1Shift to a low carbon society through energy systems design显示文摘Concern about global warming calls for an advanced approach for designing an energy system to reduce carbon emissions as well as to secure energy security for each country.Conventional energy systems tend to introduce different technologies with high conversion efficiency,leading to a higher average efficiency.Advanced energy systems can be achieved not by an aggregate form of conversion technologies but by an innovative system design itself.The concept of LCS(low carbon society) is a unique approach having multi-dimensional considerations such as social,economic and environmental dimensions.The LCS aims at an extensive restructuring of worldwide energy supply/demand network system by not only replacing the conventional parts with the new ones,but also integrating all the necessary components and designing absolutely different energy networks.As a core tool for the LCS design,energy-economic models are applied to show feasible solutions in future with alternatives such as renewable resources,combined heat and power,and smart grid operations.Models can introduce changes in energy markets,technology learning in capacity,and penetration of innovative technologies,leading to an optimum system configuration under priority settings.The paper describes recent trials of energy models application related to waste-to-energy,clean coal,transportation and rural development.Although the modelling approach is still under investigation,the output clearly shows possible options having variety of technologies and linkages between supply and demand sides.Design of the LCS means an energy systems design with the modelling approach,which gives solution for complex systems,choices among technologies,technology feasibility,R&D targets,and what we need to start.Toshihiko NAKATA Mikhail RODIONOV Diego SILVA Joni JUPESTA 2010Science China(Technological Sciences)2010,53,1:19
2Hyperglycemia is a significant prognostic factor of hepatocellular carcinoma after curative therapy显示文摘AIM:To evaluate whether metabolic factors are related to distant recurrence of hepatocellular carcinoma(HCC) and survival after curative treatment.METHODS:This retrospective study included 344 patients whose HCC was treated curatively by radiofrequency ablation(RFA) therapy.The mean age was 67.6 years and the mean observation period was 4.04 years.The etiological background of liver disease was hepatitis B virus infection in 30,hepatitis C virus infection in 278,excessive alcohol drinking in 9,and other in 27 patients.The Child-Pugh classification grade was A(n = 307) or B(n = 37).The number of HCC nodules was one in 260,two in 61,and three in 23 patients.For surveillance of HCC recurrence after curative therapy with RFA,patients were radiologically evaluated every 3 mo.Factors associated with distant recurrence of HCC or survival were studied.RESULTS:Inadequate maintenance of blood glucose in diabetic patients was associated with higher incidence of distant recurrence.The 1-,2-,and 3-year recurrence rates were significantly higher in diabetic patients with inadequate maintenance of blood glucose compared with the others:50.6% vs 26.8%,83.5% vs 54.4%,and 93.8% vs 73.0%,respectively(P = 0.0001).Inadequate maintenance of blood glucose was an independent predictor of distant recurrence [adjusted relative risk 1.97(95%CI,1.33-2.91),(P = 0.0007)] after adjustment for other risk factors,such as number of HCC nodules [2.03(95%CI,1.51-2.73),P < 0.0001] and initial level of serum alpha fetoprotein(AFP) [1.43(95%CI,1.04-1.97),P = 0.028].Obesity was not an independent predictor of recurrence.The incidence of distant recurrence did not differ between diabetic patients with adequate maintenance of blood glucose and non-diabetic patients.Among 232 patients who had HCC recurrence,138 had a second recurrence.The 1-,2-,and 3-year rates of second recurrence were significantly higher in diabetic patients with inadequate maintenance of blood glucose than in the others:9.0% vs 5.9%,53.1% vs 24.3%,and 69.6% vs 42.3%,respectively(P = 0.0021).Inadequate maintenance of blood glucose in diabetic patients [1.99(95%CI,1.23-3.22),P = 0.0049] and presence of multiple HCC nodules [1.53(95%CI,1.06-2.22),P = 0.024] were again significantly associated with second HCC recurrence.Inadequate maintenance of blood glucose in diabetic patients was also a significant predictor of poor survival [2.77(95%CI,1.38-5.57),P = 0.0046] independent of excessive alcohol drinking [6.34(95%CI,1.35-29.7),P = 0.019],initial level of serum AFP [3.40(95%CI,1.88-6.18),P < 0.0001] and Child-Pugh classification grade B [2.24(95%CI,1.12-4.46),P = 0.022].Comparing diabetic patients with inadequate maintenance of blood glucose vs the others,the 1-,2-,and 3-year survival rates were significantly lower in diabetic patients with inadequate maintenance of blood glucose:92% vs 99%,85% vs 96%,and 70% vs 92%,respectively(P = 0.0003).CONCLUSION:Inadequate maintenance of blood glucose in diabetic patients is a significant risk factor for recurrence of HCC and for poor survival after curative RFA therapy.Takanori Hosokawa Masayuki Kurosaki Kaoru Tsuchiya Shuya Matsuda Masaru Muraoka Yuichiro Suzuki Nobuharu Tamaki Yutaka Yasui Toru Nakata Takashi Nishimura Shoko Suzuki Ken Ueda Hiroyuki Nakanishi Jun Itakura Yuka Takahashi Namiki Izumi 2013World Journal of Gastroenterology2013,19,2:12
