2Laparoscopic Partial Nephrectomy With Segmental Renal Artery Clamping:Technique and Clinical Outcomes显示文摘Background:Warm ischemic injury is one of the most important factors affecting renal function in partial nephrectomy(PN).The technique of segmental renal artery clamping emerges as an alternative to conventional renal artery clamping for renal hilar control.Objective:To evaluate the feasibility and efficiency of laparoscopic PN(LPN) with segmental renal artery clamping in comparison with the conventional technique.Design,setting,and participants:A total of 75 patients underwent LPN from June 2007 to November 2009.All patients had T1a or T1b tumor in one kidney and a normal contralateral kidney.Thirty-seven patients underwent surgeries with main renal artery clamping,and 38 underwent surgeries with segmental artery clamping.Intervention:All procedures were performed by the same laparoscopic surgeon.Measurements:Blood loss,operation time,warm ischemia(WI) time,and complications affected renal function before and after operation were recorded.Results and limitations:All LPNs were completed without conversion to open surgery or nephrectomy.The novel technique slightly increased WI time(p < 0.001) and intraoperative blood loss(p = 0.006),while it provided better postoperative affected renal function(p < 0.001) compared with the conventional technique.The total complication rate was 12%.Among the 38 cases where segmental renal artery clamping was performed,7 had to convert to the conventional method.Tumor size and location influenced the number of clamped segmental arteries.Long-term postoperative renal function is still awaited.Conclusions:LPN with segmental artery clamping is safe and feasible in clinical practice.It minimizes the intraoperative WI injury and improves early postoperative affected renal function compared with main renal artery clamping.(C) 2010 European Association of Urology.Published by Elsevier B.V.All rights reserved.Shao,P.F. 11Ideal cardiovascular health and incidence of atherosclerotic cardiovascular disease among Chinese adults:the China-PAR project显示文摘Existing evidence on the relationship between cardiovascular health(CVH) metrics and cardiovascular disease(CVD) was primarily derived from western populations. We aimed to evaluate the benefits of ideal CVH metrics on preventing incident atherosclerotic CVD(ASCVD) in Chinese population. This study was conducted among 93,987 adults from the China-PAR project(Prediction for ASCVD Risk in China) who were followed up until 2015. Cox proportional hazard regression models were used to estimate the hazard ratios(HRs) and their corresponding 95% confidence intervals(CIs) of CVH metrics for the risk of ASCVD, including coronary heart disease(CHD), stroke and ASCVD death. We further estimated the population-attributable risk percentage(PAR%) of these metrics in relation to each outcome. We observed gradient inverse associations between the number of ideal CVH metrics and ASCVD incidence. Compared with participants having ≤2 ideal CVH metrics, the multivariable-adjusted HRs(95% CIs) of ASCVD for those with 3, 4, 5, 6 and 7 ideal CVH metrics were 0.83(0.74–0.93), 0.66(0.59–0.74), 0.55(0.48–0.61), 0.44(0.38–0.50) and 0.24(0.18–0.31), respectively(P for trend <0.0001). Approximately 62.1% of total ASCVD, 38.7% of CHD, 66.4% of stroke, and 60.5% of ASCVD death were attributable to not achieving all the seven ideal CVH metrics. After adjusting effects of ideal health factors, having four ideal health behaviors could independently bring adults health benefits in preventing 17.4% of ASCVD, 18.0% of CHD, 16.7% of stroke, and 10.1% of ASCVD death. Among all the seven CVH metrics, to keep with ideal blood pressure(BP) implied the largest public health gains against various ASCVD events(PAR% between 33.0% and 47.2%), while ideal diet was the metric most difficult to be achieved in the long term. Our study indicates that the more ideal CVH metrics adults have, the less ASCVD burden there is in China. 12Injectable hydrogels for cartilage and bone tissue engineering显示文摘Tissue engineering has become a promising strategy for repairing damaged cartilage and bone tissue. Among the scaffolds for tissue-engineering applications, injectable hydrogels have demonstrated great potential for use as three-dimensional cell culture scaffolds in cartilage and bone tissue engineering, owing to their high water content, similarity to the natural extracellular matrix(ECM), porous framework for cell transplantation and proliferation, minimal invasive properties, and ability to match irregular defects. In this review, we describe the selection of appropriate biomaterials and fabrication methods to prepare novel injectable hydrogels for cartilage and bone tissue engineering. In addition, the biology of cartilage and the bony ECM is also summarized. 