| 2 | Increased liver stiffness in alcoholic liver disease:Differentiating fibrosis from steatohepatitis显示文摘AIM:To test if inflammation also interferes with liver stiffness (LS) assessment in alcoholic liver disease (ALD) and to provide a clinical algorithm for reliable fibrosis assessment in ALD by FibroScan (FS).METHODS:We first performed sequential LS analysis before and after normalization of serum transaminases in a learning cohort of 50 patients with ALD admitted for alcohol detoxification. LS decreased in almost all patients within a mean observation interval of 5.3 d. Six patients (12%) would have been misdiagnosed with F3and F4 fibrosis but LS decreased below critical cut-off values of 8 and 12.5 kPa after normalization of trans-aminases. RESULTS:Of the serum transaminases,the decrease in LS correlated best with the decrease in glutamic oxaloacetic transaminase (GOT). No significant chang-es in LS were observed below GOT levels of 100 U/L. After establishing the association between LS and GOT levels,we applied the rule of GOT < 100 U/L for reliable LS assessment in a second validation cohort of 101 patients with histologically confi rmed ALD. By ex-cluding those patients with GOT > 100 U/L at the time of LS assessment from this cohort,the area under the receiver operating characteristic (AUROC) for cirrhosis detection by FS improved from 0.921 to 0.945 while specificity increased from 80% to 90% at a sensitivity of 96%. A similar AUROC could be obtained for lower F3 fibrosis stage if LS measurements were restricted to patients with GOT < 50 U/L. Histological grading of inflammation did not further improve the diagnostic accuracy of LS.CONCLUSION:Coexisting steatohepatitis markedly increases LS in patients with ALD independent of fibrosis stage. | 3 | Computed tomography and patient risk:Facts,perceptions and uncertainties显示文摘Since its introduction in the 1970s, computed tomography(CT) has revolutionized diagnostic decision-making. One of the major concerns associated with the widespread use of CT is the associated increased radiation exposure incurred by patients. The link between ionizing radiation and the subsequent development of neoplasia has been largely based on extrapolating data from studies of survivors of the atomic bombs dropped in Japan in 1945 and on assessments of the increased relative risk of neoplasia in those occupationally exposed to radiation within the nuclear industry. However, the association between exposure to low-dose radiation from diagnostic imaging examinations and oncogenesis remains unclear. With improved technology, significant advances have already been achieved with regards to radiation dose reduction. There are several dose optimization strategies available that may be readily employed including omitting unnecessary images at the ends of acquired series, minimizing the number of phases acquired, and the use of automated exposure control as opposed to fixed tube current techniques. In addition, new image reconstruction techniques that reduce radiation dose have been developed in recent years with promising results. | 4 | Anemia and long-term outcome in adjuvant and neoadjuvant radiochemotherapy of stage Ⅱ and Ⅲ rectal adenocarcinoma:The Freiburg experience(1989-2002)显示文摘AIM:To evaluate the long-term outcome of standard5-FU based adjuvant or neoadjuvant radiochemotherapyand to identify the predictive factors,especially anemiabefore and after radiotherapy as well as hemoglobinincrease or decrease during radiotherapy.METHODS:Two hundred and eighty-six patientswith Union International Contre Cancer(UICC)stageⅡ and Ⅲ rectal adenocarcinomas,who underwentresection by conventional surgical techniques(lowanterior or abdominoperineal resection),receivedeither postoperative(n=233)or preoperative(n=53)radiochemotherapy from January 1989 until July 2002.Overall survival(OAS),cancer-specific survival(CSS),disease-free survival(DFS),local-relapse-free(LRS)and distant-relapse-free survival(DRS)were evaluatedusing Kaplan-Meier,Log-rank test and Cox's proportionalhazards as statistical methods.Multivariate analysis wasused to identify prognostic factors.Median follow-uptime was 8 years.RESULTS:Anemia before radiochemotherapy was anindependent prognostic factor for improved DFS(riskratio 0.76,P=0.04)as well as stage,grading,R status(free radial margins),type of surgery,carcinoembryonicantigen(CEA)levels,and gender.The univariate analysisrevealed that anemia was associated with impaired LRS (better local control)but with improved DFS.In contrast,hemoglobin decrease during radiotherapy was anindependent risk factor for DFS(risk ratio 1.97,P=0.04).During radiotherapy,only 30.8% of RO-resected patientssuffered from hemoglobin decrease compared to 55.6%if R1/2 resection was performed(P=0.04).The 5-yearOAS,CSS,DFS,LRS and DRS were 47.0%,60.0%,41.4%,67.2%,and 84.3%,respectively.Significantdifferences between preoperative and postoperativeradiochemotherapy were not found.CONCLUSION:Anemia before radiochemotherapyand hemoglobin decrease during radiotherapy have nopredictive value for the outcome of rectal cancer.Stage,grading,R status(free radial margins),type of surgery,CEA levels,and gender have predictive value for theoutcome of rectal cancer. | Christian Weissenberger Geissler Michael Otto Florian Barke Annette Henne Karl von Plehn Georg Rein Alex Müller Christine Bartelt Susanne Henke Michael | 2006 | World Journal of Gastroenterology2006，12，12： | 4 |
