defined by the presence of HBV DNA in the liver tissue of individuals who test negative for hepatitis B surface antigen(HBsAg).Patients who have recovered from acute hepatitis B can carry HBV genomes for a long time and show histological patterns of mild necro-inflammation,even fibrosis,years after the resolution of acute hepatitis,without showing any clinical or biochemical evidence of liver disease.At least in conditions of immunocompetence,OBI is inoffensive itself,but when other relevant causes of liver damage are present it might make the course of the liver disease worse.The risk of HBV transmission through transfusion is related to blood donations negative for HBsAg that have been collected during the pre-seroconversion period or during chronic OBI.Use of HBV nucleic acid amplification testing and multivalent anti-HBs antibodies in the HBsAg assays is recommended for detection of true and false OBI,respectively.It is not known if prior hepatitis B immunization with an optimal anti-HBs response in cases of HBV transmission through organ transplantation can effectively modulate or abort the infection.Use of anti-viral agents as prophylaxis in patients with serological evidence of past HBV infection prevents reactivation of OBI after transplantation in most cases.Reactivation of OBI has been observed in other conditions that cause immunosuppression,in which antiviral therapy could be delayed until the HBV DNA or HBsAg becomes detectable.OBI might contribute to the progression of liver fibrosis and hepatocellular carcinoma development in patients with chronic liver disease. | JoséLuis Lledó Conrado Fernández María Luisa Gutiérrez Sara Ocaa | 2011 | World Journal of Gastroenterology2011,17,12: | 9 |

| 7 | Clinical characteristics and surgical treatment of schwannomas of the esophagus and stomach: A case series and systematic review显示文摘BACKGROUND Gastrointestinal schwannomas are slow-growing benign mesenchymal neoplasms that originate from Schwann cells of the nerve sheath of Auerbach′s plexus or less frequently from Meissner′s plexus.The main differential diagnosis of gastric schwannomas are the gastrointestinal stromal tumors(GISTs),which are classified by their immunohistochemistry.The treatment of choice for gastric schwannomas is surgery where laparoscopy plays an important role.Wedge resection,subtotal or total gastrectomy can be done.In its counterpart,esophageal schwannomas are benign tumors of the esophagus that are very uncommon since they comprise less than 2%of all esophageal tumors.The main differential diagnosis is the leiomyoma which corresponds to the most common benign esophageal tumor,followed by GIST.The treatment consists on tumoral enucleation or esophagectomy.AIM To review the available literature about gastrointestinal schwannomas;especially lesions from de stomach and esophagus,including diagnosis,treatment,and follow up,as well as,reporting our institutional experience.METHODS A systematic review was conducted following the Preferred Reporting Items for Systematic Reviews and Meta-Analyzes guidelines.The following databases were used for reviewing process:Pub Med,Ovid,MEDLINE,and Scopus.Only English language manuscripts were included.All gastrointestinal schwannomas specifically located in the esophagus and stomach were included.Cases that did not report long-term follow-up were excluded.RESULTS Gastric localization showed a higher prevalence in both,the literature review and our institution:94.95%(n=317)and 83%(n=5)respectively.With a follow-up with disease-free survival greater than 36 mo in most cases:62.01%(n=80)vs66.66%(n=4).In both groups,the median size was>4.1 cm.Surgical treatment is curative in most cases CONCLUSION Schwannoma must be taken into account in the differential diagnosis of gastrointestinal mesenchymal tumors.It has a good prognosis,and most are benign.A disease-free survival of more than 36 mo can be achieved by surgery. | Jesús Morales-Maza Francisco Ulises Pastor-Sifuentes Germán E Sánchez-Morales Emilio Sanchez-Garcia Ramos Oscar Santes Uriel Clemente-Gutiérrez Adriana Simoneta Pimienta-Ibarra Heriberto Medina-Franco | 2019 | World Journal of Gastrointestinal Oncology2019,11,9: | 7 |
| 8 | Subclinical carotid atherosclerosis predicts all-cause mortality and cardiovascular events in obese patients with negative exercise echocardiography显示文摘BACKGROUND Obesity is a major health problem due to its high prevalence. The relationship between obesity and cardiovascular disease is unclear. Some studies agree that certain conditions associated with obesity, such as physical inactivity or cardiovascular risk factors, are responsible for cardiovascular risk excess among obese people. Carotid intima-media thickness and carotid plaques(CP) have been associated with cardiovascular adverse events in healthy populations, and recent data suggest a higher prevalence of subclinical carotid atherosclerosis in obese and metabolically unhealthy patients. However, there are no studies correlating subclinical atherosclerosis and adverse events(AE) in obese subjects.AIM To determine the association between carotid disease and AE in obese patients with negative exercise echocardiography(EE).METHODS From January 1, 2006 to December 31, 2010, 2000 consecutive patients with a suspicion of coronary artery disease were submitted for EE and carotid ultrasonography. Exclusion criteria included previous vascular disease, left ventricular