2Laparoscopic Partial Nephrectomy With Segmental Renal Artery Clamping:Technique and Clinical Outcomes显示文摘Background:Warm ischemic injury is one of the most important factors affecting renal function in partial nephrectomy(PN).The technique of segmental renal artery clamping emerges as an alternative to conventional renal artery clamping for renal hilar control.Objective:To evaluate the feasibility and efficiency of laparoscopic PN(LPN) with segmental renal artery clamping in comparison with the conventional technique.Design,setting,and participants:A total of 75 patients underwent LPN from June 2007 to November 2009.All patients had T1a or T1b tumor in one kidney and a normal contralateral kidney.Thirty-seven patients underwent surgeries with main renal artery clamping,and 38 underwent surgeries with segmental artery clamping.Intervention:All procedures were performed by the same laparoscopic surgeon.Measurements:Blood loss,operation time,warm ischemia(WI) time,and complications affected renal function before and after operation were recorded.Results and limitations:All LPNs were completed without conversion to open surgery or nephrectomy.The novel technique slightly increased WI time(p < 0.001) and intraoperative blood loss(p = 0.006),while it provided better postoperative affected renal function(p < 0.001) compared with the conventional technique.The total complication rate was 12%.Among the 38 cases where segmental renal artery clamping was performed,7 had to convert to the conventional method.Tumor size and location influenced the number of clamped segmental arteries.Long-term postoperative renal function is still awaited.Conclusions:LPN with segmental artery clamping is safe and feasible in clinical practice.It minimizes the intraoperative WI injury and improves early postoperative affected renal function compared with main renal artery clamping.(C) 2010 European Association of Urology.Published by Elsevier B.V.All rights reserved.Shao,P.F. 4中国大陆肺炎发病率与死亡率:1985-2008年中英文文献的系统分析显示文摘目的肺炎是全球传染性疾病最主要的死因之一,但中国的肺炎疾病负担还知之甚少,因为很多数据资料都是以非英文文献的形式发表的。方法系统回顾了中国大陆关于肺炎发病率与死亡率的中文和英文文献,这些研究都是具有第一手数据的研究,1985-2008年有37篇论文符合纳入标准。结果各项研究的质量差异较大。5岁以下儿童肺炎发病率为(0.06~0.27)次/人年,肺炎死亡率为184/10万~1 223/10万人口。肺炎的发病率与死亡率随着时间稳定不变或者呈下降趋势,且农村高于城市。结论肺炎仍然是中国儿童的主要公共卫生问题之一。肺炎发病率与死亡率估计的变化差异大。继续降低发病率与死亡率需要可靠的监测数据和新的预防措施的实施,特别是在农村等高发地区。官旭华 Benjamin J Silk Wenkai Li Aaron T. 7Spatial-Temporal Pattern and Driving Forces of Land Use Changes in Xiamen显示文摘Using Landsat TM data of 1988, 1998 and 2001, the dynamic process of the spatial-temporal characteristics of land use changes during 13 years from 1988 to 2001 in the special economic zone of Xiamen, China was analyzed to improve understanding and to find the driving forces of land use change so that sustainable land utilization could be practiced. During the 13 years cropland decreased remarkably by nearly 11304.95 ha. The areas of rural-urban construction and water body increased by 10 152.24 ha and 848.94 ha, respectively. From 1988 to 2001, 52.5% of the lost cropland was converted into rural-urban industrial land. Rapid urbanization contributed to a great change in the rate of cropland land use during these years. Land-reclamation also contributed to a decrease in water body area as well as marine ecological and environmental destruction. In the study area 1) urbanization and industrialization, 2) infrastructure and agricultural intensification, 3) increased affluence of the farming community, and 4) policy factors have driven the land use changes. 