13Roux-en-Y choledochojejunostomy using novel magnetic compressive anastomats in canine model of obstructive jaundice显示文摘BACKGROUND:The traditional hand-sewn Roux-en-Y choledochojejunostomy is technically complicated,and the incidence of postoperative complications has remained high.A set of novel magnetic compressive anastomats was introduced to facilitate choledochojejunostomy and improve the prognosis of patients.METHODS:After ligating the common bile duct for 7 days,16 dogs were randomly divided into two groups (n=8 per group).Anastomats were used in the study group,and the traditional hand-sewn method was used in the control group for standard Roux-en-Y choledochojejunostomy.We compared the operation time,incidence of complications,gross appearance,and pathological disparity in stoma between the two groups in 1-month and 3-month follow-up examinations.RESULTS:The time spent on constructing the anastomosis for the study group was significantly shortened.Although no anastomotic stenosis occurred in the two groups,the narrowing rate of biliary-enteric anastomosis was much higher in the control group.There was one case of bile leakage in the control group,whereas no bile leakage occurred in the study group.A smoother surface,an improved layer apposition,and a lower local inflammatory response were identified in the anastomosis of the study group.CONCLUSION:The structures of the novel magnetic compressive anastomats are simple,and they are time-saving,safe and efficient for performing Roux-en-Y choledocho- jejunostomy procedures in a canine model of obstructive jaundice.Chao Fan,Xiao-Peng Yan,Shi-Qi Liu,Chun-Bao Wang,Jian-Hui Li,Liang Yu,Zheng Wu and Yi Lv Department of Hepatobiliary Surgery and Department of Pathology,First Affiliated Hospital,School of Medicine,Xi’an Jiaotong University,Xi’an 710061,China Department of Surgical Oncology,Third Affiliated Hospital,School of Medicine,Xi’an Jiaotong University,Xi’an 710068,China 2012Hepatobiliary & Pancreatic Diseases International2012,11,1:49
143-N-butylphthalide improves neuronal morphology after chronic cerebral ischemia显示文摘3-N-butylphthalide is an effective drug for acute ischemic stroke. However, its effects on chronic cerebral ischemia-induced neuronal injury remain poorly understood. Therefore, this study ligated bilateral carotid arteries in 15-month-old rats to simulate chronic cerebral ischemia in aged humans. Aged rats were then intragastrically administered 3-n-butylphthalide. 3-N-butylphthalide administration improved the neuronal morphology in the cerebral cortex and hippocampus of rats with chronic cerebral ischemia, increased choline acetyltransferase activity, and decreased malondialdehyde and amyloid beta levels, and greatly improved cognitive function. 16A Review on Lithium-ion Power Battery Thermal Management Technologies and Thermal Safety显示文摘Lithium-ion power battery has become one of the main power sources for electric vehicles and hybrid electric vehicles because of superior performance compared with other power sources. In order to ensure the safety and improve the performance, the maximum operating temperature and local temperature difference of batteries must be maintained in an appropriate range. The effect of temperature on the capacity fade and aging are simply investigated. The electrode structure, including electrode thickness, particle size and porosity, are analyzed. It is found that all of them have significant influences on the heat generation of battery. Details of various thermal management technologies, namely air based, phase change material based, heat pipe based and liquid based, are discussed and compared from the perspective of improving the external heat dissipation. The selection of different battery thermal management(BTM) technologies should be based on the cooling demand and applications, and liquid cooling is suggested being the most suitable method for large-scale battery pack charged/discharged at higher C-rate and in high-temperature environment. 