| 1 | 北京市居民抗生素使用情况调查显示文摘[目的]了解北京市居民的抗生素使用情况。[方法]随机选择北京市8个区,收集当地人口和电话分布情况,并随机选择电话号码,进行电话调查。并采用EpiInfo3.3.2软件对数据进行录入和分析。主要的统计学方法有描述性分析和卡方检验等。[结果]2004年,调查人群中有47%的人使用过抗生素。其中54%的人就诊以后使用抗生素。在没有就诊的人群中,自行使用抗生素的人群中,74%的人认为病情不严重,能够自行处理,不需要就诊。9%的人认为没有时间前去就诊。使用的抗生素中,30%为头孢菌素类,16%是青霉素,12%为中药类抗生素。是否根据处方购买抗生素,与被调查者教育程度的差异无统计学意义(小学程度以下,占42%;高中及中专,占52%;大学及以上,占61%;P=0.59)但是对抗生素使用知识的了解程度上,和教育程度的差异有高度统计学意义(小学程度以下,占7%;中学程度,44%;大学程度,42%;大学以上,5%;其他占2%;P=0.001)。对于抗生素使用的了解情况调查,女性比男性更多了解,两者差异有统计学意义(男,占45%;女,占66%;P=0.0001)。居民使用的抗生素的来源,主要是医院和药店,占51%和33%。60%的居民说,如果没有处方,药店里不能购买到抗生素。[结论]在北京居民中,46%的人不根据医生处方而使用抗生素。医院里没有根据医嘱需要而开处方是抗生素使用的主要问题,应该加强医院出售抗生素的管理。 | 李芙蓉 熊传龙 蔡剑 常利涛 王希江 曾光 Robert E. | 2 | Endoscopic ultrasound staging for early esophageal cancer: Are we denying patients neoadjuvant chemo-radiation?显示文摘AIM To evaluate the accuracy of endoscopic ultrasound(EUS) in early esophageal cancer(EC) performed in a highvolume tertiary cancer center. METHODS A retrospective review of patients undergoing esophagectomy was performed and patients with c T1 N0 and c T2 N0 esophageal cancer by EUS were evaluated. Patient demographics, tumor characteristics, and treatment were reviewed. EUS staging was compared to surgical pathology to determine accuracy of EUS. Descriptive statistics was used to describe the cohort. Student's t test and Fisher's exact test or χ~2 test was used to compare variables. Logistic regression analysis was used to determine if clinical variables such as tumor location and tumor histology were associated with EUS accuracy.RESULTS Between 2000 and 2015, 139 patients with clinical stage Ⅰ or Ⅱ?A esophageal cancer undergoing esophagectomy were identified. There were 25(18%) female and 114(82%) male patients. The tumor location included the middle third of the esophagus in 11(8%) and lower third and gastroesophageal junction in 128(92%) patients. Ninety-three percent of patients had adenocarcinoma. Preoperative EUS matched the final surgical pathology in 73/139 patients for a concordance rate of 53%. Twenty-nine patients(21%) were under-staged by EUS; of those, 19(14%) had unrecognized nodal disease. Positron emission tomography(PET) was used in addition to EUS for clinical staging in 62/139 patients. Occult nodal disease was only found in 4 of 62 patients(6%) in whom both EUS and PET were negative for nodal involvement. CONCLUSION EUS is less accurate in early EC and endoscopic mucosal resection might be useful in certain settings. | 6 | Low doses of intravenous epinephrine for refractory sustained monomorphic ventricular tachycardia显示文摘We report three cases of sustained monomorphic ventricular tachycardia(VT) in the setting of coronary artery disease,resistant to beta-blockers in two patients and to amiodarone in all,successfully terminated by low doses of intravenous(IV) epinephrine.VT was the first manifestation of coronary artery disease in one patient,whereas the other two patients had a previous history of myocardial infarction and were recipients of an implantable cardioverter-defibrillator(ICD).One of these two patients experienced an arrhythmic storm.All had hemodynamic instability at the time of epinephrine administration.A single slow administration of IV epinephrine(0.5 to 1 mg administered over 30 to 60 s) restored sinus rhythm after 30-90 s with only minor side effects.In the ICD patient with recurrent VT and several cardioversions due to transformation of VT to ventricular fibrillation,epinephrine injection led to the avoidance of further shocks.Although potentially harmful,low doses of IV epinephrine used alone or in combination with beta-blocker treatment and electrical cardioversion may be an alternative effective therapy for sustained monomorphic VT refractory to amiodarone.The role of epinephrine in the termination of VT should be studied further,especially in patients pre-treated with amiodarone in combination with beta-blockers. | Aimé Bonny Antonio De Sisti Manlio F Márquez Richard Megbemado Franoise Hidden-Lucet Guy Fontaine | 2012 | World Journal of Cardiology2012，4，10： | 3 |