9Microbial Community Characteristics in a Degraded Wetland of the Yellow River Delta显示文摘Five different sites with a soluble salt gradient of 3.0-17.7 g kg-1 dry soil from the coast to the inland were selected, and the microbial population size, activity and diversity in the rhizospheres of five common plant species and the adjacent bulk soils (non-rhizosphere) were compared in a degraded wetland of the Yellow River Delta, Shandong Province, China to study the effiects of soil environment (salinity, seasonality, depth, and rhizosphere) on microbial communities and the wetland's ecological function, thus providing basic data for the bioremediation of degraded wetlands. There was a significant negative linear relationship between the salinity and the total number of microorganisms, overall microbial activity, or culturable microbial diversity. Salinity adversely affected the microbial community, and higher salinity levels resulted in smaller and less active microbial communities. Seasonal changes were observed in microbial activity but did not occur in the size and diversity. The microbial size, activity and diversity decreased with increasing soil depth. The size, activity and diversity of culturable microorganisms increased in the rhizospheres. All rhizospheres had positive effiects on the microbial communities, and common seepweed had the highest rhizosphere effect. Three halophilic bacteria (Pseudomonas mendocina, Burkholderia glumae, and Acinetobacter johnsonii) were separated through BIOLOG identification, and common seepweed could be recommended for bioremediation of degraded wetlands in the Yellow River Delta.WANG Zhen-Yu XIN Yuan-Zheng GAO Dong-Mei LI Feng-Min J. 10喜马拉雅山佩枯岗日冰川地貌的年代学、平衡线高度和气候研究显示文摘喜马拉雅山珠穆朗玛峰-希夏邦马峰地区是青藏高原南部现代冰川集中发育区之一,古冰川遗迹亦十分丰富,是研究第四纪青藏高原冰川形成和演化的关键区,一直备受地貌和第四纪环境研究者的关注.应用原地宇宙核素10Be暴露年龄测试技术,对采自希夏邦马峰西北佩枯岗日拉曲谷地冰碛垄上的冰川漂砾进行年代学研究;结合冰川地貌分析方法,对古冰川平衡线高度(ELA)和气候特征进行探讨.结果表明:末次冰期时,拉曲谷地冰川扩张,发育了山麓冰川,拉曲Ⅰ冰川面积99.9 km2,平衡线海拔5 620 m;拉曲Ⅱ冰川面积97.6 km2,平衡线海拔5 600 m;与现代平衡线高度相比,拉曲Ⅰ和拉曲Ⅱ的平衡线分别下降380 m、400 m.拉曲在末次冰期存在多次冰进,拉曲Ⅰ的10Be暴露年龄在42.1~22.3 ka;拉曲Ⅱ的10Be暴露年龄在18.6~14.8 ka.根据地貌法恢复末次冰期拉曲Ⅰ平衡线高度的温度为-10.7℃,拉曲Ⅱ为-10.6℃,降水量分别为251~461 mm和261~480 mm,显示出当时雪线高度的温度状况和现代冰川雪线高度的温度状况接近,但是降水比现在少得多,冰川气候类型为亚大陆型,印度季风减弱带来的水气变少.刘耕年 Li Yingkui 陈艺鑫 张梅 李川川 孔屏 Harbor J. 11Wavelet denoising via sparse representation显示文摘Wavelet threshold denoising is a powerful method for suppressing noise in signals and images. However, this method often uses a coordinate-wise processing scheme, which ignores the structural properties in the wavelet coefficients. We propose a new wavelet denoising method using sparse representation which is a powerful mathematical tool recently developed. Instead of thresholding wavelet coefficients individually, we minimize the number of non-zero coefficients under certain conditions. The denoised signal is reconstructed by solving an optimization problem. It is shown that the solution to the optimization problem can be obtained uniquely and the estimates of the denoised wavelet coefficients are unbiased, i.e., the statistical means of the estimates are equal to the noise-free wavelet coefficients. It is also shown that at least a local optimal solution to the denoising problem can be found. 15Impact of Land Use and Soil Fertility on Distributions of Soil Aggregate Fractions and Some Nutrients显示文摘The size distribution of water-stable aggregates and the variability of organic C,N and P contents over aggregate size fractions were studied for orchard,upland,paddy,and grassland soils with high,medium,and low fertility levels.The results showed that > 5 mm aggregates in the cultivated upland and paddy soils were 44.0% and 32.0%,respectively,less than those in the un-tilled orchard soil.Organic C and soil N in different size aggregate fractions in orchard soil with high fertility were significantly higher than those of other land uses.However,the contents of soil P in different size aggregates were significantly greater in the paddy soil as compared to the other land uses.Soil organic C,N and P contents were higher in larger aggregates than those in smaller ones.The amount of water-stable aggregates was positively correlated to their contribution to soil organic C,N and P.For orchard and grassland