1铅对肾上腺皮质细胞线粒体的氧化损伤显示文摘【目的】研究铅对肾上腺皮质细胞氧化应激和线粒体功能的影响 ,为了解其肾上腺皮质毒作用机制提供依据。【方法】原代分离培养豚鼠肾上腺皮质细胞 ,以 0、6 2 5、12 5、2 5、5 0、10 0 μmol/L醋酸铅 (PbAc)处理细胞 ,观察PbAc诱导肾上腺皮质细胞活性氧 (ROS)产生和线粒体损伤作用。ROS检测采用荧光分光光度法 ,线粒体膜电位 (MMP)和细胞存活状态采用Rh12 3和PI双标记、流式细胞术检测 ,细胞ATP水平采用化学发光法测定。【结果】PbAc染毒后肾上腺皮质细胞ROS形成水平随剂量增加而增加 ,具有剂量效应关系 [^y=4 16 +10 2 1× 1g(x+1) ,P <0 0 1,R2 =0 6 41];线粒体膜电位呈剂量依赖性降低 ,Rh12 3的平均荧光强度 (MFI)在 6 2 5~ 10 0 μmol/L各剂量组依次为 1 0 1,0 94,0 96 ,0 95和 0 91,与对照组(1 35 )比较具有显著性差异 (P <0 0 1) ;染毒后细胞死亡率轻度增加 ,与对照组 (1 0 2 % )比较 ,5 0 μmol/L和 10 0 μmol/L的PbAc组分别为 3 16 %和 3 40 % ,差异具有显著性 (P <0 0 5 ) ;ATP水平降低与PbAc剂量之间存在剂量效应关系 [^y =2 12 96 5 7- 5 15 92 5× 1g(x +1) ,P <0 0 1,R2 =0 5 6 8],剂量和时间对ATP水平的影响呈协同抑制作用 (P <0 0 1)。【结论】线粒体氧化应激介导肾?杨杏芬 庄志雄 魏青 凌莉 范瑞泉 Shen H M Ong C N 2001中山医科大学学报2001,22,1:8
2Assessment of lymph node involvement in colorectal cancer显示文摘Lymph node metastasis informs prognosis and is a key factor in deciding further management, particularly adjuvant chemotherapy. It is core to all contemporary staging systems, including the widely used tumor node metastasis staging system. Patients with nodenegative disease have 5-year survival rates of 70%-80%, implying a significant minority of patients with occult lymph node metastases will succumb to disease recurrence. Enhanced staging techniques may help to identify this subset of patients, who might benefit from further treatment. Obtaining adequate numbers of lymph nodes is essential for accurate staging. Lymph node yields are affected by numerous factors, many inherent to the patient and the tumour, but others related to surgical and histopathological practice. Good lymph node recovery relies on close collaboration between surgeon and pathologist. The optimal extent of surgical resection remains a subject of debate. Extended lymphadenectomy, extra-mesenteric lymph node dissection, high arterial ligation and complete mesocolic excision are amongst the surgical techniques with plausible oncological bases, but which are not supported by the highest levels of evidence. With further development and refinement, intra-operative lymphatic mapping and sentinel lymph node biopsy may provide a guide to the optimum extent of lymphadenectomy, but in its present form, it is beset by false negatives, skip lesions and failures to identify a sentinel node. Once resected, histopathological assessment of the surgical specimen can be improved by thorough dissection techniques, step-sectioning of tissue blocks and immunohistochemistry. More recently, molecular methods have been employed. In this review, we consider the numerous factors that affect lymph node yields, including the impact of the surgical and histopathological techniques. 3Season of the year influences infection rates following total hip arthroplasty显示文摘AIM To research the influence of season of the year on periprosthetic joint infections.METHODS We conducted a retrospective review of the entire Medicare files from 2005 to 2014. Seasons were classified as spring, summer, fall or winter. Regional variations were accounted for by dividing patients into four geographic regions as per the United States Census Bureau(Northeast, Midwest, West and South). Acute postoperative infection and deep periprosthetic infections within 90 d after surgery were tracked. RESULTS In all regions, winter had the highest incidence of periprosthetic infections(mean 0.98%, SD 0.1%) and was significantly higher than other seasons in the Midwest, South and West(P < 0.05 for all) but not the Northeast(P = 0.358). Acute postoperative infection rates were more frequent in the summer and were significantly affected by season of the year in the West.CONCLUSION Season of the year is a risk factor for periprosthetic joint infection following total hip arthroplasty(THA).