| 8 | Effect of sustained virological response on long-term clinical outcome in 113 patients with compensated hepatitis C-related cirrhosis treated by interferon alpha and ribavirin显示文摘AIM:To assess the long-term clinical benefit of sustained virological response(SVR)in patients with hepatitis C virus(HCV)cirrhosis treated by antiviral therapy using mostly ribavirin plus interferon either standard or pegylated.METHODS:One hundred and thirteen patients with uncomplicated HCV biopsy-proven cirrhosis,treated by at least one course of antiviral treatment ≥ 3 mo and followed ≥ 30 mo were included.The occurrence of clinical events hepatocellular carcinoma(HCC),decompensation and death was compared in SVR and non SVR patients.RESULTS:Seventy eight patients received bitherapy and 63 had repeat treatments.SVR was achieved in 37 patients(33%).During a mean follow-up of 7.7 years,clinical events occurred more frequently in non SVR than in SVR patients,with a significant difference for HCC(24/76 vs 1/37,P = 0.01).No SVR patient died while 20/76 non-SVR did(P = 0.002),mainly in relation to HCC(45%).CONCLUSION:In patients with HCV-related cirrhosis,SVR is associated with a significant decrease in the incidence of HCC and mortality during a follow-up period of 7.7 years.This result is a strong argument to perform and repeat antiviral treatments in patients with compensated cirrhosis. | Roland El Braks Nathalie Ganne-Carrié Hélène Fontaine Jacques Paries Véronique Grando-Lemaire Michel Beaugrand Stanislas Pol Jean-Claude Trinchet | 2007 | World Journal of Gastroenterology2007，13，42： | 3 |
| 9 | Chemical constituents, cytotoxic, antifungal and antimicrobial properties of Centaurea diluta Ait. subsp. algeriensis(Coss. & Dur.) Maire显示文摘Objective:To investigate the chemical composition of a moderately polar extract(CHC1_3 soluble part of the MeOH-H_2O extract) obtained from the aerial parts(leaves and flowers) of Centaurea diluta Ait.subsp.algeriensis(Coss.& Dur.) Maire,a species endemic to Algeria and Morocco on which no reports are available to date.To evaluate in vitro the cytotoxic,antifungal and antimicrobial activities of this extract and the cytotoxic and antimicrobial activities of its isolated secondary metabolites.Methods:The cytotoxic effects of the extract were investigated on 3 human cancer cell lines i.e.the A549 non-small-cell lung carcinoma(NSCLC),the MCF7 breast adenocarcinoma and the U373 glioblastoma using a MTT colorimetric assay.Biological data allowed to guide the fractionation of the extract by separation and purification on silica gel 60(CC and TLC).The isolated compounds which were characterized by spectral analysis,mainly HR-ESIMS,HR-EIMS,UV and NMR experiments(~1H,^(13)C,COSY,ROESY,HSQC and HMBC) and comparison of their spectroscopic data with those reported in the literature,were evaluated for cytotoxic activities on six cancer cell lines(A549,MCF7,U373,Hs683 human glioma,PC3 human prostate and B16-F10 murine melanoma).The direct and indirect antibacterial and antifungal activities were determined using microdilution methods for