3上海和宜兴地区食品和人乳腺组织中有机氯农药污染现状显示文摘[目的 ]了解上海和宜兴地区环境和人体中有机氯农药的污染现状。 [方法 ]分别在上海和宜兴两城市采集2 0种食物样品及 5份人乳腺组织 ,采用GC ECD方法对样品中有机氯含量进行测定。 [结果 ]滴滴涕及其代谢物 (DDTs)是此次检测到的主要有机氯农药污染物 ,DDTs、六六六 (HCHs)、六氯苯及其代谢物 (HCB)、氯丹 (CHLs)在食品中的检出率分别为 85 %、64 %、5 9%和 5 3 % ,食品中HCHs ,HCB ,CHLs含量较低 ;而人乳腺组织中DDTs和HCHs的含量很高 ,可分别达 190 0 0ng/g脂肪和 170 0 0ng/g脂肪。 [结论 ]上海及宜兴地区食物和人乳腺组织中普遍还能检测到有机氯农药的残留 。李卫华 张蕴晖 范奇元 Nakata H Kitano T Abe S 丁训诚 2005环境与职业医学2005,22,1:10
4KL-6 mucin expression in carcinoma of the ampulla of Vater: Association with cancer progression显示文摘AIM: To assess histochemical expression of KL-6 and its clinicopathological significance in carcinoma of the ampulla of Vater.METHODS: Ampullary carcinoma tissues were collected from 38 patients who underwent pancreatoduodenectomy or local resection. Tissues were subjected to immunohistochemical analysis using KL-6 antibody.RESULTS: Positive staining of ampullary carcinoma cells was observed in 26 (68.4%) cases. Staining was not found in the surrounding non-cancer regions of the ampullary tissues. Remarkable KL-6 expression was observed in invasive carcinoma cells in pancreatic and duodenal tissues and in metastatic carcinoma cells in lymph nodes. Positive KL-6 expression was related to lymph node metastasis (P = 0.020), pancreatic invasion (P = 0.016), duodenal invasion (P = 0.034), and advanced stage of TNM clinical classification (P = 0.010). 5Direct joining of oxygen-free copper and carbon-fiber-reinforced plastic by friction lap joining显示文摘Oxygen-free copper(Cu) was successfully joined to carbon-fiber-reinforced thermoplastic(CFRTP,polyamide 6 with 20 wt% carbon fiber addition) by friction lap joining(FLJ) at joining speeds of 200–1600 mm/min with a constant rotation rate of 1500 rpm and a nominal plunge depth of 0.9 mm.It is the first time to report the joining of CFRTP to Cu by FLJ. As the joining speed increased, the tensile shear force(TSF) of joints increased first, and decreased thereafter. The maximum TSF could reach 2.3 kN(15 mm in width). Hydrogen bonding formed between the amide group of CFRTP and the thin Cu_2O layer on the Cu surface, which mainly contributed to the joint bonding. The influence factors of the TSF of the joints at different joining speeds were discussed. The TSF was mainly affected by the joining area, the degradation of the plastic matrix and the number and the size of bubbles. As the joining speed increased,the influence factors varied as follows: the joining area increased first and then decreased; the degradation of the plastic matrix and the number and the size of bubbles decreased. 6Co-effect of Demand-control-support Model and Effort-reward Imbalance Model on Depression Risk Estimation in Humans: Findings from Henan Province of China显示文摘Objective To investigate the co-effect of Demand-control-support(DCS) model and Effort-reward Imbalance(ERI) model on the risk estimation of depression in humans in comparison with the effects when they are used respectively. Methods A total of 3 632 males and 1 706 females from 13 factories and companies in Henan province were recruited in this cross-sectional study. Perceived job stress was evaluated with the Job Content Questionnaire and Effort-Reward Imbalance Questionnaire(Chinese version). Depressive symptoms were assessed by using the Center for Epidemiological Studies Depression Scale(CES-D). Results DC(demands/job control ratio) and ERI were shown to be independently associated with depressive symptoms. The outcome of low social support and overcommitment were similar. High DC and low social support(SS), high ERI and high overcommitment, and high DC and high ERI posed greater risks of depressive symptoms than each of them did alone.ERI model and SS model seem to be effective in estimating the risk of depressive symptoms if they are used respectively. Conclusion The DC had better performance when it was used in combination with low SS. The effect on physical demands was better than on psychological demands. 