2018加拿大心境障碍与焦虑障碍治疗协作组/国际双相障碍学会指南:双相障碍的管理显示文摘加拿大心境障碍与焦虑障碍治疗协作组(Canadian Network for Mood and Anxiety Treatments,CANMAT)曾于2005年发布了第1版双相障碍管理指南,并分别于2007、2009和2013年对该指南进行了更新,其中最近的2次更新是与国际双相障碍学会(International Society for Bipolar Disorders,ISBD)合作完成。 | Lakshmi N Yatham Sidney H Kennedy Sagar V Parikh Ayal Sehaffer David J Bond Benicio N Frey Verinder Sharma Benjamin I Goldstein Soham Rej Serge Beaulieu Martin Alda Glenda MaeQueen Roumen V Milev Arun Ravindran Claire O'Donovan Diane Mclntosh Raymond W Lam Gustavo Vazquez Flavio Kapczinski Roger S Melntyre Jan Kozicky Shigenobu Kanba Beny Lafer Trisha Suppes Joseph R Calabrese Eduard Vieta Gin Malhi Robert M Post Michael Berk 胡晨(译) 王刚(译) | 2019 | 中华精神科杂志2019,52,1: | 25

Switchgrass Biochar Effects on Plant Biomass and Microbial Dynamics in Two Soils from Different Regions显示文摘Biochar amendments to soils may alter soil function and fertility in various ways, including through induced changes in the microbial community. We assessed microbial activity and community composition of two distinct clayey soil types, an Aridisol from Colorado(CO) in the U.S. Central Great Plains, and an Alfisol from Virginia(VA) in the southeastern USA following the application of switchgrass(Panicum virgatum) biochar. The switchgrass biochar was applied at four levels, 0%, 2.5%, 5%, and 10%, approximately equivalent to biochar additions of 0, 25, 50, and 100 t ha-1, respectively, to the soils grown with wheat(Triticum aestivum) in an eight-week growth chamber experiment. We measured wheat shoot biomass and nitrogen(N) content and soil nutrient availability and N mineralization rates, and characterized the microbial fatty acid methyl ester(FAME) profiles of the soils. Net N mineralization rates decreased in both soils in proportion to an increase in biochar levels, but the effect was more marked in the VA soil, where net N mineralization decreased from-2.1 to-38.4 mg kg-1. The 10% biochar addition increased soil p H, electrical conductivity, Mehlichand bicarbonate-extractable phosphorus(P), and extractable potassium(K) in both soil types. The wheat shoot biomass decreased from 17.7 to 9.1 g with incremental additions of biochar in the CO soil, but no difference was noted in plants grown in the VA soil. The FAME recovery assay indicated that the switchgrass biochar addition could introduce artifacts in analysis, so the results needed to be interpreted with caution. Non-corrected total FAME concentrations indicated a decline by 45% and 34% with 10% biochar addition in the CO and VA soils, respectively, though these differences became nonsignificant when the extraction efficiency correction factor was applied. A significant decline in the fungi:bacteria ratio was still evident upon correction in the CO soil with biochar. Switchgrass biochar had the potential to cause short-term negative impacts on plant biomass and alter soil microbial community structure unless measures were taken to add supplemental N and labile carbon(C). | Charlene N.KELLY Francisco C.CALDERóN Verónica ACOSTA-MARTíNEZ Maysoon M.MIKHA Joseph BENJAMIN David W.RUTHERFORD Colleen E.ROSTAD | 2015 | Pedosphere2015,25,3: | 11
Management of adults with coarctation of aorta显示文摘Coarctation of the aorta(CoA)is a relatively common congenital cardiac defect often causing few symptoms and therefore can be challenging to diagnose. The hallmark finding on physical examination is upper extremity hypertension, and for this reason,CoA should be considered in any young hypertensive patient, justifying measurement of lower extremity blood pressure at least once in these individuals. The presence of a significant pressure gradient between the arms and legs is highly suggestive of the diagnosis.Early diagnosis and treatment are important as long-term data consistently demonstrate that patients with CoA have a reduced life expectancy and increased risk of cardiovascular complications.Surgical repair has traditionally been the mainstay of therapy for correction, although advances in endovascular technology with covered stents or stent grafts permit nonsurgical approaches for the management of older children and adults with native CoA and complications.Persistent hypertension and vascular dysfunction can lead to an increased risk of coronary disease, which, remains the greatest cause of long-term mortality. Thus, blood pressure control and periodic reassessment with transthoracic echocardiography and threedimensional imaging(computed tomography or cardiac magnetic resonance)for should be performed regularly as cardiovascular complications may occur decades after the intervention. | Pradyumna Agasthi Sai Harika Pujari Andrew Tseng Joseph N Graziano Francois Marcotte David Majdalany Farouk Mookadam Donald J Hagler Reza Arsanjani | 2020 | World Journal of Cardiology2020,12,5: | 8
Machine learning in orthopaedic surgery显示文摘Artificial intelligence and machine learning in orthopaedic surgery has gained mass interest over the last decade or so.In prior studies, researchers have demonstrated that machine learning in orthopaedics can be used for different applications such as fracture detection, bone tumor diagnosis, detecting hip implant mechanical loosening, and grading osteoarthritis. As time goes on,the utility of artificial intelligence and machine learning algorithms, such as deep learning, continues to grow and expand in orthopaedic surgery. The purpose of this review is to provide an understanding of the concepts of machine learning and a background of current and future orthopaedic applications of machine learning in risk assessment, outcomes assessment, imaging, and basic science fields. In most cases, machine learning has proven to be just as effective,if not more effective, than prior methods such as logistic regression in assessment and prediction. With the help of deep learning algorithms, such as artificial neural networks and convolutional neural networks, artificial intelligence in orthopaedics has been able to improve diagnostic accuracy and speed, flag the most critical and urgent patients for immediate attention, reduce the amount of human error, reduce the strain on medical professionals, and improve care.Because machine learning has shown diagnostic and prognostic uses in orthopaedic surgery, physicians should continue to research these techniques and be trained to use these methods effectively in order to improve orthopaedic treatment. | Simon P Lalehzarian Anirudh K Gowd Joseph N Liu | 2021 | World Journal of Orthopedics2021,12,9: | 4