ejection fraction < 50%, positive EE, significant valvular heart disease and inferior to submaximal EE. An AE was defined as all-cause mortality,myocardial infarction and cerebrovascular accident. Subclinical atherosclerosis was defined as CP presence according to Manheim and the American Society of Echocardiography Consensus.RESULTS Of the 652 patients who fulfilled the inclusion criteria, 226(34.7%) had body mass indexes ≥ 30 kg/m2, and 76 of them(33.6%) had CP. During a mean follow-up time of 8.2(2.1) years, 27 AE were found(11.9%). Mean event-free survival at 1, 5 and 10 years was 99.1%(0.6), 95.1%(1.4) and 86.5%(2.7), respectively. In univariate analysis, CP predicted AE [hazard ratio(HR) 2.52, 95% confidence interval(CI) 1.17-5.46; P = 0.019]. In multivariable analysis, the presence of CP remained a predictor of AE(HR 2.26, 95%CI 1.04-4.95, P = 0.041). Other predictors identified were glomerular filtration rate(HR 0.98, 95%CI 0.96-0.99; P= 0.023), peak metabolic equivalents(HR 0.83, 95%CI 0.70–0.99, P = 0.034) and moderate mitral regurgitation(HR 5.02, 95%CI 1.42–17.75, P = 0.012).CONCLUSION Subclinical atherosclerosis defined by CP predicts AE in obese patients with negative EE. | 9 | Gut epithelial barrier dysfunction in humanimmunodeficiency virus-hepatitis C virus coinfectedpatients:Influence on innate and acquired immunity显示文摘Even in cases where viral replication has been controlled by antiretroviral therapy for long periods of time, human immunodeficiency virus(HIV)-infected patients have several non-acquired immunodeficiency syndrome(AIDS) related co-morbidities, including liver disease, cardiovascular disease and neurocognitive decline, which have a clear impact on survival. It has been considered that persistent innate and acquired immune activation contributes to the pathogenesis of these non-AIDS related diseases. Immune activation has been related with several conditions, remarkably with the bacterial translocation related with the intestinal barrier damage by the HIV or by hepatitis C virus(HCV)-related liver cirrhosis. Consequently, increased morbidity and mortality must be expected in HIV-HCV coinfected patients. Disrupted gut barrier lead to an increased passage of microbial products and to an activation of the mucosal immune system and secretion of inflammatory mediators, which in turn might increase barrier dysfunction. In the present review, the intestinal barrier structure, measures of intestinal barrier dysfunction and the modifications of them in HIV monoinfection and in HIV-HCV coinfection will be considered. | 10 | Classification of anemia for gastroenterologists显示文摘Most anemia is related to the digestive system by dietary deficiency, malabsorption, or chronic bleeding. We review the World Health Organization definition of anemia, its morphological classifi cation (microcytic, macrocytic and normocytic) and pathogenic classi-fication (regenerative and hypo regenerative), and integration of these classifications. Interpretation of laboratory tests is included, from the simplest (blood count, routine biochemistry) to the more specific (iron metabolism, vitamin B12, folic acid, reticulocytes, erythropoietin, bone marrow examination and Schilling test). In the text and various algorithms, we propose a hierarchical and logical way to reach a diagnosis as quickly as possible, by properly managing the medical interview, physical examination, appropriate laboratory tests, bone marrow examination, and other comple-mentary tests. The prevalence is emphasized in all sections so that the gastroenterologist can direct the diagnosis to the most common diseases, although the tables also include rare diseases. Digestive diseases potentially causing anemia have been studied in preference, but other causes of anemia have been included in the text and tables. Primitive hematological diseases that cause anemia are only listed, but are not discussed in depth. | 13 | Surface Properties of Fe_4N Compounds Layer on AISI 4340 Steel Modified by Pulsed Plasma Nitriding显示文摘In this work,the effect of nitriding current density on hardness,crystalline phase composition,layer thickness and corrosion rate of AISI 4340 steel has been studied.X-ray diffraction analysis shows that thin layers formed during nitriding process are constituted of y-Fe_4N for samples processed between 1 and 2.5 mA/cm^2. | 14 | Aminoguanidine impedes human pancreatic tumor growth and metastasis development in nude mice显示文摘AIM:To study the action of aminoguanidine on pancreatic cancer xenografts in relation to cell proliferation,apoptosis,redox status and vascularization.METHODS:Xenografts of PANC-1 cells were developed in nude mice. The animals were separated into two groups:control and aminoguanidine treated. Tumor growth,survival and appearance of metastases were determined in vivo in both groups. Tumors were excised and ex vivo histochemical studies were performed. Cell growth was assessed by Ki-67 expression. Apoptosis was studied by intratumoral expression of B cell lymphoma-2 protein (Bcl-2) family proteins and Terminal deoxynucleotidyl transferase biotin-dUTP Nick End Labeling (Tunel). Redox status was evaluated by the expression of endothelial nitric oxide synthase (eNOS),catalase,copper-zinc superoxide dismutase (CuZnSOD),manganese superoxide dismutase (MnSOD) and glutathione peroxidase (GPx). Finally,vascularization was determined by Massons trichromic staining,and by VEGF and CD34 