| 1 | Necessity and indications of invasive treatment for Budd-Chiari syndrome显示文摘BACKGROUND:The development of collaterals in Budd-Chiari syndrome has been described and these collaterals play an important role in the presentation of this disease.These collaterals are diagnostic and their use in management strategy has never been evaluated.This study aimed to investigate the indications,feasibility and necessity of invasive treatment for patients with Budd-Chiari syndrome and to determine whether such a strategy is necessary for optimal management.METHODS:Twenty-nine patients who had been treated at our unit were enrolled in this study.Based on physical and biochemical examination,and hemodynamic compensation by collaterals,18 patients underwent radiological intervention (group A),while the other 11 had no invasive treatment (group B).The related hemodynamic parameters were acquired when percutaneous angiography was performed.RESULTS:In group A,all patients underwent successfully inferior vena cava (IVC) balloon angioplasty with or without stenting.Four patients also underwent hepatic vein angioplasty.In these patients,the mean IVC pressure before and after treatment was statistically different (29.3±9.2 vs 15.1±4.6 mmHg,P<0.01).The mean IVC pressure was much lower in group B than in group A (12.9±2.4 vs 29.3±9.2 mmHg,P<0.01),but there was no difference from that of the patients after radiological treatment (12.9±2.4 vs 15.1±4.6 mmHg,P>0.05).Median follow-up was 32.3 months (mean 21.3 months;range 3-61 months).In the course of follow-up,the patients in group A survived with good systemic status except for re-stenosis in one patient who underwent re-canalization of the IVC.In group B,10 patients had good systemic status except one patient who had a meso-caval shunt because of deterioration.CONCLUSIONS:The rationale of 'early diagnosis and early treatment' is not suitable for all patients with Budd-Chiari syndrome.Satisfactory survival can be achieved in some patients without invasive treatment,who are completely compensated by rich collaterals.Nonetheless,a positive treatment procedure should be performed if the patient's situation worsens in the course of regular follow-up. | Department of General Surgery (Fu Y,Sun YL,Ma XX,Xu PQ,Feng LS,Tang Z and Luo CH),Institute of Hepatic Vascular Disease (Sun YL),Department of Radiological Intervention (Guan S and Wang ZW),First Affiliated Hospital,Zhengzhou University School of Medicine,Zhengzhou 450052,China | 2011 | Hepatobiliary & Pancreatic Diseases International2011,10,3: | 10 |
| 2 | HBsAg loading on dendritic cells in patients with chronic hepatitis B: expressions of phenotypic molecules显示文摘BACKGROUND: The antigen reducing ability of dentritic cells (DCs), a kind of antigen presenting cells (APCs) initiating immune response, is associated with the specific immune tolerance of chronic hepatitis B(CHB) patients. However, the dysfunction of DCs can be possibly reversed by the stimulation of antigen peptides. In this study, DCs were cultured from peripheral blood monocytes (PBMCs) in patients with CHB in vitro, and the expression of phenotypic molecules on DCs loaded by different concentrations of HBsAg was observed. METHODS: Forty patients with CHB were divided randomly into 4 groups(10 patients in each group). PBMCs were isolated, and DCs were cultured after addition of granulocyte/macrophage colony-stimulating factor (GMCSF) and interleukin 4(IL-4). On the 9th day, DCs of the experimental groups were loaded at HBsAg concentrations of 2.5mg/L, 5mg/L and 10mg/L for 24 hours, whereas those of the control group were not loaded. An electron microscope was used to analyze the morphological changes of the DCs. The expression of phenotypic molecules on DCs in different groups was detected with flow cytometry. RESULTS: A combination of GM-CSF and IL-4 produced DCs from PBMCs in patients with CHB after being cultured for 9 days, whose morphological changes were tested by an electron microscope. The expression of phenotypic molecules on DCs in the control group was as low as CD83 (8.02±3.99)%, CD80(8.77±2.06)%, and MHC-DR (14.05±2.66)%. Loaded by different concentrations of HBsAg, the up-regulation of phenotypic molecules on DCs was found, with CD83(18.35±2.93)%, CD80(42.63±7.15)% and MHC-DR(47.49±6.59)% in 2.5mg/L HBsAg loading group, CD83(17.88±3.12)%, CD80(45.24± 10.93)% and MHC-DR(47.07±8.52)% in 5mg/L HBsAg loading group and CD83(16.74±2.86)%, CD80(44.59±6.99)% and MHC-DR(48.59±7.42)% in 10mg/L HBsAg loading group, respectively. Compared with the control group, the phenotypic molecules in the experimental groups were all different significantly (P<0.01), but among them, there were no differences (P>0.05). CONCLUSIONS: DCs cultured from PBMCs in the patients with CHB under the conditions of GM-CSF and IL-4 present on the typical dendritic morphology but are immature for expressing low phenotypic molecules. | 3 | Interoperability of Design Intent in Integrated Product and Process Design显示文摘Concurrent engineering is currently an overwhelming t rend in product development since it takes the down-stream product development issues and product life-cycle issues into consideration in up-front design pro cess. At this stage, evaluation, verification and validation of design concepts and design schemes is critical to the right-to-market product development. I dentification and extraction of design intent from the part digital mock-ups in different data formats and created in different CAD systems for DFX considerati on, evaluation, verification, and validation is crucial too. Traditionally, fro m manufacturing point of view, feature identification and extraction is to get t he features and feature-related information for manufacturing planning and inte gration. Presently, from the viewpoint of product development and product servi ce life cycle, design intent identification and extraction is to get all the des ign-related information for DFX consideration in early design stage, such as de sign for assembly, design for service, design for tooling, design for functional ity and design for environment etc. Therefore, how to successfully extract desi gn intent such as design and machining features and design-related information from the prototyping digital mock-ups created by different CAD systems for DFX consideration in upstream design stage is a very useful topic in integrated prod uct and process design and development. Presently, there are heterogeneous CAD systems in the market and the interoperat ion among the different CAD systems is indispensable. The interoperability of d ifferent CAD systems is by way of data exchanged technology such as STEP and IGE S. The critical weakness of this interoperation technology is that much of the design intent is lost in the interoperation process. Once the product digital m ock-ups is exchanged via data exchange technology such as IGES or STEP, the des ign intent including the dimension-driven design features and other attributes embedded in the part model are lost in the data translation process. Models rea d out of CAD systems come into other systems as featureless chunks of geometry e ntities. Obviously, the interoperation technology does not satisfy the industry need. It has been reported that this poor interoperation technology, which pro vides poor CAD models in neutral-format geometry had cost the automobile indust ry alone $1 billon per year. Fortunately, some researchers have been aware of t he criticality of this since only the United States industry spends billons of d ollars as a result of poor interoperability between computer-aided engineering software tools. In the past, product development was often done within a single company by co-located design team. In more recent years, there has been a busi ness paradigm shift in product development as the product development is being d one more often by geographically and temporally distributed design teams. There is a high level of outsourcing, not only of manufacturing but also of actual pr oduct development effort. Besides, product development across companies, and ev en within a single company, is often done within a heterogeneous software tool e nvironment. How to improve the interoperability of design intent created in het erogeneous software tool environment geographically has become a critical issue in next-generation product development system. Design intent in this research is defined as the outcome of design process. Des ign process goes from the identification of needs to satisfaction of that needs. Design intent satisfies this kind of needs. In this research, design intent i s created in a CAD system and embedded in part CAD model.