Metabolic syndrome does not affect sustained virologic response of direct-acting antivirals while hepatitis C clearance improves hemoglobin A1c显示文摘AIM To determine whether successful treatment with direc-tacting antivirals(DAA) is associated with improvements in hemoglobin A1 c(HbA1 c) and if type 2 diabetes mellitus(T2 DM) or metabolic syndrome affects sustained virologic response(SVR).METHODS We performed a retrospective analysis of all hepatitis C virus(HCV) patients at the VA Greater Los Angeles Healthcare System treated with varying DAA therapy between 2014-2016. Separate multivariable logistic regression was performed to determine predictors of HbA1 c decrease ≥ 0.5 after DAA treatment and predictors of SVR 12-wk post treatment(SVR12).RESULTS A total of 1068 patients were treated with DAA therapy between 2014-2016. The presence of T2 DM or metabolic syndrome did not adversely affect SVR12. 106 patients had both HCV and T2 DM. Within that cohort, patients who achieved SVR12 had lower mean HbA1 c pre treatment(7.35 vs 8.60,P = 0.02),and lower mean HbA1 c post-treatment compared to non-responders(6.55 vs 8.61,P = 0.01). The mean reduction in HbA1 c after treatment was greater for those who achieved SVR12 than for non-responders(0.79 vs 0.01,P = 0.03). In adjusted models, patients that achieved SVR12 were more likely to have a HbA1 c decrease of ≥ 0.5 than those that did not achieve SVR12(adjusted OR = 7.24,95%CI: 1.22-42.94). CONCLUSION In HCV patients with T2 DM,successful treatment with DAA was associated with a significant reduction in HbA1 c suggesting that DAA may have a role in improving insulin sensitivity. Furthermore, the presence of T2 DM or metabolic syndrome does not adversely affect SVR12 rates in patients treated with DAA. | Tien S Dong Elizabeth S Aby Jihane N Benhammou Jenna Kawamoto Steven-Huy Han Folasade P May Joseph R Pisegna | 2018 | World Journal of Hepatology2018,10,9: | 4

Effect of bone loss in anterior shoulder instability显示文摘Anterior shoulder instability with bone loss can be a difficult problem to treat. It usually involves a component of either glenoid deficiency or a Hill-Sachs lesion. Recent data shows that soft tissue procedures alone are typically not adequate to provide stability to the shoulder. As such, numerous surgical procedures have been described to directly address these bony deficits. For glenoid defects, coracoid transfer and iliac crest bone block procedures are popular and effective. For humeral head defects, both remplissage and osteochondral allografts have decreased the rates ofrecurrent instability. Our review provides an overview of current literature addressing these treatment options and others for addressing bone loss complicating anterior glenohumeral instability. | Grant H Garcia Joseph N Liu David M Dines Joshua S Dines | 2015 | World Journal of Orthopedics2015,6,5: | 3
Multimodality functional imaging using DW-MRI and 18F-FDG-PET/CT during radiation therapy for human papillomavirus negative head and neck squamous cell carcinoma: Meixoeiro Hospital of Vigo Experience显示文摘AIM To noninvasively investigate tumor cellularity measured using diffusion-weighted magnetic resonance imaging(DW-MRI) and glucose metabolism measured by 18 Flabeled fluorodeoxyglucose positron emission tomography/computed tomography(18F-FDG-PET/CT) during radiation therapy(RT) for human papillomavirus negative(HPV-) head and neck squamous cell carcinoma(HNSCC).METHODS In this prospective study, 6 HPV- HNSCC patients underwent a total of 34 multimodality imaging examinations(DW-MRI at 1.5 T Philips MRI scanner [(n = 24) pre-, during-(2-3 wk), and post-treatment(Tx), and 18F-FDG PET/CT pre- and post-Tx(n = 10)]. All patients received RT. Monoexponential modeling of the DW-MRI data yielded the imaging metric apparent diffusion coefficient(ADC) and the mean of standardized uptake value(SUV) was measured from 18F-FDG PET uptake. All patients had a clinical follow-up as the standard of care and survival status was documented at 1 year.RESULTS There was a strong negative correlation between the mean of pretreatment ADC(ρ =-0.67, P = 0.01) and the pretreatment 18F-FDG PET SUV. The percentage(%) change in delta(?) ADC for primary tumors and neck nodal metastases between pre- and Wk2-3 Tx were as follows: 75.4% and 61.6%, respectively, for the patient with no evidence of disease, 27.5% and 32.7%, respectively, for those patients who were alive with disease, and 26.9% and 7.31%, respectively, for those who were dead with disease.CONCLUSION These results are preliminary in nature and are indicative, and not definitive, trends rendered by the imaging metrics due to the small sample size of HPV- HNSCC patients in a Meixoeiro Hospital of Vigo Experience. | David Aramburu Núnez Antonio Lopez Medina Moisés Mera Iglesias Francisco Salvador Gomez Abhay Dave Vaios Hatzoglou Ramesh Paudyal Alfonso Calzado Joseph O Deasy Amita Shukla-Dave Victor M Munoz | 2017 | World Journal of Radiology2017,9,1: | 2