the raw extract and TLC-bioautography and microdilution methods against standard and clinical strains for the isolated compounds.Results:The raw extract reduced cell viability with IC_(50)s of 27,25 and 21 μg/mL on A549,MCF7 and U373,respectively.Five secondary metabolites:two phenolic compounds(vanillin 1,paridol 3),a lignan[(-)-arctigenin 2]and two flavonoid aglycones(eupatilin 4 and jaceosidin 5),were then isolated from this extract.Moderate cytotoxic effects were observed for(-)-arctigenin 2(IC_(50)s:28 and 33μM on Hs683 and B16-F10,respectively),eupatilin 4(IC_(50)s:33 and 47 μM on B16-F10 and PC3,respectively) and jaceosidin 5(IC_(50)s:32 and 40 μM on PC3 and B16-F10,respectively).Conclusions:All the isolated compounds were described for the first time from this species.Although inactive against 7 tested microorganisms(fungi,bacteria and yeast,human or plant pathogens),the raw extract was able to potentiate the effect of beta-lactam antibiotics on methicillin-resistant Staphylococcus aureus(MRSA),reducing the minimal inhibitory concentrations(MICs) by a factor of 2-32-fold.No synergy was found between the extract and streptomycin.From the five isolated compounds only jaseosidin 5 showed a moderate antimicrobial activity. | Hanene Zater Joelle Huet Veronique Fontaine Samir Benayache Caroline Stevigny Pierre Duez Fadila Benayache | 2016 | Asian Pacific Journal of Tropical Medicine2016，9，6： | 2 |

| 14 | Vanadium-dependent activation of glucose transport in adipocytes by catecholamines is not mediated via adrenoceptor stimulation or monoamine oxidase activity显示文摘BACKGROUND Benzylamine and methylamine activate glucose uptake in adipocytes.For tyramine,this effect has even been extended to cardiomyocytes.AIM To investigate the effects of catecholamines and other amines on glucose uptake.METHODS A screening compared 25 biogenic amines on 2-deoxyglucose(2-DG)uptake activation in rat adipocytes.Pharmacological approaches and transgenic mouse models were then used to decipher the mode of action of several hits.RESULTS In rat adipocytes,insulin stimulation of 2-DG uptake was reproduced with catecholamines.100μmol/L or 1 mmol/L adrenaline,noradrenaline,dopamine and deoxyepinephrine,maximally activated hexose transport only when sodium orthovanadate was added at 100μmol/L.Such activation was similar to that already reported for benzylamine,methylamine and tyramine,well-recognized substrates of semicarbazide-sensitive amine oxidase(SSAO)and monoamine oxidase(MAO).Several,but not all,tested agonists ofβ-adrenoreceptors(β-ARs)also activated glucose transport whileα-AR agonists were inactive.Lack of blockade byα-andβ-AR antagonists indicated that catecholamine-induced 2-DG uptake was not mediated by AR stimulation.Adipocytes from mice lackingβ1-,β2-andβ3-ARs(triple KO)also responded to millimolar doses of adrenaline or noradrenaline by activating hexose transport in the presence of 100μmol/L vanadate.The MAO blocker pargyline,and SSAO inhibitors did not block the effects of adrenaline or noradrenaline plus vanadate,which were blunted by antioxidants.CONCLUSION Catecholamines exert unexpected insulin-like actions in adipocytes when combined with vanadium.For limiting insulin resistance by activating glucose consumption at least in fat stores,we propose that catecholamine derivatives combined with vanadium can generate novel complexes that may have low toxicity and promising anti-diabetic properties. | Jessica Fontaine Geneviève Tavernier Nathalie Morin Christian Carpéné | 2020 | World Journal of Diabetes2020，11，12： | 2 |