17Dynamic expression of apoptosis-related genes during development of laboratory hepatocellular carcinoma and its relation to apoptosis显示文摘AIM:To explore the expression of p53,bcl-2,bax,survivin and the cell apoptosis during the development of tree shrew hepatocellular carcinoma(HCC),the relationship between expression of these genes,its impact on HCC development,and its relation to cell apoptosis.METHONS:Tree shrew HCC was induced with aflatoxin B1(AFB1),and regular biopsy of liver tissues was carried out and the biopsy tissues were collected during cancer inducement.Liver biopsy tisue and HCC tissue were collected from 35pre-cancerous experimental animats at wk 30 and 60 and at the 30th_,60th_,and 90th-wk,Liver biopsy tissues were collected from 13 blank control animals at wk 30,60,and 90.Expression of p53,bcl-2,bax,and survivin at each stage was examined by immunohistochemistry method.Apoptotic cells were detected in situ by the terminal deoxynucleotidyl transferase-mediated nick end labeling(tunel)technique.RESULTS:The apoptosis rate of normal hepatic cells was extremely low,whereas it increased during the formation of HCC.Expression of the apoptosis-related genes p53,bd-2,bax,and suvivin during the formation of HCC presented an increasing tendency.Expression of p53 did not noticeably relate to that of bcl-2,bax,and survivin,whereas expression of bcl-2 and bax was closely related.In HCC,p53 did not present a distinct relation to cell apoptosis,whereas its high level expression was probably related to liver cell proliferation.Survivin negetively correlated apoptosis index,and its overexpression could inhibit cell apoptosis.CONCLUSION:Apoptosis-related genes p53,bcl-2,bax,and survivin are all related to the occurrence of HCC.The anti-apoptosis effect of bcl-2 is influenced by bax,and ratio bcl/bax reflects more correctly the extent of cell apoptosis.Xiao-Xian Duan Jing-Sheng Ou Yuan Li Jian-Jia Su Chao Ou Chun Yang Hui-Fen Yue Ke-Chen Ban 2005World Journal of Gastroenterology2005,11,30:38
18Yield and Nicotine Content of Flue-Cured Tobacco as Affected by Soil Nitrogen Mineralization显示文摘Nitrogen(N)supply is the most important factor affecting yield and quality of flue-cured tobacco(FCT).A field experiment and an in situ incubation method were used to study the effects of soil N mineralization in the later stages of growth on yield and nicotine content of FCT in Fenggang and Jinsha,Guizhou Province.The yield and market value of FCT at Fenggang were much lower than those at Jinsha.However,the nicotine content of middle and upper leaves was much higher at Fenggang than at Jinsha when the same rate of fertilizer N was applied,which might be due to a higher N supply capacity at the Fenggang site.At later stages of growth(7-16 weeks after transplanting),the soil net N mineralization at Fenggang(56 kg N ha^(-1))was almost double that at Jinsha(30 kg N ha^(-1)).While soil NH_4-N and NO_3-N were almost exhausted by the plants or leached 5 weeks after transplanting,the N taken up at the later growth stages at Fenggang were mainly derived from soil N mineralization,which contributed to a high nicotine content in the upper leaves.The order of soil N contribution to N buildup in different leaves was:upper leaves>middle leaves>lower leaves.Thus,soil N mineralization at late growth stages was an important factor affecting N accumulation and therefore the nicotine content in the upper leaves.JU Xiao-Tang CHAO Feng-Chun LI Chun-Jian JIANG Rong-Feng P. 20De novo characterization of the root transcriptome of a traditional Chinese medicinal plant Polygonum cuspidatum显示文摘Various active components have been extracted from the root of Polygonum cuspidatum.However,the genetic basis for their activity is virtually unknown.In this study,25600002 short reads (2.3 Gb) of P.cuspidatum root transcriptome were obtained via Illumina HiSeq 2000 sequencing.A total of 86418 unigenes were assembled de novo and annotated.Twelve,18,60 and 54 unigenes were respectively mapped to the mevalonic acid (MVA),methyl-D-erythritol 4-phosphate (MEP),shikimate and resveratrol biosynthesis pathways,suggesting that they are involved in the biosynthesis of pharmaceutically important anthraquinone and resveratrol.Eighteen potential UDP-glycosyltransferase unigenes were identified as the candidates most likely to be involved in the biosynthesis of glycosides of secondary metabolites.Identification of relevant genes could be important in eventually increasing the yields of the medicinally useful constituents of the P.cuspidatum root.From the previously published transcriptome data of 19 non-model plant taxa,1127 shared orthologs were identified and characterized.This information will be very useful for future functional,phylogenetic and evolutionary studies of these plants.HAO DaCheng MA Pei MU Jun CHEN ShiLin XIAO PeiGen PENG Yong HUO Li XU LiJia SUN Chao 2012Science China(Life Sciences)2012,55,5:36
