| 3 | Promotion of spinosad biosynthesis by chromosomal integration of the Vitreoscilla hemoglobin gene in Saccharopolyspora spinosa显示文摘To promote spinosad biosynthesis by improving the limited oxygen supply during high-density fermentation of Saccharopolyspora spinosa, the open reading frame of the Vitreoscilla hemoglobin gene was placed under the control of the promoter for the erythromycin resistance gene by splicing using overlapping extension PCR. This was cloned into the integrating vector pSET152, yielding the Vitreoscilla hemoglobin gene expression plasmid pSET152EVHB. This was then introduced into S. spinosa SP06081 by conjugal transfer, and integrated into the chromosome by site-specific recombination at the integration site ΦC31 on pSET152EVHB. The resultant conjugant, S. spinosa S078-1101, was genetically stable. The integration was further confirmed by PCR and Southern blotting analysis. A carbon monoxide differential spectrum assay showed that active Vitreoscilla hemoglobin was successfully expressed in S. spinosa S078-1101. Fermentation results revealed that expression of the Vitreoscilla hemoglobin gene significantly promoted spinosad biosynthesis under normal oxygen and moderately oxygen-limiting conditions (P<0.01). | 5 | Implications of preoperative hypoalbuminemia in colorectal surgery显示文摘Serum albumin has traditionally been used as a quantitative measure of a patient's nutritional status because of its availability and low cost. While malnutrition has a clear definition within both the American and European Societies for Parenteral and Enteral Nutrition clinical guidelines, individual surgeons often determine nutritional status anecdotally. Preoperative albumin level has been shown to be the best predictor of mortality after colorectal cancer surgery. Specifically in colorectal surgical patients, hypoalbuminemia significantly increases the length of hospital stay, rates of surgical site infections, enterocutaneous fistula risk, and deep vein thrombosis formation. The delay of surgical procedures to allow for preoperative correction of albumin levels in hypoalbuminemic patients has been shown to improve the morbidity and mortality in patients with severe nutritional risk. The importance of preoperative albumin levels and the patient's chronic inflammatory state on the postoperative morbidity and mortality has led to the development of a variety of surgical scoring systems to predict outcomes efficiently. | 7 | Use of three-dimensional printing in preoperative planning in orthopaedic trauma surgery: A systematic review and meta-analysis显示文摘BACKGROUND With the increasing complexity of surgical interventions performed in orthopaedic trauma surgery and the improving technologies used in threedimensional(3D)printing,there has been an increased interest in the concept.It has been shown that 3D models allow surgeons to better visualise anatomy,aid in planning and performing complex surgery.It is however not clear how best to utilise the technique and whether this results in better outcomes.AIM To evaluate the effect of 3D printing used in pre-operative planning in orthopaedic trauma surgery on clinical outcomes.METHODS We performed a comprehensive systematic review of the literature and a metaanalysis.Medline,Ovid and Embase were searched from inception to February 8,2018.Randomised controlled trials,case-control studies,cohort studies and case series of five patients or more were included across any area of orthopaedic trauma.The primary outcomes were operation time,intra-operative blood loss and fluoroscopy used.RESULTS Seventeen studies(922 patients)met our inclusion criteria and were reviewed.The use of 3D printing across all specialties in orthopaedic trauma surgery demonstrated an overall reduction in operation time of 19.85%[95%confidence intervals(CI):(-22.99,-16.71)],intra-operative blood loss of 25.73%[95%CI:(-31.07,-20.40)],and number of times fluoroscopy was used by 23.80%[95%CI:(-38.49,-9.10)].CONCLUSION Our results suggest that the use of 3D printing in pre-operative planning in orthopaedic trauma reduces operative time,intraoperative blood loss and the number of times fluoroscopy is used. | Catrin Morgan Chetan Khatri Sammy A Hanna Hutan Ashrafian Khaled M Sarraf | 2020 | World Journal of Orthopedics2020,11,1: | 7 |

| 9 | Modulators affecting the immune dialogue between human immune and colon cancer cells显示文摘The link between chronic inflammation and colorectal cancer has been well established. The events proceeding along tumorigenesis are complicated and involve cells activated at the cancer microenvironment, tumor infiltrating polymorphonuclears, immune cells including lymphocyte subtypes and peripheral blood mononuclear cells(PBMC), as well as tumor-associated macrophages. The immune cells generate inflammatory cytokines, several of them playing a crucial role in tumorigenesis. Additional factors, such as gene expression regulated by cytokines, assembling of tumor growth- and transforming factors, accelerated angiogenesis, delayed apoptosis, contribute all to initiation, development and migration of tumor cells. Oxygen radical species originating from the inflammatory area promote cell mutation and cancer proliferation. Tumor cells may over-express pro-inflammatory mediators that in turn activate immune cells for inflammatory cytokines production. Consequently, an immune dialogue emerges between immune and cancer cells orchestrated through a number of activated molecular pathways. Cytokines, encompassing migration inhibitory factor, transforming growth factor beta 1, tumor