9Endoscopic management for congenital esophageal stenosis: A systematic review显示文摘Congenital esophageal stenosis(CES) is an extremely rare malformation, and standard treatment have not been completely established. By years of clinical research, evidence has been accumulated. We conducted systematic review to assess outcomes of the treatment for CES, especially the role of endoscopic modalities. A total of 144 literatures were screened and reviewed. CES was categorized in fibromuscularthickening, tracheobronchial remnants(TBR) and membranous web, and the frequency was 54%, 30% and 16%, respectively. Therapeutic option includes surgery and dilatation, and surgery tends to be reserved for ineffective dilatation. An essential point is that dilatation for TBR type of CES has low success rate and high rate of perforation. TBR can be distinguished by using endoscopic ultrasonography(EUS). Overall success rate of dilatation for CES with or without case selection by using EUS was 90% and 29%, respectively. Overall rate of perforation with or without case selection was 7% and 24%, respectively. By case selection using EUS, high success rate with low rate of perforation could be achieved. In conclusion, endoscopic dilatation has been established as a primary therapy for CES except TBR type. 10Influence of Changes in Solar Radiation on Changes of Surface Temperature in China显示文摘The long-term trends of total surface solar radiation(SSR),surface diffuse radiation,and surface air temperature were analyzed in this study based on updated 48-yr data from 55 observational stations in China,and then the correlation between SSR and the diurnal temperature range(DTR) was studied.The effect of total solar radiation on surface air temperature in China was investigated on the basis of the above analyses.A strong correlation between SSR and DTR was found for the period 1961-2008 in China.The highest correlation and steepest regression line slope occurred in winter,indicating that the solar radiation effect on DTR was the largest in this season.Clouds and water vapor have strong influences on both SSR and DTR,and hence on their relationship.The largest correlations between SSR and DTR occurred in wintertime in northern China,regardless of all-day(including clear days and cloudy days) or clear-day cases.Our results also showed that radiation arriving at the surface in China decreased significantly during 1961-1989(dimming period),but began to increase during 1990-2008(brightening period),in agreement with previous global studies.The reduction of total SSR offset partially the greenhouse warming during 1961-1989.However,with the increase of SSR after 1990,this offsetting effect vanished;on the contrary,it even made a contribution to the accelerated warming.Nonetheless,the greenhouse warming still played a controlling role because of the increasing of minimum and mean surface temperatures in the whole study period of 1961-2008.We estimated that the greenhouse gases alone may have caused surface temperatures to rise by 0.31-0.46℃(10 yr) 1 during 1961-2008,which is higher than previously estimated.Analysis of the corresponding changes in total solar radiation,diffuse radiation,and total cloud cover indicated that the dimming and brightening phenomena in China were likely attributable to increases in absorptive and scattering aerosols in the atmosphere,respectively.张华 尹青 Teruyuki NAKAJIMA Nakata Mukai MAKIKO 卢鹏 何金海 2013Acta meteorologica Sinica2013,27,1:6

12Three benefits of microcatheters for retrograde transvenous obliteration of gastric varices显示文摘AIM:To evaluate the usefulness of the microcatheter techniques in balloon-occluded retrograde transvenous obliteration (BRTO) of gastric varices.METHODS:Fifty-six patients with gastric varices underwent BRTOs using microcatheters.A balloon catheter was inserted into gastrorenal or gastrocaval shunts.A microcatheter was navigated close to the varices,and sclerosant was injected into the varices through the microcatheter during balloon occlusion.The next morning,thrombosis of the varices was evaluated by contrast enhanced computed tomography (CE-CT).In patients with incomplete thrombosis of the varices,a second BRTO was performed the following day.Patients were followed up with CE-CT and endoscopy.RESULTS:In all 56 patients,sclerosant was selectively injected through the microcatheter close to the varices.In 9 patients,microcoil embolization of collateral veinswas performed using a microcatheter.In 12 patients with incomplete thrombosis of the varices,additional injection of sclerosant was performed through the microcatheter that remained inserted overnight.Complete thrombosis of the varices was achieved in 51 of 56 patients,and the remaining 5 patients showed incomplete thrombosis of the varices.No recurrence of the varices was found in the successful 51 patients after a median follow up time of 10.5 mo.We experienced one case of liver necrosis,and the other complications were transient.CONCLUSION:The microcatheter techniques are very effective methods for achieving a higher success rate of BRTO procedures.Tetsuo Sonomura Wataru Ono Morio Sato Shinya Sahara Kouhei Nakata Hiroki Sanda Nobuyuki Kawai Hiroki Minamiguchi Motoki Nakai Kazushi Kishi 2012World Journal of Gastroenterology2012,18,12:5