expression.RESULTS:Tumor volumes after 32 d of treatment by aminoguanidine (AG) were significantly lower than in control mice (P < 0.01). Median survival of AG mice was significantly greater than control animals (P < 0.01). The appearance of both homolateral and contralateral palpable metastases was significantly delayed in AG group. | 16 | Resource economics and coordination among above- and below-ground functional traits of three dominant shrubs from the Chilean coastal desert显示文摘Aims Plant functional traits determine how plants respond to environmen-tal factors and influence ecosystem processes.Among them,root traits and analyses of relations between above and below-ground traits in natural communities are scarce.Methods we characterized a set of above-and below-ground traits of three dominant shrub species in a semiarid shrub-steppe that had contrasting leaf pheno-logical habits(deciduous,semideciduous and evergreen).We ana-lysed if there was coordination among above-and below-ground resource economics patterns:i.e.patterns of biomass allocation,construction costs and lifespan.Important Findings Above-and below-ground traits and their resource economics relations pointed to species-specific functional strategies to cope with drought and poor soils and to a species ranking of fast to slow whole-plant strategies in terms of resource uptake,biomass con-struction costs and turnover.The deciduous shrub,Proustia cunei-folia,had relatively deep and even distribution of roots,and high proportion of short-lived tissues of low C construction costs:it had high fine to coarse root and high leaf-to-stem biomass ratios,high specific leaf area(SLA),and stems of low wood density.This strat-egy allows Proustia to maximize and coordinate above-and below-ground resources uptake as long as the most limiting factor(water)is available,but at the cost of having relative high plant biomass turnover.The evergreen Porlieria chilensis,instead,displayed a more conservative and slow strategy in terms of resource econom-ics.It had~80%of the roots in the 40 cm topsoil profile,low pro-portion of fine compared with coarse roots and low leaf-to-stem ratios,low SLA and stems of high wood density,i.e.it invested in C costly tissues that,overall,persist longer but probably at the cost of having lower plant resource uptake rates.Traits in the semide-ciduous Adesmia bedwellii were in between these two functional extremes.Our results revealed high functional diversity and above-and below-ground complementarity in resource economics among these three codominant species in the Chilean coastal desert. | Joaquín Morales Francisco A.Squeo Yann Tracol Cristina Armas Julio R.Gutiérrez | 2015 | Journal of Plant Ecology2015,8,1: | 3 |
| 17 | Downregulation of the Basic Peroxidase Isoenzyme from Zinnia elegans by Gibberellic Acid显示文摘Hypocotyl formation during the epigeal germination of seedlings is under strict hormonal regulation. In a 3 d old Zinnia elegans seedling system, gibberellic acid (GA3) exerts an opposite effect to that exerted by light on hypocotyl photomorphogenesis because GA3 promotes an etiolated-like growth with an inhibition of radial (secondary) growth. For this reason, the effect of GA3 on the basic peroxidase isoenzyme from Z. elegans (ZePrx), an enzyme involved in hypocotyl lignin biosynthesis, was studied. The results showed that GA3 reduces ZePrx activity, similarly to the way in which it reduces seedling secondary growth. This hormonal response is supported by the analysis of the ZePrx promoter, which contains four types of GA3-responsive cis-elements: the W Box/O2S; the Pyr Box; the GARE; and the Amy Box. Soil porosity is used as paradigm of complex natural porous media in this study. The use of several imaging devices and fluorescent compounds to enhance the contrast between the solid and the pore phase is tested. Finally a protocol is reached using a photo camera and a confocal microscope. It is the first time that confocal microscopy is used for this purpose. Artificial porous images are created through random Sierpinski carpet fractals and the statistical information of real soil images. These ground truth images are used in an objective comparison of automatic segmentation algorithms for the obtained images. | 19 | Effect of a bacterial inoculum and additive on dry matter in situ degradability of sugarcane silage显示文摘The objective of this study was to evaluate the effect of adding a bacterial inoculum and a handmade additive to sugarcane silage(SCS) on the in situ digestibility of dry matter(DM). The treatments were: T1) sugarcane silage(SCS) and T2) sugarcane silage with 1% inoculum and 1% additive(SCS+). The bacterial inoculum consisted of 10.0% molasses, 1.0% yogurt, 5.0% chicken manure, 0.5% urea, and 83.0% water, and the additive was formulated with 1.0% urea, 0.1% ammonium sulfate, and 0.25% phosphorus. In situ dry matter digestibility(DMD) was determined using the nylon bag technique with four cows equipped with ruminal fistulas. Cows were fed with ensiled sugarcane supplemented with 1 kg of commercial concentrate. 5 g of ground sample for each sugarcane treatment were weighted in nylon bags and incubated for 4, 8, 12, 24, 48, and 72 h in a completely randomized design with six replicates. The DMD(%) was higher(P<0.05) for SCS+ for all incubation times when compared with SCS. There were no differences in ruminal p H between the treatments for all the incubation times.
