| 1 | Generalized megaviscera of lupus:Refractory intestinal pseudo-obstruction,ureterohydronephrosis and megacholedochus显示文摘Dilated dysfunction involving multiple visceral organs has been reported in patients with systemic lupus erythematosus(SLE).Chronic intestinal pseudo-obstruction(CIPO) resulting from intestinal smooth muscle damage has presented in conjunction with ureterohydronephrosis and, more rarely, biliary dilatation(megacholedochus).While the molecular pathogenesis is largely unknown, observed histo-pathologic features include widespread myositis, myocyte necrosis in the intestinal muscularis propria with subsequent atrophy and fibrosis, preserved myenteric innervations and little vasculitis.High dose immunosuppression usually results in resolution of symptoms with recovery of smooth muscle function, indicative of an autoimmune etiology.We report a patient with SLE who presented with intestinal pseudo-obstruction, ureterohydronephrosis and megacholedochus, and present images that illustrate megaviscera simultaneously involving all 3 visceral organs.Since the co-manifestation of all 3 is unusual and has been reported only once previously, wehave termed this rare clinical syndrome generalized megaviscera of lupus(GML).Although the SLE disease-activity parameters responded to aggressive immunomodulative therapy in our patient, clinical evidence of peristaltic dysfunction persisted in all involved viscera.This is a variation from the favorable outcomes reported previously in SLE patients with GML and we attribute this poor clinical outcome to disease severity and, most importantly, delayed clinical presentation.Since inflammation followed by atrophy and fibrosis are key aspects in the pathogenesis and natural history of GML, the poor response in our patient who presented late in the clinical course may be the result of ‘burnt out' inflammation with irreversible end-stage flbrosis.Thus, early recognition and timely initiation of treatment may be the key to recover visceral peristaltic function in patients with GML. | Frederick D Park Jeffrey K Lee Ganga D Madduri Pradipta Ghosh | 2009 | World Journal of Gastroenterology2009,15,28: | 20 |
| 2 | Risk of New-Onset Atrial Fibrillation in Elderly Patients with the Overlap Syndrome: A Retrospective Cohort Study显示文摘Background Co-existence of obstructive sleep apnea (OSA) and chronic obstructive pulmonary disease (COPD) is referred to as overlap syndrome. Overlap patients have greater degree of hypoxia and pulmonary hypertension than patients with OSA or COPD alone. Studies showed that elderly patients with OSA alone do not have increased risk of atrial fibrillation (AF) but it is not known if overlap patients have higher risk of AF. Objective To determine whether elderly patients with overlap syndrome have an increased risk of AF. Methods In this single center, community-based retrospective cohort analysis, data were collected on 2,873 patients > 65 years of age without AF, presenting in the year 2006. Patients were divided into OSA group (n = 60), COPD group (n = 416), overlap syndrome group (n = 28) and group with no OSA or COPD(n = 2369). The primary endpoint was incidence of new-onset AF over the following two years. Logistic regression was performed to adjust for heart failure (HF), coronary artery disease, hypertension (HTN), cerebrovascular disease, cardiac valve disorders, diabetes mellitus, hyperlipidemia, chronic kidney disease (CKD) and obesity. Results The incidence of AF was 10% in COPD group, 6% in OSA group and 21% in overlap syndrome group (P < 0.05). After adjusting for age, sex, HF, CKD, and HTN, patientswith overlap syndrome demonstrated a significant association with new-onset AF (OR = 3.66, P = 0.007). HF, CKD and HTNwere also significantly associated with new-onset AF (P < 0.05). Conclusion Among elderly patients, the presence of overlap syndrome is associatedwith a marked increase in risk of new-onset AF as compared to the presence of OSA or COPDalone. | Harsha V. | 3 | Endoscopic management of postcholecystectomy biliary leakage显示文摘BACKGROUND: Biliary leak is an uncommon but significant complication following cholecystectomy. Endotherapy is an established method of treatment. However, the optimal intervention is not known. METHOD: Eighty-five patients with postcholecystectomy biliary leaks from July 2000 to March 2009 were retrospectively evaluated. RESULTS: The study population was 20 males and 65 females with a mean age of 42.47 years. Patients presented with abdominal pain (46), jaundice (23), fever (23), abdominal distension (42), or bilious abdominal drain (67). Endoscopic retrograde cholangiopancreatography detected a leak at the cystic duct stump in 45 patients, stricture with middle common bile duct leak in 4, leak from the right hepatic duct in 3, and a ligated common bile duct in 32. Twelve also had bile duct stones. One had a broken T-tube with stones Endotherapy was possible in 53 patients. Three patients with stones, one with a broken T-tube with stones, and 4 with stricture of the common bile duct with a leak were managed with sphincterotomy and stenting. Eight patients with a cystic duct stump leak with stones were managed with sphincterotomy and stone extraction. Three outpatients and 12 inpatients with a cystic duct stump leak were managed with sphincterotomy and stent and sphincterotomy and nasobiliary drain, respectively. Five patients with a cystic duct stump leak were managed with stenting. Sixteen with coagulopathy were managed with only nasobiliary drain (9) or stent (7). Leak closure was achieved in 100% patients Four developed mild pancreatitis which improved with conservative treatment.CONCLUSIONS: Endoscopic intervention is a safe and effective method of treatment of postcholecystectomy biliary leaks. | 4 | Conditioning and PPP processing of smartphone GNSS measurements in realistic environments显示文摘Smartphones typically compute position using duty-cycled Global Navigation Satellite System (GNSS) L1 code measurements and Single Point Positioning (SPP) processing with the aid of cellular