14Unsteady bio-fluid dynamics in flying and swimming显示文摘Flying and swimming in nature present sophisticated and exciting ventures in biomimetics, which seeks sustainable solutions and solves practical problems by emulating nature's time-tested patterns, functions, and strategies. Bio-fluids in insect and bird flight, as well as in fish swimming are highly dynamic and unsteady; however, they have been studied mostly with a focus on the phenomena associated with a body or wings moving in a steady flow. Characterized by unsteady wing flapping and body undulation, fluid-structure interactions, flexible wings and bodies, turbulent environments, and complex maneuver, bio-fluid dynamics normally have challenges associated with low Reynolds number regime and high unsteadiness in modeling and analysis of flow physics. In this article, we review and highlight recent advances in unsteady bio-fluid dynamics in terms of leading-edge vortices, passive mechanisms in flexible wings and hinges, flapping flight in unsteady environments, and micro-structured aerodynamics in flapping flight, as well as undulatory swimming, flapping-fin hydrodynamics, body–fin interac-tion, C-start and maneuvering, swimming in turbulence,collective swimming, and micro-structured hydrodynamics in swimming. 16Effects of wing deformation on aerodynamic performance of a revolving insect wing显示文摘Flexible wings of insects and bio-inspired micro air vehicles generally deform remarkably during flapping flight owing to aerodynamic and inertial forces,which is of highly nonlinear fluid-structure interaction(FSI)problems.To elucidate the novel mechanisms associated with flexible wing aerodynamics in the low Reynolds number regime,we have built up a FSI model of a hawkmoth wing undergoing revolving and made an investigation on the effects of flexible wing deformation on aerodynamic performance of the revolving wing model.To take into account the characteristics of flapping wing kinematics we designed a kinematic model for the revolving wing in two-fold:acceleration and steady rotation,which are based on hovering wing kinematics of hawkmoth,Manduca sexta.Our results show that both aerodynamic and inertial forces demonstrate a pronounced increase during acceleration phase,which results in a significant wing deformation.While the aerodynamic force turns to reduce after the wing acceleration terminates due to the burst and detachment of leading-edge vortices(LEVs),the dynamic wing deformation seem to delay the burst of LEVs and hence to augment the aerodynamic force during and even after the acceleration.During the phase of steady rotation,the flexible wing model generates more ver-tical force at higher angles of attack(40°–60°)but less horizontal force than those of a rigid wing model.This is because the wing twist in spanwise owing to aerodynamic forces results in a reduction in the effective angle of attack at wing tip,which leads to enhancing the aerodynamics performance by increasing the vertical force while reducing the horizontal force.Moreover,our results point out the importance of the fluid-structure interaction in evaluating flexible wing aerodynamics:the wing deformation does play a significant role in enhancing the aerodynamic performances but works differently during acceleration and steady rotation,which is mainly induced by inertial force in acceleration but by aerodynamic forces in steady rotation.Ryusuke Noda Toshiyuki Nakata Hao Liu 2014Acta Mechanica Sinica2014,30,6:5
17Cap polyposis refractory to Helicobacter pylori eradication treated with endoscopic submucosal dissection显示文摘Cap polyposis is a rare intestinal disorder. Characteristic endoscopic findings are multiple inflammatory polypoid lesions covered by caps of fibrous purulent exudate. Although a specific treatment has not been established, some studies have suggested that eradication therapy for Helicobacter pylori(H. pylori) is effective. We report a case of a 20-year-old man with cap polyposis presenting with hematochezia. Colonoscopy showed the erythematous polyps with white caps from the sigmoid colon to rectum. Histopathological findings revealed elongated, tortuous, branched crypts lined by hyperplastic epithelium with a mild degree of fibromusculosis in the lamina propria. Although H. pylori eradication was instituted, there was no improvement over six months. We then performed en bloc excision of the polyps by endoscopic submucosal dissection(ESD), which resulted in complete resolution of symptoms. ESD may be a treatment option for cap polyposis refractory to conservative treatments. 