2结构性植骨修复膝关节翻修术中严重骨缺损的临床效果及相关分析(英文)显示文摘目的探讨同种异体结构性植骨在膝关节翻修术中大块骨缺损中应用的临床效果和意义。方法1994~2001年芬兰坦佩雷大学医院应用单一翻修假体及同种异体骨结构性植骨治疗膝关节置换术大块骨缺损患者10例(膝),男1例,女9例,平均年龄70岁(61—77岁),平均随访5年(1~8年),所有手术均由两名高年资专科医师执行,采取KSS评分评估术前术后膝关节功能。结果最后随访时,患者膝关节KSS评分由术前的平均39分(4~51分)提高至81分(28—102分;P〈0.05);疼痛评分由术前的18分(0—30分)提高至42分(10—50分;P〈0.05)。2例出现假体周围透亮线(〈1mm),但没有任何松动症状;所有结构性植骨均获得满意的愈合,最后随访时没有出现吸收征象,1例患者术后出现膝前疼痛,经髌骨表面置换后症状消失。结论同种异体骨结构性植骨应用在膝关节翻修术中大块骨缺损中可取得满意的临床效果,重建下肢力线、第三代骨水泥技术的应用及有由专科医师实施手术是获得良好临床效果的保证。盛璞义 Esa Jamsen Matti Lehto Yrj T Konttinen Jorma Pajamki Pekka Halonen 2009中华关节外科杂志(电子版)2009,3,4:3

4Increased prevalence of intestinal inflammation in patients with liver cirrhosis显示文摘AIM To investigate the pathophysiology of the digestive tract in patients with liver cirrhosis.METHODS In 42 cirrhotic patients and 20control subjects, the following fecal proteinswere measured by enzyme-linkedimmunosorb6nt assay: albumin (Alb ),transferrin (Tf), and al-antitrypsin (a,-AT) as amarker for intestinal protein l0ss, hemoglobin(Hb) for bleeding, PMN-eIastase for intestinalinflammation, and secretory lgA for intestinalimmunity.RESULTS The fecaI concentrations of Hb, Alb,Tf, al-AT, and PMN’eIastase were increased in13 (3l%), 8(19%), l0(24%), 6(14%), and 1l(26%) cases among 42 patients, resPectiveIy.F6cal concentration of secretory IgA wasdecreased in 7 (l7%) of 42 patients. However,these fecal concentrations were not related tothe severity or etiology of liver cirrhosis. 5全膝关节置换翻修术的Meta分析显示文摘目的通过对已报道的全膝关节置换翻修术文献进行总结分析,讨探膝关节置换翻修术前后的膝关节功能、翻修的主要原因、主要并发症及不同假体的术后疗效。方法按照以下标准收集和分析有关全膝关节置换翻修术的文献:①1990年至2002年间发表,②报告患者数大于10例,③采用通用的膝关节评分标准。一名骨科专科医生独立收集数据,一名医学统计学专家独立采用Meta统计方法分析数据。结果共有33篇符合条件的文献被收集。患者共1356例,其中男429例,女611例(部分文献性别分类数据缺失),平均年龄67岁(45~90岁),加权平均随访时间57个月(6~108个月),加权平均术前膝关节功能总评分为49分(15~84分),术后为84分(58~109分),全膝关节置换翻修术前后的总评分、功能评分、活动范围等有显著性提高,差异有统计学意义(总评分:t=12.507,P<0.01,功能评分:t=4.704,P<0.01,活动范围:t=5.346,P<0.01)。全膝关节置换翻修术的原因主要是假体松动(55%),其它包括聚乙烯磨损(11%)、假体不稳(10%)、感染(7%)。翻修术后的主要并发症仍然为假体松动(18%),其它包括假体不稳(16%)、感染(16%)、髌骨问题(15%)及不明原因的膝关节疼痛(13%)。髌骨问题包括髌骨脱位、半脱位、髌韧带撕裂、髌股关节疼痛等。结论可以认为膝关节置换后翻修术是一种安全有效的手术。假体松动是膝关节置换翻修的主要原因和并发症。盛璞义 Matti Lehto 廖威明 何爱珊 傅明 Matti Kataja 张紫机 Teemu Moilanen Jorma Pajamki Yrj T Konttinen 2007中华关节外科杂志(电子版)2007,1,1:2