soils,the > 5 mm aggregates made the greatest contribution to soil nutrients,while for upland soil,the 0.25-0.053 mm aggregates contributed the most to soil nutrients.Therefore,the land use with minimum disturbance was beneficial for the formation of a better soil structure.The dominant soil aggregates in different land use types determined the distribution of soil nutrients.Utilization efficiency of soil P could be improved by converting other land uses to the paddy soil.LIU Xiao-Li HE Yuan-Qiu H. 17Land Suitability Assessment and Land Use Change in Fujian Province,China显示文摘To present the current status of land quality and distribution and the trends in land use change, the physical suitability of cropland and forestland and the associated changes in Fujian Province were evaluated and analyzed using data obtained from geographical information systems (GIS) and remote sensing (RS).Of the total land area of Fujian Province,first class suitable cropland accounted for only 4.21%, whereas unsuitable cropland accounted for 84.78%. The percentage of first class suitable cropland in the southeastern region (5.32%) was much higher than that in the northwestern area (2.91%).Only 13.63% of the existing cropland area consisted of first class cropland and 70.08% was classified as unsuitable for cultivation.Of the total land area of Fujian Province,the first class forestland comprised 55.25% and the unsuitable forestland (including third class) comprised 21.2%.The percentage of unsuitable forest area in the existing forestland was only 5.5%.From 1996 to 2001, cropland and unused land decreased significantly, whereas forestland and land used for urban and transport increased rapidly.Therefore,the major tasks ahead will be the land development for full grain production potential,the better coordination of allocating land to different uses, and the regulation inappropriate activities that damage agricultural ecosystems.QUAN Bin ZHU He-Jian CHEN Song-Lin M. 18A Review of the Functional and Anatomical Default Mode Network in Schizophrenia显示文摘Schizophrenia is a severe mental disorder characterized by impaired perception,delusions,thought disorder,abnormal emotion regulation,altered motor function,and impaired drive. The default mode network(DMN),since it was first proposed in 2001,has become a central research theme in neuropsychiatric disorders,including schizophrenia. In this review,first we define the DMN and describe its functional activity,functional and anatomical connectivity,heritability,and inverse correlation with the task positive network. Second,we review empirical studies of the anatomical and functional DMN,and anti-correlation between DMN and the task positive network in schizophrenia. 20Tooth regeneration: a revolution in stomatology and evolution in regenerative medicine显示文摘A tooth is a complex biological organ and consists of multiple tissues including the enamel, dentin, cementum and pulp. Tooth loss is the most common organ failure. Can a tooth be regenerated? Can adult stem cells be orchestrated to regenerate tooth structures such as the enamel, dentin, cementum and dental pulp, or even an entire tooth? If not, what are the therapeutically viable sources of stem cells for tooth regeneration? Do stem cells necessarily need to be taken out of the body, and manipulated ex vivo before they are transplanted for tooth regeneration? How can regenerated teeth be economically competitive with dental implants? Would it be possible to make regenerated teeth affordable by a large segment of the population worldwide? This review article explores existing and visionary approaches that address some of the above-mentioned questions. Tooth regeneration represents a revolution in stomatology as a shift in the paradigm from repair to regeneration: repair is by metal or artificial materials whereas regeneration is by biological restoration.
