2Clinical trial:Lactobacillus plantarum 299v(DSM 9843)improves symptoms of irritable bowel syndrome显示文摘AIM:To assess the symptomatic efficacy of Lactobacillus plantarum 299v(L.plantarum 299v)(DSM 9843) for the relief of abdominal symptoms in a large subset of irritable bowel syndrome(IBS) patients fulfilling the Rome Ⅲ criteria.METHODS:In this double blind,placebo-controlled,parallel-designed study,subjects were randomized to daily receive either one capsule of L.plantarum 299v(DSM 9843) or placebo for 4 wk.Frequency and intensity of abdominal pain,bloating and feeling of incomplete rectal emptying were assessed weekly on a visual analogue scale while stool frequency was calculated.RESULTS:Two hundred and fourteen IBS patients were recruited.After 4 wk,both pain severity(0.68 + 0.53 vs 0.92 + 0.57,P < 0.05) and daily frequency(1.01 + 0.77 vs 1.71 + 0.93,P < 0.05) were lower with L.plantarum 299v(DSM 9843) than with placebo.Similar results were obtained for bloating.At week 4,78.1 % of the patients scored the L.plantarum 299v(DSM 9843) symptomatic effect as excellent or good vs only 8.1 % for placebo(P < 0.01).CONCLUSION:A 4-wk treatment with L.plantarum 299v(DSM 9843) provided effective symptom relief,particularly of abdominal pain and bloating,in IBS patients fulfilling the Rome Ⅲ criteria.Philippe Ducrotté Prabha Sawant Venkataraman Jayanthi 2012World Journal of Gastroenterology2012,18,30:17

4二氧化硫对低氧性肺动脉高压大鼠肺动脉胶原堆积的影响显示文摘目的探讨二氧化硫(SO2)对低氧性肺动脉高压大鼠肺动脉胶原堆积的影响。方法将雄性Wistar大鼠34只分为对照组(8只)、低氧组(8只)、低氧加SO2组(腹腔注射亚硫酸钠/亚硫酸氢钠混合液0.5mL/d)(9只)和低氧加天冬氨酸异羟肟酸(HDX)组[腹腔注射HDX3.7mg/(kg·d)](9只)。对低氧组、低氧加SO2组和低氧加HDX组大鼠进行低氧处理21d,氧体积分数为(100±5)mL/L,低氧处理6h/d。采用羟脯氨酸水平测定及天狼猩红染色法分析低氧性肺动脉高压大鼠肺组织及肺动脉胶原的变化。采用SPSS13.0软件进行统计学分析。结果4组大鼠肺组织羟脯氨酸水平有显著性差异(F=12.363 Pa<0.01)。与对照组比较,低氧组大鼠羟脯氨酸水平升高34.53%(P<0.01),随着低氧性肺动脉高压的形成,大鼠肺动脉中羟脯氨酸水平升高。与低氧组比较,低氧加SO2组大鼠羟脯氨酸水平降低17.58%(P<0.05),低氧加HDX组大鼠羟脯氨酸水平升高21.30%(P<0.05)。天狼猩红偏振光法分析显示,与对照组比较,低氧组大鼠血管壁胶原纤维明显增加;与低氧组比较,低氧加SO2组大鼠血管壁Ⅰ型胶原较少,呈红色或黄色粗纤维状,与Ⅲ型胶原伴行,低氧加HDX组大鼠血管壁胶原明显增加。结论SO2能显著降低低氧大鼠的肺动脉胶原堆积。Mainali prabha 田悦 金红芳 刘雪芹 唐朝枢 杜军保 2008实用儿科临床杂志2008,23,10:12

7Management of pregnancy in Crigler Najjar syndrome type 2显示文摘Crigler Najjar syndrome is associated with indirect hyperbilirubinemia due to a deficiency of enzyme Uridine Di Phospho Glucoronosyl Transferase(UDPGT).Presented here is a case of a female in the first trimester of pregnancy,who was diagnosed to have type 2 Crigler Najjar syndrome.We also discuss the management of this rare disease especially in pregnancy.Unconjugated bilirubin can cross the placental barrier causing neuro-logical damage in the newborn.Patient was carefully monitored during pregnancy and treatment with pheno-barbitone in low doses was adjusted such that the serum bilirubin levels were below 10 mg/d L.Crigler Najjar syndrome being rare needs to be diagnosed early in pregnancy to avoid adverse fetal outcomes.Phenobarbitone being an inducer of enzyme UDPGT is used as the first line of treatment and is not teratogenic in the low doses used.Treatment protocol followed was on the basis of previous reported cases and successful perinatal outcome was achieved.Alisha Nitin Chaubal Ruchir Patel Dhaval Choksi Kaivan Shah Meghraj Ingle Prabha Sawant 2016World Journal of Hepatology2016,8,11:4
8Effect of angiotensin converting enzyme gene I/D polymorphism in South Indian children with nephrotic syndrome显示文摘Nephrotic syndrome is one of the most common childhood kidney diseases. It is mostly found in the age group of 2 to 8 years. Around 10%-15% of nephrotic syndrome cases are non-responders of steroid treatment(SRNS).Angiotensin converting enzyme(ACE)(I/D) gene association studies are important for detecting kidney disease and herein we assessed the association of ACE(I/D) polymorphism with nephrotic syndrome in South Indian children. We recruited 260 nephrotic syndrome(162 boys and 98 girls) and 218(140 boys and 78 girls) control subjects. ACE I/D polymorphism was analyzed by PCR using genotype allele specific primers. In ACE(I/D), we did not find significant association for the ungrouped data of nephrotic syndrome children and the control subjects. Kidney biopsies were done in 86 nephrotic syndrome cases(minimal change disease, n = 51;focal segmental glomerulosclerosis, n = 27;diffuse mesangial proliferation, n = 8). We segregated them into the minimal change disease/focal segmental glomerulosclerosis groups and observed that the ACE’D’ allele was identified with borderline significance in cases of focal segmental glomerulosclerosis and the ’Ⅰ’ allele was assessed as having very weak association in cases of minimal change disease. ’Ⅱ’ genotype was weakly associated with minimal change disease. Gender specific analysis revealed weak association of’ID’ genotype with female nephrotic syndrome in females. Dominant expression of DD genotype was observed in males with nephrotic syndrome. Our finding indicated that ACE(I/D) has moderate association with focal segmental glomerulosclerosis.