18Effects of anti-hypertensive drugs on esophageal body contraction显示文摘AIM:To clarify the effects of anti-hypertensive drugs on esophageal contraction and determine their possi-ble relationship with gastro-esophageal reflux disease.METHODS:Thirteen healthy male volunteers were enrolled. Esophageal body peristaltic contractions and lower esophageal sphincter (LES) pressure were measured using high resolution manometry. All subjects were randomly examined on four separate occasions following administrations of nifedipine,losartan,and atenolol,as well as without any drug administration.RESULTS:Peristaltic contractions by the esophageal body were separated into three segments by two troughs. The peak peristaltic pressures in the mid and lower segments of the esophageal body under atenolol administration were signifi cantly higher than those without medication in a supine position. On the other hand,peristaltic pressures under nifedipine administration were lower than those observed without drug ad-ministration. Losartan did not change esophageal body peristalsis. 19Usefulness of the clip-flap method of endoscopic submucosal dissection: A randomized controlled trial显示文摘AIM To prospectively investigate the efficacy and safety of clipflap assisted endoscopic submucosal dissection(ESD) for gastric tumors.METHODS From May 2015 to October 2016, we enrolled 104 patients with gastric cancer or adenoma scheduled for ESD at Shiga University of Medical Science Hospital. We randomized patients into two subgroups using the minimization method based on location of the tumor(upper, middle or lower third of the stomach), tumor size(< 20 mm or > 20 mm) and ulcer status: ESD using an endoclip(the clip-flap group) and ESD without an endoclip(the conventional group). Therapeutic efficacy(procedure time) and safety(complication: Gastrointestinal bleeding and perforation) were assessed. RESULTS En bloc resection was performed in all patients. Four patients had delayed bleeding(3.8%) and two had perforation(1.9%). No significant differences in en bloc resection rate(conventional group: 100%, clip flap group: 100%), curative endoscopic resection rate(conventional group: 90.9%, clip flap group: 89.8%, P = 0.85), procedure time(conventional group: 70.8 ± 46.2 min, clip flap group: 74.7 ± 53.3 min, P = 0.69), area of resected specimen(conventional group: 884.6 ± 792.1 mm^2, clip flap group: 1006.4 ± 1004.8 mm^2, P = 0.49), delayed bleeding rate(conventional group: 5.5%, clip flap group: 2.0%, P = 0.49), or perforation rate(conventional group: 1.8%, clip flap group: 2.0%, P = 0.93) were found between the two groups. 20Surgery for gastric cancer patients of age 85 and older: Multicenter survey显示文摘AIM To investigate the surgical therapies for gastric cancer(GC) patients of age 85 or older in a multicenter survey.METHODS Therapeutic opportunities for elderly GC patients have expanded in conjunction with extended life expectancy. However, the number of cases encountered in a single institution is usually very small and surgical therapies for elderly GC patients have not yet been standardized completely. In the present study, a total of 134 GC patients of age 85 or older who underwent surgery in 9 related facilities were retrospectively investigated. The relationships between surgical therapies and clinicopathological or prognostic features were analyzed.RESULTS Eighty-nine of the patients(66%) presented with a comorbidity, and 26(19% overall) presented with more than two comorbidities. Radical lymphadenectomy was performed in 59 patients(44%), and no patient received pre- or post-operative chemotherapy. Forty of the patients(30%) experienced perioperative complications, but no surgical or perioperative mortality occurred. Laparoscopic surgery was performed in only 12 of the patients(9.0%). Univariate and multivariate analyses of the 113 patients who underwent R0 or R1 resection identified the factors of p T3/4 and limited lymphadenectomy as predictive of worse prognosis(HR = 4.68, P = 0.02 and HR =2.19, P = 0.05, respectively). Non-cancer-specific death was more common in c Stage Ⅰ patients than in c Stage Ⅱ or Ⅲ patients. Limited lymphadenectomy correlated with worse cancer-specific survival(P = 0.01), particularly in c Stage Ⅱ patients(P < 0.01). There were no relationships between limited lymphadenectomy and any comorbidities, except for cerebrovascular disease(P = 0.07).
