| 1 | Outcome of gall bladder polypoidal lesions detected by transabdominal ultrasound scanning: A nine year experience显示文摘AIM: To determine the outcome of polypoidal lesions within the gall bladder (PLG) diagnosed by trans-abdominal scanning.METHODS: A nine-year (1993-2002) retrospective casenote review of all patients who underwent ultrasound scanning after referral to a single Upper GI Surgeon at a District General Hospital was conducted. Patients who were diagnosed with a PLG were included in our study. A database was constructed and patient details, investigations including ultrasound scan (USS) findings, treatment and histology and final diagnosis were recorded. RESULTS: Twenty-three (out of 651) patients were diagnosed pre-operatively by USS to have a polyp-likegall bladder lesion (PLG). Post cholecystectomy histological examination revealed 12 gallstones, 7 cholesterol polyps, 3 adenocarcinomas within polyps and 1 normal gall bladder. The specificity of USS in the diagnosis of PLG was 92.3%. All the true polyps were malignant. Overall USS had 66.66% sensitivity and 100% specificity in the pre-operative suspicion of malignancy. Using size greater than 10 mm as measured on USS as a cut-off, we find 100% sensitivity and 86.95% specificity with a positive predictive value of 50% in the diagnosis of malignancy in PLG.CONCLUSION: A large number of PLG are in fact calculi within diseased gall bladder. In cases of gall bladder polyps more then 10 mm in size on USS further imaging (crosssectional and/or EUS) is indicated prior to surgery. | 3 | A new design of 3D-printed orthopedic bone plates with auxeticstructures to mitigate stress shielding and improve intra-operative bending显示文摘Orthopedic bone plates are most commonly used for bone fracture fixation for more than 100 years.The bone plate design had evolved over time overcoming many challenges such as insufficient strength and excessive plate–bone contact affecting the blood circulation.However,it is only made of two materials,either stainless steel(AISI 316L)or titanium(Ti–6Al–4V).There are two main limitations of metallic bone implants,namely stress shielding and the problem of malocclusion caused by the displacement of the fracture site during healing.To overcome the two problems,a new bone plate design with the incorporation of auxetic structures is proposed in this work.This study aims to use auxetic structure section in the bone plate that would decrease the stiffness of the region,thereby mitigating the stress-shielding effect and at the same time act as a deformable section to enable intra-operative bending for effective alignment while having enough bending strength and stiffness.Two different auxetic structures namely re-entrant honeycomb and missing rib structures were considered.The auxetic structure incorporated bone plates were designed,finite element analysis was done,fabricated using direct metal laser sintering technique,and tested.The results indicate that the re-entrant honeycomb structure incorporated bone plates serve as an effective bone design compared to the conventional bone plate design,in terms of stress shielding and intra-operative bending while offering similar mechanical and bending strength. | Sanjairaj Vijayavenkataraman Akhil Gopinath Wen F.Lu | 2020 | Bio-Design and Manufacturing2020,3,2: | 4 |

| 9 | Malaria elimination in Lao PDR:the challenges associated with population mobility显示文摘Although the Lao People’s Democratic Republic(Lao PDR)is comparatively small landlocked country with patterns of both in-and out-migration,its human migration situation has been poorly studied.This is despite all of the country’s 18 provinces sharing both official and unofficial border checkpoints with neighboring countries.Economic reforms in the last decade have seen a gradual increase in the promotion of foreign investment,and main towns and transportation networks have been expanding thus offering new opportunities for livelihoods and economic activities.In the last decade,there has also been a significant reduction of reported malaria cases in Lao PDR and while this is an important prerequisite for eliminating malaria in the country,malaria outbreaks reported in the last four years suggest that population mobility,particularly in the south,is an important factor challenging current control efforts.Bolder investment in social sector spending should be geared towards improving health service provision and utilization,ensuring equitable access to primary health care(including malaria)through efforts to achieve universal health coverage targets.This should be extended to populations that are mobile and migrants.The local government plays a critical role in supporting policy and enforcement issues related to private sector project development in the provinces.Cross-border initiatives with neighboring countries,especially in terms of data sharing,surveillance,and response,is essential.Mechanisms to engage the private sector,especially the informal private sector,needs to be explored within the context of existing regulations and laws.Existing and new interventions for outdoor transmission of malaria,especially in forest settings,for high-risk groups including short-and long-term forest workers and their families,mobile and migrant populations,as well as the military must be combined into integrated packages with innovative delivery mechanisms through social marketing approaches.This should happen at multiple points in the mobility pathway and involve the private sector rather than being fully reliant on the national malaria vertical program This article based on the review of existing literature from abstracts and full texts,includes published,peer-reviewed English language literature sourced through PubMed and grey literature sources through Google and Google Scholar.The review included also case reports,sector reports,conference proceedings,research reports,epidemiology studies,qualitative studies,and census reports in both Lao and English languages.The authors used the search terms:malaria and mobile populations,malaria control program and elimination,health system performance,malaria outbreak,Lao PDR;and included articles published until June 2015. | Sengchanh Kounnavong Deyer Gopinath Bouasy Hongvanthong Chanthalone Khamkong Odai Sichanthongthip | 2017 | Infectious Diseases of Poverty2017,6,1: | 2 |