necrosisfactor-α, Interleukin(IL)-6, IL-10, IL-12, IL-17, IL-23 have been reported to be involved in human cancer development. Some cytokines, namely IL-5, IL-6, IL-10, IL-22 and growth factors promote tumor development and metastasis, and inhibit apoptosis via activation of signal transducer activator transcription-3 transcription factor. Colon cancer environment comprises mesenchymal, endothelial and immune cells. Assessment of the interaction between components in the tumor environment and malignant cells requires a reconsideration of a few topics elucidating the role of chronic inflammation in carcinogenesis, the function of the immune cells expressed by inflammatory cytokine production, the immunomodulation of cancer cells and the existence of a cross-talk between immune and malignant cells leading to a balance in cytokine production. It is conceivable that the prevalence of anti-inflammatory cytokine production by PBMC in the affected colonic mucosa will contribute to the delay, or even to halt down malignant expansion. Targeting the interplay between immune and cancer cells by mediators capable to alter cytokine secretion toward increased anti-inflammatory cytokine release by PBMC and tumor associated macrophages, may serve as an additional strategy for treatment of malignant diseases. | 11 | Myofibroblastic cell activation and neovascularization predict native liver survival and development of esophageal varices in biliary atresia显示文摘AIM:To study the relation between collagen 1,α-smooth muscle actin(α-SMA)and CD34 expression and the most essential portoenterostomy(PE)outcomes.METHODS:Liver specimens were obtained at PE from33 biliary atresia(BA)patients for immunohistochemical analysis of collagen 1,α-SMA and CD34.Liver biopsies from 35 organ donors were used as controls.Expression patterns were related to clinical data including age at PE,serum total and conjugated bilirubin concentration at the time of PE and during follow-up,incidence of esophageal varices in follow-up upper gastrointestinal endoscopies,and native liver survival as well as to detailed histopathological findings.RESULTS:Collagen 1(16.4%vs 4.5%,P<0.0001),α-SMA(17.9%vs 4.6%,P<0.0001)and CD34(4.9%vs 3.8%,P=0.017)were markedly overexpressed in BA patients compared with controls.Patients who underwent liver transplantation by age of two years had significantly higher expression of collagen 1(18.6%vs 13.7%,P=0.024),α-SMA(20.4%vs 15.4%,P=0.009)and CD34(5.9%vs 4.0%,P=0.029)at PE compared with native liver survivors.CD34-positive microvessels were identified in the centrizonal region close to central vein in every BA patient.In majority of BA cases(56%)neovascularization was frequent as CD34-positive microvessels were observed in over half of the hepatic lobules.In controls,the CD34-positive microvessels were rare as they were completely absent in 40%and were found in less than 5%of the hepatic lobules in the rest.The difference between BA patients and controls was significant(P<0.0001).Patients who developed esophageal varices by two years had significantly higher expression of CD34 at PE compared with patients without varices(5.6%vs 4.0%,P=0.019).Expression ofα-SMA(r=0.758,P<0.0001)and collagen 1(r=0.474,P=0.016),and the amount of CD34-positive microvessels(r=0.356,P=0.047)were related to patient age at PE.CONCLUSION:Hepatic myofibroblastic cell activation,fibrogenesis and neovascularization are enhanced in BA,progress with increasing PE age and relate to native liver survival and development of esophageal varices. | Janne S Suominen Hanna Lampela Pivi Heikkil Jouko Lohi Hannu Jalanko Mikko P Pakarinen | 2014 | World Journal of Gastroenterology2014,20,12: | 6 |

| 16 | Sodium alendronate loaded poly(L-lactideco-glycolide)microparticles immobilized on ceramic scaffolds for local treatment of bone defects显示文摘Bone tissue regeneration in critical-size defects is possible after implantation of a 3D scaffold and can be additionally enhanced once the scaffold is enriched with drugs or other factors supporting bone remodelling and healing.Sodium alendronate(Aln),a widely used anti-osteoporosis drug,exhibits strong inhibitory effect on bone resorption performed by osteoclasts.Thus,we propose a new approach for the treatment of bone defects in craniofacial region combining biocompatible titanium dioxide scaffolds and poly(L-lactide-co-glycolide)microparticles(MPs)loaded with Aln.The MPs were effectively attached to the surface of the scaffolds’pore walls by human recombinant collagen.Drug release from the scaffolds was characterized by initial burst(2466% of the drug released within first 24 h)followed by a sustained release phase(on average 5 mg of Aln released per day from Day 3 to Day 18).In vitro tests evidenced that Aln at concentrations of 5 and 2.5 mg/ml was not cytotoxic for MG-63 osteoblast-like cells(viability between 8166% and 9863% of control),but it prevented RANKL-induced formation of osteoclast-like cells from macrophages derived from peripheral blood mononuclear cells,as shown by reduced fusion capability and decreased tartrateresistant acid phosphatase 5b activity(5665% reduction in comparison to control after 8 days of culture).Results show that it is feasible to design the scaffolds providing required doses of Aln inhibiting osteoclastogenesis,reducing osteoclast activity,but not affecting osteoblast functions,which may be beneficial in the treatment of critical-size bone tissue defects. | Lucja Rumian Cornelia Wolf-Brandstetter Sina Roβler Katarzyna Reczynska Hanna Tiainen Havard J.Haugen Dieter Scharnweber Elzbieta Pamula | 2020 | Regenerative Biomaterials2020,7,3: | 5 |