and other measurements. This internal positioning solution has an accuracy of several tens to hundreds of meters in realistic environments (hand-held, vehicle dashboard, suburban, urban forested, etc.). With the advent of multi-constellation, dual-frequency GNSS chips in smartphones, along with the ability to extract raw code and carrier-phase measurements, it is possible to use Precise Point Positioning (PPP) to improve positioning without any additional equipment. This research analyses GNSS measurement quality parameters from a Xiaomi MI 8 dual-frequency smartphone in varied, realistic environments. In such environments, the system suffers from frequent phase loss-of-lock leading to data gaps. The smartphone measurements have low and irregular carrier-to-noise (C/N0) density ratio and high multipath, which leads to poor or no positioning solution. These problems are addressed by implementing a prediction technique for data gaps and a C/N0-based stochastic model for assigning realistic a priori weights to the observables in the PPP processing engine. Using these conditioning techniques, there is a 64% decrease in the horizontal positioning Root Mean Square (RMS) error and 100% positioning solution availability in sub-urban environments tested. The horizontal and 3D RMS were 20 cm and 30 cm respectively in a static open-sky environment and the horizontal RMS for the realistic kinematic scenario was 7 m with the phone on the dashboard of the car, using the SwiftNav Piksi Real-Time Kinematic (RTK) solu-tion as reference. The PPP solution, computed using the YorkU PPP engine, also had a 5-10% percentage point more availability than the RTK solution, computed using RTKLIB software, since missing measurements in the logged file cause epoch rejection and a non-continuous solution, a problem which is solved by prediction for the PPP solution. The internal unaided positioning solution of the phone obtained from the logged NMEA (The National Marine Elec-tronics Association) file was computed using point positioning with the aid of measurements from internal sensors. | 5 | Investigation of hepatoprotective activity of Cyathea gigantea(Wall.ex.Hook.)leaves against paracetamol-induced hepatotoxicity in rats显示文摘Objective:To investigate the hepatoprotective activity of methanolic leaf extract of Cyathea gigantea(C.gigantea)against paracetamol induced liver damage in rats.Methods:The hepatoprotective activity for plant extract was investigated for paracetamol induced hepatoxicity in rats.Wislar albino rats of either sex were divided into five groups of 6 animals each and are given orally the following treatment for seven days.The normal control group was given 1%Na.CMC 1mL/kg bw,p.o.Paracetamol at dose of 1g/kg bw,p.o.was given as toxic dose for inducing hepatoloxicity.Silymarin(50mg/kg.p.o.) was given as reference standard.Two doses of C. gigantea extract i.e.,100 mg/kg.p.o.and 200 mg/kg,p.o.were tested for hepatoprotective activity. The treatment was given for seven days and after 24 h of last treatment blood was collected from retro-orbital plexus and analysed for various serum parameters like serum glutamic-oxaloacetic transaminase(SGOT),serum glutamic pyruvic transaminase(SGPT),alkaline phosphatase(ALP),total bilirubin(TB)and total protein(TP)in different groups.Results:The paracetamol intoxication lead to histological and biochemical deteriorations.The treatment with methanolic leaf extract of C.gigantea reduced the elevated levels of SCOT,SGPT,ALP,TB and also reversed the hepatic damage towards normal which further supports the hepatoprotective activity of leaf extract of C.gigantea.Conclusions:The methanolic extract of leaves of C.gigantea at doses of 100 mg/kg bw and 200 mg/kg bw have significant effect on liver of paracetamol induced hepatotoxicity model in rats. | P Madhu Kiran A Vijaya Raju B Ganga Rao | 2012 | Asian Pacific Journal of Tropical Biomedicine2012,2,5: | 7 |
| 6 | Evaluation of SWAT sub-daily runoff estimation at small agricultural watershed in Korea显示文摘A study was undertaken for the prediction of runoff flow from 0.8 ha field-sized agricultural watershed in South Korea using Soil and Water Assessment Tool (SWAT) sub-daily. The SWAT model with sub-daily configuration predicted flow from the watershed within the range of acceptable accuracy. The SWAT sub-daily simulations were carried out for a total of 18 rainfall events, 9 each for calibration and validation. Overall trend and extent of matching simulated flow for the rainfall events in 2007-2008 with measured data during the calibration process were coefficient of determination (R2) value of 0.88 and Nash and Sutcliffe Efficiency (ENs) value of 0.88. For validation, R2 and ENs values were 0.9 and 0.84, respectively. Whereas R2 and ENs values for simulation results using daily rainfall data were 0.79 and -0.01, respectively, that were observed to be out of acceptable limits for the model simulation. The importance of higher time resolution (hourly) precipitation records for flow simulation were evaluated by comparing Ra and ENs with 15 min, 2 h, 6 h and 12 h precipitation data, which resulted in lower statistics with increases in time resolution of precipitation data. The SWAT sub-daily sensitivity analysis was performed with the consideration of hydraulic parameter and was found as in the rank order of CN2 (curve number), ESCO (soil evaporation compensation factor), GW_DELAY (ground water delay time), ALPHA BF (base flow alpha factor), GWQMN (a threshold-minimum depth of water in the shallow aquifer required for return flow to occur), REVAPMN (minimum depth of water in shallow aquifer for re-evaporation to occur), LAT TIME (lateral flow travel time) respectively. These sensit]-ve parameters were evaluated at 10% higher and lower values of the parameters, corresponding to 70.5% higher and 23.2% lower in simulated flow out from the SWAT model. The methoxy substituted indolecarbazole parent nucleus, which was firstly synthesized, is condensed with bromine substituted amino acid methyl ester to produce the target compounds.
